Exchange nailing for aseptic nonunion of femoral shaft: a retrospective cohort study for effect of reaming size.

BACKGROUND: Exchange nailing has been the favored method to treat aseptic nonunions of the femoral shaft. The recommended amount of over-reaming is at least 2 mm. The purpose of this study was to determine whether the effectiveness of nonunion treatment is greatly reduced with less than 2 mm of over-reaming. METHODS: Aseptic nonunions of the femoral shaft were treated by exchange nailing with 1 mm (n = 37) or at least 2 mm (n = 44) of over-reaming. Union rates, union periods, and complications were compared. Indications for exchange nailing were aseptic nonunions of the femoral shaft with an

Compartment pressures during nonreamed tibial nailing without traction.

OBJECTIVE: To assess the intracompartmental pressure changes during the nailing of acute tibia fractures with the extrinsic factors of 90 degrees/90 degrees positioning, posterior thigh posts, continuous traction, and remaining removed. STUDY DESIGN: Prospective case control. METHODS: Fifty-eight acute tibia fractures were nailed using an unreamed technique without leg elevation, thigh post, or continuous traction. Two presented with compartment syndrome and had fasciotomy before nailing. Thirty of the remaining fifty-six tibias had continuous intracompartmental pressure monitoring of the an

Negative impact of medical ozone therapy on femur fracture healing in a rat model.

INTRODUCTION: The purpose of the study is to evaluate the effect of medical ozone therapy on fracture healing in rats. MATERIALS AND METHODS: 20 male Wistar-Albino rats were randomly divided into two groups as Control Group (n = 10) and Ozone Group (n = 10). The fracture model was created by bilateral femur transverse osteotomy and fixation with an intramedullary Kirschner wire. No medical treatment was applied to the control group, whereas Ozone gas at a dose of 1 cc/kg at a concentration of 20 µg/ml was administered intraperitoneally to the ozone group for 8 weeks. All groups were sacrifice

Efficacy of Infection Eradication in Antibiotic Cement-Coated Intramedullary Nails for Fracture-Related Infections, Nonunions, and Fusions.

Antibiotic cement-coated intramedullary nails (ACCINs) are increasing in popularity as a viable solution for the treatment of fracture-related infections (FRIs), infected long bone nonunions, and arthrodeses without an external fixator. ACCINs effectively manage to fulfill three of the basic principles for eradicating osteomyelitis: dead space management, antibiotic delivery, and bone stability. We performed a retrospective review of 111 patients who were treated with ACCINs between January 2014 and December 2020. In our series, 87.4% (n = 97) of patients achieved healed and uninfected bone or

Micromotion and Push-Out Evaluation of an Additive Manufactured Implant for Above-the-Knee Amputees.

In comparison to through-knee amputees the outcomes for above-the-knee amputees are relatively poor; based on this novel techniques have been developed. Most current percutaneous implant-based solutions for transfemoral amputees make use of high stiffness intramedullary rods for skeletal fixation, which can have risks including infection, femoral fractures, and bone resorption due to stress shielding. This work details the cadaveric testing of a short, cortical bone stiffness-matched subcutaneous implant, produced using additive manufacture, to determine bone implant micromotion and push-out l

Fibrous dysplasia of bone and the Weil-Albright syndrome. A study of thirteen cases with special reference to the orthopaedic treatment.

Four patients with monostotic and nine with polyostotic fibrous dysplasia of bone, including one with the classical Weil-Albright syndrome and a male child who may have had this syndrome, are presented. While monostotic forms affect adults and are amenable to curative surgery, polyostotic involvement may become evident in childhood and early infancy, and represent a difficult therapeutic problem. Screening of the skeleton is essential for proper diagnosis and treatment, and should be achieved by total body scan in preference to a complete radiological survey. Biopsy is of particular importance

Clinical compatibility of magnetic resonance imaging with magnetic intramedullary nails: a feasibility study.

INTRODUCTION: The use of magnetic resonance imaging (MRI) with a magnetic intramedullary lengthening nail in place is contraindicated per the manufacturer due to the concern of implant activation and migration. A prior in vitro study did not confirm these complications only noting that a 3.0 T MRI weakened the internal magnet. Therefore, a retrospective analysis of patients who underwent an MRI with a magnetic nail in place was performed to determine if any adverse effects occurred in the clinical setting. MATERIALS AND METHODS: A retrospective review of all patients who underwent an MRI with

Retrograde Tibial Nail and Trabecular Titanium Spacer Block for the Treatment of Missing Talus: A Rare Case Report.

Astragalus enucleation with complete bone loss is a rare pathology, and there is no current classification or treatment protocol. A 34-year-old woman sustained an open total enucleation of the talus without bone recovery in a motorcycle accident. Initially, she was treated at the emergency department with wound cleaning, surgical debridement, and external fixation with intravenous antibiotic therapy. Definitive treatment was delayed 8 weeks because of acute infection, which was treated with lavage, surgical debridement, and specific antibiotic therapy. To avoid leg-length discrepancy, we perfo

Alar Pinning in Rigid External Distraction for Midfacial Hypoplasia.

BACKGROUND: Distraction osteogenesis with a rigid external distractor is a widely accepted treatment for midfacial hypoplasia. In this study, the authors introduce the utilization of alar pinning with the external halo distractor for maxillary advancement, in place of an oral splint. METHODS: A retrospective chart review was conducted of 7 patients who successfully underwent distraction osteogenesis using the alar pinning technique. Midfacial hypoplasia was secondary to Crouzon syndrome (n = 4), Apert syndrome (n = 1), Pfeiffer syndrome (n = 1), or bacterial meningitis (n = 1). Three patients

Intramedullary rodding in type III osteogenesis imperfecta. Effects on neuromotor development in 10 children.

We studied retrospectively gross motor development and the impact of intramedullary rodding in 10 children with type III osteogenesis imperfecta (OI). There was a pronounced delay in motor development and the order in achieving gross motor milestones differed from the normal developmental sequence. Static milestones developed at an earlier stage than dynamic milestones. Intramedullary rodding of the lower extremities prior to the age of 3.5 years enhanced neuromotor development, especially regarding the milestones supported standing, rolling from prone to supine and crawling with abdomen on th

Elongating intramedullary rods in the treatment of osteogenesis imperfecta.

Patients with osteogenesis imperfecta who have undergone multiple osteotomies with realignment and intramedullary rod fixation of a deformed bone frequently require replacement of the rod because the bone grows and angulates when the rod no longer is long enough to support the bone from metaphysis to metaphysis. The Bailey-Dubow intramedullary rod, which elongates with growth, is an attempt to solve this problem. This is a review of 153 rod-fixations in seventy-two bones (twenty patients), forty-seven being Bailey-Dubow rods. Use of the elongating rod effectively increased the average length o

Quantification of TGF-ß1, PDGF and IGF-1 cytokine expression after fracture treatment vs. non-union therapy via masquelet.

INTRODUCTION: Biochemical processes during bone regeneration can be analysed via quantification of peripheral serum cytokine levels. To date, serum levels of cytokines in patients treated with masquelet technique and patients with normal bone healing have not been compared. This comparison is supposed to deliver novel insights into the process of bone regeneration. Our aim was to validate this established method in the monitoring of bone regeneration after non-union treatment in masquelet technique. MATERIALS AND METHODS: Between 04/2008 and 01/2014 three groups were recruited: G1 (10 patient

A Giant Cell Tumor of the Distal Femur Managed by Excision and Knee Arthrodesis Using a Custom Made Long Intramedullary Interlocking Nail: A Case Report and Review of the Literature.

Giant cell tumors (GCTs) are primary bone tumors that occur most commonly in long bones, with half such tumors occurring in the distal femur, proximal tibia, and fibula. Around 12% of patients present with a pathological fracture indicating more aggressive disease. Arthrodesis after tumor resection is a popular choice due to its affordability and early postoperative mobilization, as well as low risks of implant loosening, infections, malignant lesions, or mortality. A free fibular graft is a popular option in limb-sparing surgery for long bone tumors. A bone graft and nail can be used to recon

Impact of Combining Alfacalcidol With Proximal Femoral Nail Antirotation on Bone Mineral Density, Serum Bone Metabolites, and Inflammatory Markers in Elderly Patients With Osteoporotic Intertrochanteric Fractures.

AIM: Proximal femoral nail antirotation (PFNA) fixation remains an effective surgical method in effectively managing intertrochanteric fractures in elderly patients with osteoporosis. While postoperative anti-osteoporotic therapy is essential, only a part of the elderly patients adhere to anti-osteoporosis treatment. Therefore, this study aims to investigate the therapeutic efficacy of combining alfacalcidol (an anti-osteoporotic drug) with PFNA fixation, as well as their effects on serum bone metabolic markers and inflammatory indicators in elderly patients with osteoporotic intertrochanteric

Shortening of the distal ulna to correct ulnar variance in osteoporotic wrist fractures treated with intra-focal pining.

The aim of this study is to correct the ulnar variance in severe osteoporotic wrist fracture treated with intra-focal pining of the distal radius, with shortening osteotomy of the distal ulna. Thirteen patients, 12 females and one male, 61-83 years old, 6-year follow-up, were treated by intra-focal pining (kapandji). When preoperatively the ulnar variance did not return to a normal value comparing to the opposite wrist, a shortening osteotomy of the ulna was performed in one stage, 4 cm proximal to the ulnar head. The fractures healed 6-8 weeks with stability and normal motion: flexion 70°, ex
