Effect of reaming bone grafting on treating femoral shaft aseptic nonunion after plating.

Rigid intramdullary nailing with cancellous bone grafting provided by intramedullary reaming was prospectively used to treat femoral shaft aseptic nonunions after plating. Indications for this technique were a femoral shaft nonunion with an inserted plate, no previous infection sign in the treatment course, less than 1.5 cm shortening, and no segmental bony defects. After the plate was removed, a flexible guidewire was inserted antegradely. The local wound was closed, and intramedullary reaming was done as widely as possible until some resistance to it occurred. Finally, a rigid intramedullary

CT-Based Evaluation of Bone Mineral Density Distribution of Proximal Femur in Patients With Femoral Trochanteric Fracture.

BACKGROUND: Surgical treatment of proximal femoral fractures typically involves fixation with intramedullary nailing or sliding hip screws, where screws inserted into the femoral head stabilize the fracture site. However, few studies have quantitatively assessed the distribution of bone density and quality within the femoral head. We investigated the distribution of bone mineral density (BMD) within the proximal femoral head, evaluated inter-patient variability, and examined associated factors based on computed tomography (CT) values. METHODS: This multicenter prospective observational study

Insertion sites of the muscles attached to the clavicle: a cadaveric study of the clavicle.

BACKGROUND: Clavicle fractures are common injuries, especially in young, active individuals. Operative treatment is recommended for completely displaced clavicle shaft fractures, and plate fixation is stronger than the use of intramedullary nails. Few studies have reported on iatrogenic injuries to the muscle attached to the clavicle during fracture surgery. The aim of this study was to clarify the area of the insertion sites of muscles attached to the clavicle in Japanese cadavers using gross anatomy and three-dimensional (3D) analysis. We also aimed to compare the effects of anterior plate t

Thompson's quadricepsplasty for stiff knee.

BACKGROUND: Stiffness of the knee after trauma and/or surgery for femoral fractures is one of the most common complications and is difficult to treat. Stiffness in extension is more common and can be reduced by vigorous physiotherapy. If it does not improve then quadricepsplasty is indicated. The present study was undertaken to evaluate the results of Thompsons quadricepsplasty. MATERIALS AND METHODS: Twenty-two male patients (age range 20-45 years) with posttraumatic knee stiffness following distal femoral fractures underwent Thompson's quadricepsplasty where knee flexion range was less than

Treatment of segmental defects of the humerus with an osteoseptocutaneous fibular transplant.

BACKGROUND: There are limited reconstructive options for the treatment of segmental bone defects of the upper extremity that are >6 cm in length, especially those that are associated with soft-tissue defects. The purpose of the present review was to report on our experience with fifteen patients who received an osteoseptocutaneous fibular transplant for reconstruction of a humeral defect. METHODS: The study cohort included eight male patients and seven female patients with an average age of forty-one years. The indications for the procedure included segmental nonunion (nine patients), a gunsh

Maladaptive immune-fibrotic axis drives impaired long bone regeneration under mechanical instability.

Delayed and nonhealing fractures, affecting 5 to 10% of cases, are associated with prolonged disability and diminished quality of life. Although acute inflammation is required to initiate repair, persistent mechanical instability can sustain maladaptive immune and fibrotic responses that impede regeneration. Existing animal models do not adequately recapitulate mechanical instability, the principal driver of hypertrophic nonunion in clinical settings, thereby limiting translational relevance. We developed a murine model of delayed fracture healing using tunable intramedullary fixation to impos

Painful locking screws with tibial nailing, an underestimated complication.

BACKGROUND: Tibial fractures represent approximately 3-4% of reported fractures. Locked, intramedullary nails are commonly used to restore length and alignment and provide rotational stability. Few studies have assessed the complication rate of locking screws. MATERIALS AND METHODS: We conducted a retrospective observational study of all patients who underwent tibial nailing at our institution between the 01/01/15 and 30/06/17. All patients were followed up for at least 1 year post-operatively. For inclusion, patients had to be over 16 years of age and had undergone tibial nail fixation follo

Retrospective comparison of postoperative infection and bone union between late and immediate intramedullary nailing of Gustilo grades I, II, and IIIA open tibial shaft fractures.

BACKGROUND: The optimal method of skeletal stabilization is still controversial. Therefore, we examined the clinical outcomes associated with late (L) versus immediate intramedullary nailing (IMN). METHODS: This was a retrospective comparative study of trauma registry data from an emergency medical care center (university hospital). We examined 85 open tibial shaft fractures (85 patients) treated with L or immediate (I) IMN from January 2004 to December 2010. The L and I groups comprised 37 (33 men, 4 women) and 48 (44 men, 4 women) patients, respectively. The postoperative infection rate, ti

Long-Term Clinical Outcomes of Intercalary Allograft Reconstruction for Lower-Extremity Bone Tumors.

BACKGROUND: Improved survival rates for patients with primary bone tumors of the extremities have increased the demand for reliable and durable reconstruction techniques. Some authors have stated that, after successful ingrowth, allografts are a durable long-term solution. This hypothesis is largely based on small studies with short-to-midterm follow-up. In order to determine the durability of intercalary allograft reconstructions in the lower extremities, we evaluated the long-term clinical outcomes at a minimum of 10 years. METHODS: All patients who received an intercalary allograft reconst

Transtrochanteric vs. posterolateral approaches in hemiarthroplasty after proximal femoral nail complications: A comparative study.

BACKGROUND: Proximal femoral nailing is a widely used technique for treating intertrochanteric femur fractures but may result in complications requiring revision surgery. Hemiarthroplasty is a common solution, with the transtrochanteric and posterolateral approaches being two surgical options. This study aimed to compare these approaches in terms of surgical outcomes and complication rates. METHODS: A retrospective analysis was conducted on 79 patients who underwent revision hemiarthroplasty for complications related to proximal femoral nailing between 2019 and 2022. Patients were divided int

Comparison of transverse and longitudinal incisions in tibia intramedullary nailing: Implications for pain, numbness, and functional recovery.

BACKGROUND: The current standard of care for surgically managed tibia shaft fractures is intramedullary nailing. Chronic postoperative knee pain is the most common complication of the infrapatellar technique in intramedullary nailing. Literature suggests it may be related to infrapatellar nerve injury. OBJECTIVE: This study compares the incidence of anterior knee pain after tibia nail insertion between conventional longitudinal and transverse incisions. METHODS: This clinical trial study was conducted in our Level I trauma center from February 2022 to August 2024. Patients with closed diaphy

Interlocking screw backout from a preassembled polymer inlay in a retrograde femoral nail system: A retrospective review.

Interlocking screw backout is a significant complication of femoral nailing that may lead to pain, fixation failure, and need for reoperation. The risk for this is increased in osteoporotic bone or in cases of so-called "extreme nailing" when the articular segment is short, and fixation is limited. A newly developed retrograde femoral nailing system (DePuy Synthes RFN-Advanced™ Retrograde Femoral Nailing System (DePuy Synthes, Monument, CO)) was designed specifically to prevent this known failure mode and was recently approved for use within the United States. A preassembled polymer inlay at t

Relation between tibia malalignment after tibia nailing and foot pressure pattern.

PURPOSE: Intramedullary (IM) nailing in tibia fractures has been reported to have the potential to cause coronal plane malalignment that can be detrimental to foot pressure distribution and gait characteristics. The purpose of this paper was to explore tibia malalignment after tibia nailing and its association with foot pressure distribution using pedobarographic analysis. METHODS: 42 patients aged 18-80 years who sustained a tibia shaft fracture and who were operated with IM nailing were divided into three groups based on the degree of post-operative coronal plane angulation of the tibia: va

Differences in soft tissue damage using a percutaneous versus open approach for antegrade straight humeral nailing: a quantitative and qualitative anatomical study.

BACKGROUND: Percutaneous insertion of third-generation straight humeral nails is a recent alternative to the conventional open method. Rather than splitting, retracting and subsequently repairing the supraspinatus fibers to visualize the humeral head entry site, the percutaneous approach utilizes a cannulated awl to enter the intramedullary canal through the supraspinatus fibers without visualizing internal shoulder structures. Despite recent evidence demonstrating satisfactory outcomes in the percutaneous method, the potential for iatrogenic injury to the rotator cuff and other shoulder struc
