Use of a relative motion flexion orthosis for postoperative management of zone I/II flexor digitorum profundus repair: A retrospective consecutive case series.

STUDY DESIGN: A retrospective, single-center, consecutive case series. INTRODUCTION: In concept, a relative motion flexion (RMF) orthosis will induce a "quadriga effect" on a given flexor digitorum profundus (FDP) tendon, limiting its excursion and force of flexion while still permitting a wide range of finger motion. This effect can be exploited in the rehabilitation of zone I and II FDP repairs. PURPOSE OF THE STUDY: To describe the use of RMF orthoses to manage zone I and II FDP 4-strand repairs. METHODS: Medical record review of 10 consecutive zone I and II FDP tendon repairs managed wi

A multifactorial conceptual model of peripheral neuromusculoskeletal predisposing factors in task-specific focal hand dystonia in musicians: etiologic and therapeutic implications.

A model is presented showing how peripheral factors may cause a process of movement adaptation that leads to task-specific focal hand dystonia in musicians (FHDM). To acquire a playing technique, the hand must find effective and physiologically sustainable movements within a complex set of functional demands and anatomic, ergonomic, and physiological constraints. In doing so, individually discriminating constraints may become effective, such as limited anatomic independence of finger muscles/tendons, limited joint ranges of motion, or (subclinical) neuromusculoskeletal defects. These factors m

New "loop" suture for FDP zone I injuries.

INTRODUCTION: There are many methods describing distal juncture fixation techniques of the injured flexor digitorum profundus (FDP) or flexor tendon graft. We have noted a tendency in the surgical repair of tissues toward the more expensive and technology-dependent methods. New suture type was developed to find better FDP distal juncture suture not indulging into expensive technology-dependent techniques. The positive aspect of the most popular Bunnell pullout technique is eventual removal of the suture. The negative aspect is necessity to use tie over button to secure the end of the tendon in

Nondominant arm advantages in load compensation during rapid elbow joint movements.

This study was designed to examine interlimb asymmetries in responding to unpredictable changes in inertial loads, which have implications for our understanding of the neural mechanisms underlying handedness. Subjects made repetitive single joint speed constrained 20 degrees elbow flexion movements, while the arm was supported on a horizontal, frictionless, air-jet system. On random trials, a 2-kg mass was attached to the arm splint prior to the "go" signal. Subjects were not given explicit information about the mass prior to movement nor were they able to view their limb or the mass. Accordin

Cerebellar subjects show impaired coupling of reach and grasp movements.

We examined how cerebellar deficits in isolated reaching or grasping movements contribute to abnormalities in a combined reach and grasp movement, and whether people with cerebellar damage show abnormalities in the spatiotemporal relationships of reach and grasp movements. We studied subjects with cerebellar damage and matched controls as they performed a combined reach and grasp, an isolated reach, and an isolated grasp. These movements were performed under slow-accurate and fast speed conditions. Subjects were also tested for their ability to correctly estimate the target size based on visua

[The hand-arm vibration syndrome: (I) the clinical picture, exposure-response relationship and exposure limits].

Part I of this paper presents an overview of the medical aspects of the hand-arm vibration syndrome, as well as the relationship between occupational exposure to hand-transmitted vibration and the vascular, neurological, and musculoskeletal disorders occurring in the upper limbs of workers who use vibrating tools. There is epidemiologic evidence for an increased occurrence of peripheral sensorineural and vascular disorders in occupational groups using a great variety of vibrating tools. An excess risk for wrist osteoarthrosis and elbow arthrosis and osteophytosis has been reported in workers e

Clinical characteristics in male and female uro-arthritis or Reiter's syndrome.

To investigate the clinical characteristics of Reiter's syndrome (RS) or uro-arthritis in females, 73 consecutive patients were studied. The findings were compared to those in 72 consecutive males with RS. The mean ages and the clinical picture of musculoskeletal findings, including frequency of mono- or oligoarthritis (68-69%), polyarthritis (26%) and back complaints (76-71%), were similar in both sexes. Inflammation of the knee, sterno-clavicular joint and "sausage" toe occurred predominantly in males and finger involvements in females. Anamnestic gonorrhoea occurred in 17 males (24%) and in

Reconstruction of complex hand defects using trapezoidal osteocutaneous free fibular flap.

BACKGROUND: Traumatic defects of multiple metacarpal bones can be addressed using conventional or vascularized bone grafts. When associated with extensive skin and tendon loss, the treatment becomes more challenging. The aim of the study was to describe the results of using free osteocutaneous fibular flap placed in a new fashion for the reconstruction of complex hand defects. PATIENTS AND METHODS: Six patients with complex hand defects underwent reconstruction using free osteocutaneous fibular flap placed in trapezoidal fashion with two-stage tendon reconstruction using fascia lata graft. Use of the Swanson silicone trapezium implant for treatment of primary osteoarthritis : long-term results.

BACKGROUND: Instability of the prosthesis and silicone-induced synovitis have led most surgeons to abandon use of the Swanson trapezium implant for the treatment of primary osteoarthritis. However, the literature contains little information on the results of long-term follow-up. The present study was conducted to establish the long-term results and to highlight the problems associated with the implant. METHODS: Thirty-five patients (forty-five implants) of our initial forty-five patients (fifty-seven implants) were available for clinical review. The mean duration of follow-up was 13.8 years.

The usefulness of computer-aided joint space analysis in the assessment of rheumatoid arthritis.

OBJECTIVE: Computer-aided joint space analysis (CAJSA) is a newly developed technique for the measurement of radiogeometrically detectable joint space widths of the metacarpal-phalangeal (JSD-MCP) and proximal-interphalangeal articulations (JSD-PIP). The aim of this study was to verify the sensitivity and specificity of these CAJSA measurements in the assessment of established RA. METHODS: Four hundred and fifty-eight participants (248 healthy subjects, 210 RA patients) underwent computerized semi-automated measurements of the JSD-MCP and JSD-PIP articulations (CAJSA, Radiogrammetry Kit, Vers

[Giant cell tumor of the thumb proximal phalanx: resection-iliac graft and double arthrodesis].

Giant cell tumours (GCT) of bone are frequent, with variable behaviour, high risk of recurrence and an often benign histological appearance. Their location in the hand is uncommon, and there are few publications on the subject. Surgical treatment as described in the literature is very variable. We report a case of recurrent grade II GCT, arising in the proximal phalanx of the thumb, aggressive and causing a pathological fracture. After a surgical biopsy, we performed an "en bloc" resection of the first phalanx, with an iliac crest graft reconstruction and a double arthrodesis of the metacarpop

Assessment of the kinetic trajectory of the median nerve in the wrist by high-frequency ultrasound.

Carpal tunnel syndrome (CTS) is typically diagnosed by physical examination or nerve conduction measurements. With these diagnostics however it is difficult to obtain anatomical information in the carpal tunnel. To further improve the diagnosis of CTS, an attempt using 30 MHz high-frequency ultrasound to noninvasively detect the local anatomical structures and the kinetic trajectory of the median nerve (MN) in the wrist was explored. Measurements were performed on the right wrist of 14 asymptomatic volunteers. The kinetic trajectory of the MN corresponding to flexion (from 0° to 90°) and exten

Psoriatic arthritis: imaging techniques.

Imaging techniques to assess psoriatic arthritis (PsA) include radiography, ultrasonography (US), magnetic resonance imaging (MRI), computed tomography (CT) and bone scintigraphy. The radiographic hallmark of PsA is the combination of destructive changes (joint erosions, tuft resorption, osteolysis) with bone proliferation (including periarticular and shaft periostitis, ankylosis, spur formation and non-marginal syndesmophytes). US has an increasing important role in the evaluation of PsA. In fact, power Doppler US is useful mainly for its ability to assess musculoskeletal (joints, tendons, en
