Carotid endarterectomy for postradiation carotid stenosis in patient with hypopharyngeal cancer: a case report.

BACKGROUND: Radiation therapy for head and neck tumors is associated with radiation-induced carotid injury, which increases the risk of carotid artery stenosis and cerebrovascular events. Despite the well-established risks, routine screening for these complications has not been incorporated into clinical practice. This case emphasizes that, despite severe fibrosis and adhesion in the neck tissues resulting from radiation therapy, a well-structured surgical evaluation can still lead to favorable outcomes. It highlights the importance of timely surgical intervention in such cases. CASE PRESENTA

Suitability for Transcarotid Transcatheter Aortic Valve Replacement in the Japanese Population.

BACKGROUND: Transcarotid (TC) transcatheter aortic valve replacement (TAVR) has shown favorable outcomes in selected Western populations; however, data on its suitability in patients undergoing TAVR remain scarce. OBJECTIVES: The purpose of this study was to investigate the anatomical characteristics of the common carotid artery (CCA) and the suitability of Japanese patients for TC-TAVR. METHODS: This single-center retrospective study included consecutive patients who underwent TAVR between April 2023 and March 2024. Patients were categorized into a tough-transfemoral (TF) group, who may req

Lung Cancer Screening: Beyond Pulmonary Nodules.

Low-dose CT lung cancer screening reduces lung cancer mortality in individuals at high risk and frequently reveals clinically relevant incidental findings. In the National Lung Screening Trial, approximately one-third of screening CT examinations demonstrated abnormalities other than lung cancer, raising challenges related to interpretation, reporting, and further management. The absence of consistent guidance and the variable clinical significance of these findings may hinder the broader adoption of lung cancer screening. Opportunistic screening leverages low-dose CT examinations performed fo

Artery-to-fistula diameter ratio as a predictor of target lesion primary patency in the juxta-anastomotic segment after percutaneous transluminal angioplasty.

BACKGROUND: Percutaneous Transluminal Angioplasty (PTA) is a frequently performed intervention for repairing dysfunctional arteriovenous fistula (AVF). However, there is few data on the predictive factors for target lesion primary patency (TLPP), especially in the juxta-anastomotic segment. Our study aims to explore the relationship between the artery-to-fistula diameter ratio and TLPP in patients with mature radiocephalic AVF after PTA. METHODS: We conducted a retrospective analysis of 104 CKD patients who underwent PTA due to recurrent stenosis that only affected the juxta-anastomotic area

Giant ectopic thyroid mass: a case report and review of the literature.

BACKGROUND: Giant mediastinal ectopic thyroid tissue is a rare entity that presents a diagnostic and therapeutic challenge for clinicians investigating mediastinal masses. Most cases are asymptomatic and incidentally detected on imaging. When symptomatic, presentations are typically influenced by the mass's size and anatomical location. In this report, we present a right hemithoracic benign follicular goiter in an asymptomatic patient that was removed through a right posterolateral thoracotomy. CASE DESCRIPTION: A 39-year-old male is discovered to have an incidental mediastinal mass on a ches

Treatment extent of femoropopliteal disease and clinical outcomes following endovascular therapy.

BACKGROUND: Endovascular therapy (EVT) has become the preferred treatment modality for femoropopliteal disease. However, there is limited evidence regarding its procedural and clinical outcomes according to the affected area. AIMS: The aim of this study is to investigate clinical outcomes and device effectiveness according to treatment extent in the superficial femoral artery (SFA), popliteal artery (PA), or both. METHODS: In this study, we analysed EVT for SFA (2,404 limbs), PA (155 limbs), SFA/PA (383 limbs) using the population in the K-VIS ELLA (Korean Vascular Intervention Society Endov

A Rare Case of Low-Grade Endometrial Stromal Sarcoma Invading an Old Leiomyoma.

We present the first case of low-grade endometrial stromal sarcoma (ESS) invading a leiomyoma, which was difficult to diagnose preoperatively. A 49-year-old multiparous woman was referred to our institution due to the enlargement of an old leiomyoma after menopause. Transvaginal ultrasonography revealed a 40-mm lesion in the myometrium of the uterine body with calcification and edema. Color Doppler imaging showed blood flow along the margins of the mass. Pelvic contrast-enhanced magnetic resonance imaging (MRI) revealed a 39-mm mass in the uterine body, predominantly having a low intensity on

Application of fluorescein sodium videoangiography in aneurysm surgery.

BACKGROUND: The use of fluorescent technologies in vascular neurosurgery emerged after indocyanine green video angiography (ICG-VA) was first described in 2003. As data supporting the efficiency of ICG in preventing postoperative complications has grown substantially, it has now established itself as the standard of care. However, the predominant literature centers on ICG techniques, leaving the evaluation of cost-effective fluorescein tools pending. We report the results of a prospective study in which we demonstrated the impact of intraoperative fluorescein videoangiography (FL-VA) in aneury

Cardiovascular complications in chronic kidney disease: a review from the European Renal and Cardiovascular Medicine Working Group of the European Renal Association.

Chronic kidney disease (CKD) is classified into five stages with kidney failure being the most severe stage (stage G5). CKD conveys a high risk for coronary artery disease, heart failure, arrhythmias, and sudden cardiac death. Cardiovascular complications are the most common causes of death in patients with kidney failure (stage G5) who are maintained on regular dialysis treatment. Because of the high death rate attributable to cardiovascular (CV) disease, most patients with progressive CKD die before reaching kidney failure. Classical risk factors implicated in CV disease are involved in the

ADAMTS-7 deficiency attenuates thoracic aortic aneurysm and dissection in mice.

Thoracic aortic aneurysm and dissection (TAAD) is a life-threatening cardiovascular disease with severe extracellular matrix (ECM) remodeling that lacks efficient early stage diagnosis and nonsurgical therapy. A disintegrin and metalloproteinase with thrombospondin motif 7 (ADAMTS-7) is recognized as a novel locus for human coronary artery atherosclerosis. Previous work by us and others showed that ADAMTS-7 promoted atherosclerosis, postinjury neointima formation, and vascular calcification. However, whether ADAMTS-7 is involved in TAAD pathogenesis is unknown. We aimed to explore the alterati…
