Validating electrical stimulation through the somatosensory system: a perspective on mismatch negativity and auditory prostheses.

Natural touch engages specialized mechanoreceptors, while electrotactile stimulation bypasses them, directly depolarizing a mixed and non-uniform population of afferent axons. Whether this difference in peripheral encoding affects the brain predictive processing remains an open empirical question. The mismatch negativity (MMN), an established electroencephalography (EEG) marker of automatic change detection, provides a powerful tool to address this question without perceptual confounds. Somatosensory studies comparing MMN responses to natural versus electrical stimulation within the same healt

Utility of hearing aid use on neurocognitive functions and auditory processing: A systematic review.

PURPOSE: Hearing loss not only affects the peripheral auditory system but also leads to changes in central auditory pathways and neurocognitive skills. The use of hearing aids has shown potential benefits in mitigating the adverse effects of hearing loss. Earlier literature showed various studies assessing the impact of hearing aid use on cognitive functions and auditory processing, a comprehensive review of the existing literature is warranted. Existing literature showed heterogeneous outcomes among studies investigated the effect of Hearing aid use in combating the adverse effects of hearing

Correlation of Promontory Vibration and Sound Emission Recorded from the Skin Surface in Bone Conduction Stimulation.

PURPOSE: Sound pressure recordings at different positions at the head such as ear canal, nasal cavity, or on the skin are effective tools to verify, fit, or follow up the output of bone conduction devices (BCDs) intra- and post-operatively. Here, we investigated the possibility of using a surface microphone (SM) as a non-invasive alternative to laser Doppler vibrometry (LDV) measuring cochlear promontory (CP) vibrations in human heads. METHODS: A percutaneous BCD (Ponto system) was implanted at the standard position in five human (four males/one female) cadaver heads (ten ears). CP vibration

Newborn screening for common genetic variants associated with permanent hearing loss: Implementation in Ontario and review of the first 3 years.

PURPOSE: Early hearing detection and intervention (EHDI) programs using audiometric screening techniques alone have a limited ability to detect noncongenital childhood permanent hearing loss (PHL). In 2019, Ontario launched universal newborn screening (NBS) for PHL risk factors, including congenital cytomegalovirus and 22 common variants in GJB2 and SLC26A4. Here, we describe our experience in screening for genetic risk factors. METHODS: Ontario newborns who participated in universal newborn hearing screening (UNHS) were offered risk factor screening using dried blood spots (DBS) collected fo

Mesenchymal stem cell- derived exosomes as cell-free therapeutics for sensorineural hearing loss.

Sensorineural hearing loss (SNHL) can result from various factors, including ototoxic drugs (such as aminoglycosides and chemotherapeutic agents), prolonged exposure to intense sound, and autoimmune or genetic disorders. In adult mammals, the loss of sensory cells in the cochlea is irreversible due to their lack of regenerative capacity. Current treatment options include hearing aids for mild to moderate hearing loss, which rely on residual hearing, and cochlear implants for severe cases, which provide limited auditory recovery while leading to the loss of any remaining natural hearing. Stem c

Induction of cell death by sodium hexachloroplatinate (IV) in the HEI-OC1 cell line, primary rat spiral ganglion cells and rat organ of Corti explants.

Although cochlear implants have become a well-established method for patients with sensory neural hearing loss, clinical results indicate that in some cases, corrosion of electrode contacts leads to high impedance that interferes with successful stimulation of the auditory nerve. As it is unclear whether corrosion products induce cell damage, we focused on cell culture models of the organ of Corti cell line (HEI-OC1), rat spiral ganglion cells (SGC) and rat organ of Corti explant (OCex) cultivated from neonatal rat cochleae to characterize the cytotoxicity of sodium hexachloroplatinate (IV) (N

Role of Otolaryngologists in the Treatment of Patients With Riboflavin Transporter Deficiency: A Case Report.

Riboflavin transporter deficiency (RTD) is a rare genetic disorder that can have detrimental effects on the nervous system, causing progressive neurodegeneration. Here, we report the second case of RTD in Saudi Arabia. An 18-month-old boy presented to the otolaryngology clinic with six weeks history of progressive noisy breathing associated with drooling, choking, and difficulty in swallowing. Progressive regression of the child's motor and communicative abilities was reported as well. Upon examination, the child had biphasic stridor, chest retractions, bilateral facial palsy, and hypotonia. T

Evaluation of the Relationship Between Facial Nerve and Lateral Semicircular Canal in the Posterior Tympanotomy Approach.

BACKGROUND: To clinically and radiologically evaluate the relationship between the facial nerve and the lateral semicircular canal during posterior tympanotomy. METHODS: Patients who received cochlear implants between 2010 and 2020 were included in the study. The relationship between the facial nerve and the lateral semicircular canal was classified into 3 types by evaluating the axial section computed tomography images. If the facial nerve passed medially without contacting the lateral semicircular canal dome, it was classified as type 1; if the facial nerve passed by contacting the medial b

The genetic and phenotypic landscapes of Usher syndrome: from disease mechanisms to a new classification.

Usher syndrome (USH) is the most common cause of deaf-blindness in humans, with a prevalence of about 1/10,000 (~ 400,000 people worldwide). Cochlear implants are currently used to reduce the burden of hearing loss in severe-to-profoundly deaf patients, but many promising treatments including gene, cell, and drug therapies to restore the native function of the inner ear and retinal sensory cells are under investigation. The traditional clinical classification of Usher syndrome defines three major subtypes-USH1, 2 and 3-according to hearing loss severity and onset, the presence or absence of ve

The Effects of Middle-ear Stiffness on the Auditory Brainstem Neural Encoding of Phase.

The middle-ear system relies on a balance of mass and stiffness characteristics for transmitting sound from the external environment to the cochlea and auditory neural pathway. Phase is one aspect of sound that, when transmitted and encoded by both ears, contributes to binaural cue sensitivity and spatial hearing. The study aims were (i) to investigate the effects of middle-ear stiffness on the auditory brainstem neural encoding of phase in human adults with normal pure-tone thresholds and (ii) to investigate the relationships between middle-ear stiffness-induced changes in wideband acoustic i

Disease mechanisms and gene therapy for Usher syndrome.

Usher syndrome (USH) is a major cause of deaf-blindness in humans, affecting ∼400 000 patients worldwide. Three clinical subtypes, USH1-3, have been defined, with 10 USH genes identified so far. In recent years, in addition to identification of new Usher genes and diagnostic tools, major progress has been made in understanding the role of Usher proteins and how they cooperate through interaction networks to ensure proper development, architecture and function of the stereociliary bundle at the apex of sensory hair cells in the inner ear. Several Usher mouse models of known human Usher genes ha

Adeno-associated virus gene replacement for recessive inner ear dysfunction: Progress and challenges.

Approximately 3 in 1000 children in the US under 4 years of age are affected by hearing loss. Currently, cochlear implants represent the only line of treatment for patients with severe to profound hearing loss, and there are no targeted drug or biological based therapies available. Gene replacement is a promising therapeutic approach for hereditary hearing loss, where viral vectors are used to deliver functional cDNA to "replace" defective genes in dysfunctional cells in the inner ear. Proof-of-concept studies have successfully used this approach to improve auditory function in mouse models of

Aminoglycoside-associated nonsyndromic deafness and speech disorder in mitochondrial A1555G mutation in a family: A case report.

RATIONALE: Mitochondrial DNA mutations have been associated with many maternal inherited diseases. A1555G mutation in mtDNA effects the gene code for rRNA, resulting in the structural change of human ribosome rending it susceptible to binding of the common antibiotic, aminoglycosides. Such mutation has linked with non-syndromic hearing loss and is one of the most common mtDNA mutations in Asian populations. PATIENT CONCERNS: A 50-year-old Taiwanese female visited our neurology department with concern for multiple members with hearing loss in her family, including herself. DIAGNOSES: Physical
