Right Indirect Carotid-Cavernous Fistula Presenting With Isolated Oculomotor Palsy: A Diagnostic Challenge and Successful Endovascular Management.

Carotid-cavernous fistulas are abnormal communications between the carotid arterial system and the cavernous sinus. Indirect carotid-cavernous fistulas are low-flow lesions that may sometimes present subtly and mimic inflammatory or neoplastic cavernous sinus disease. We report the case of a 76-year-old man with diabetes and hypertension who developed painless diplopia and right-sided partial ptosis over two weeks. Clinical examination revealed right incomplete third nerve palsy with pupillary involvement. Magnetic resonance imaging showed enhancing soft tissue thickening in the right cavernou

Embolization of a brain arteriovenous malformation using combined transarterial and transvenous dual-lumen mini-balloon catheters.

For brain arteriovenous malformations (BAVMs), in which transarterial embolization (TAE) is not feasible, transvenous embolization (TVE) offers an alternative. In TVE, the reverse "pressure cooker" technique using coils or glue as a venous plug is necessary. A dual-lumen mini-balloon catheter can be used in place of the coils or glue as a venous plug, but this has rarely been reported. An 18-year-old male experienced rebleeding due to previous incomplete TAE for a ruptured BAVM. On admission, physical examination did not detect positive signs, but computed tomography revealed right frontal lob

Extracranial-Intracranial Bypass with Reconstruction Clip Surgery Following Failed Flow Diverter Therapy for a Giant Internal Carotid Aneurysm: A Case Report.

Flow diverter (FD) is often the first-line treatment for giant internal carotid artery aneurysms, with a high rate of aneurysm occlusion. However, up to 10% of giant cerebral aneurysms increase in size after FD treatment. Surgery is usually considered if the giant internal carotid artery aneurysm continues to enlarge and cause compression. We report a case of a giant internal carotid artery aneurysm that continued to increase in size after FD treatment and was subsequently treated surgically. We also review the literature on the management of giant cerebral aneurysms that increased in size aft

A new method for repairing internal carotid artery rupture in skull base surgery: a case report and review of the literature.

Internal carotid artery rupture during skull base surgery represents a rare yet catastrophic complication, with limited reports documented in the literature. In this article, we describe a case of ICA rupture encountered during supratentorial craniotomy for recurrent pituitary adenoma resection. The rupture was successfully managed through a novel microsurgical repair technique, achieving complete tumor resection. A 48-year-old female presented with a history of progressive visual impairment in both eyes for the past two years, 14 years after the initial pituitary adenoma resection. Reoperatio

Severe Pediatric Takayasu Arteritis Presenting With Cerebral Ischemia and Supra-Aortic Trunk Occlusion.

Takayasu arteritis (TAK) is a rare, chronic, autoimmune, and granulomatous vasculitis that predominantly affects large vessels, especially the aorta and its main branches, often leading to stenosis, occlusion, or aneurysm formation. Although TAK primarily affects young adult females, its occurrence in pediatric patients is uncommon and frequently associated with a more aggressive clinical course. We report a severe neurovascular presentation of TAK in a 16-year-old Mexican female patient who developed sudden-onset right hemiparesis, global aphasia, and multiple syncopal episodes. Marked interl

Evaluation of an Antithrombotic Surface-Coated Flow Diverter in a Rabbit Model without Dual Antiplatelet Drugs.

OBJECTIVE: Flow diverters (FDs) carry the risk of thromboembolic complications associated with the device and bleeding complications associated with dual antiplatelet therapy. We hypothesize that an antithrombotic surface-coated FD (ASCFD) would have less acute thrombus formation and better endothelialization on the device surface compared with uncoated FD. METHODS: A ASCFD was developed. Acute clot formation and chronic endothelialization over the device were assessed in 8 rabbit models compared with its prototype FD (PFD) at 2 hours and 1 month by scanning electron microscopy (SEM) and hist

Predicting postinterventional rupture of intracranial aneurysms using arteriography-derived radiomic features after pipeline embolization.

BACKGROUND AND PURPOSE: Postinterventional rupture of intracranial aneurysms (IAs) remains a severe complication after flow diverter treatment. However, potential hemodynamic mechanisms underlying independent predictors for postinterventional rupture of IAs remain unclear. In this study, we employed arteriography-derived radiomic features to predict this complication. METHODS: We included 64 patients who underwent pipeline flow diversion for intracranial aneurysms, distinguishing between 16 patients who experienced postinterventional rupture and 48 who did not. We performed propensity score m

Delayed traumatic intracranial aneurysms: literature review and case series.

Traumatic intracranial aneurysm (TICA) is a rare and aggressive pathology that requires prompt treatment. Nevertheless, early vascular imaging following head trauma may yield falsely negative results, underscoring the importance of subsequent imaging within the first week to detect delayed TICAs. This study aims to report our experience with delayed TICAs and highlight the clinical importance of repeated angiographic screening for delayed TICAs. In this retrospective analysis, we evaluated patients managed for a TICA at a tertiary care teaching institution over the last decade. Additionally, w

Patient-Specific Vascular Flow Phantom for MRI- and Doppler Ultrasound Imaging.

OBJECTIVE: Intraoperative Doppler ultrasound imaging of human brain vasculature is an emerging neuro-imaging modality that offers vascular brain mapping with unprecedented spatiotemporal resolution. At present, however, access to the human brain using Doppler Ultrasound is only possible in this intraoperative context, posing a significant challenge for validation of imaging techniques. This challenge necessitates the development of realistic flow phantoms outside of the neurosurgical operating room as external platforms for testing hardware and software. An ideal ultrasound flow phantom should

Evaluation of single-delay arterial spin labeling-based spatial coefficient of variation and histogram-based parameters in relation to cerebrovascular reserve in patients with Moyamoya disease.

INTRODUCTION: Single-delay Arterial Spin Labeling (ASL)-based spatial coefficient of variation (CoVCBF) has been suggested as a measure of hemodynamic disturbance in patients with cerebrovascular diseases. However, spatial CoVCBF and other histogram-based parameters such as skewness and kurtosis and the volume of the arterial transit time artefact (ATAvol), has not been evaluated in patients with MMD nor against cerebrovascular reserve (CVR). The aim of this study was to assess whether any associations between spatial CoVCBF, skewness, kurtosis, and ATAvol are present and to analyze any potent

SUCCESSFUL RESOLUTION OF NUTCRACKER SYNDROME WITH 3D PRINTED PEEK EXTRAVASCULAR STENT IN AN ADOLESCENT BOY.

Three-dimensional printed polyetheretherketone (PEEK) extravascular stent was applied to treat a 14-year-old boy with nutcracker syndrome. Digital subtraction angiography revealed a segment of the left renal vein (LRV) with reduced contrast filling immediately before its inflow into the inferior vena cava, and high-pressure gradient. The three-dimensional reconstruction model demonstrated that the LRV and the duodenum were contracted at the aortomesenteric angle, resulting in LRV compression from the abnormal high-level duodenal compartment. When duodenum courses between the abdominal aorta an
