Torsades de pointes in a patient receiving fluconazole for cerebral cryptococcosis.

PURPOSE: A case of torsades de pointes associated with fluconazole use is described. SUMMARY: A 68-year-old woman with a history of hypertension treated with 2.5 mg of indapamide for 16 months sought medical treatment after having two falls 1 month apart. A computed tomography scan and subsequent magnetic resonance imaging of the brain revealed a lesion in the left pons and middle cerebellar peduncle. Biopsy of the pontine lesion revealed large yeast forms and subsequently revealed Cryptococcus neoformans var. gattii. The patient was initially treated with conventional amphotericin B and fluc…

Congenital ocular motor apraxia: clinical and neuroradiological findings, and long-term intellectual prognosis.

The severity of intellectual sequelae and prognosis varies in patients with congenital ocular motor apraxia (COMA). Here, we explored this p … nance imaging: 2). Dilatation of the fourth ventricle, mainly at the level of the midbrain or upper pons (n=7), and hypoplastic cerebellar vermis (n=6) were commonly observed in both the early- and late-walking groups. 'Molar tooth' signs (n=3) were exclusively noted in the late-walking group, and often accompanied by atypical oculomotor signs (3/3) an …

Cavernous malformations of the brainstem: three-dimensional-constructive interference in steady-state magnetic resonance imaging for improvement of surgical approach and clinical results.

OBJECTIVE: The indications for resection of cavernous malformations (CMs) of the brainstem include neurological deficits, (recurrent) hemorr … icrosurgical techniques and neurophysiological monitoring. RESULTS: Eleven CMs were located in the pons/pontomedullary region; 10 of the 11 were operated on via the lateral suboccipital approach. Three CMs were located near the floor of the fourth ventricle and operated on via the median suboccipital approach, with total removal of all CMs. Results we …

Cranial MR imaging with clinical correlation in preeclampsia and eclampsia.

PURPOSE: Our aim was to determine the distribution and nature of cranial MRI findings in preeclampsia/eclampsia, and also to correlate them … olved in all patients, followed by the parietal, frontal, and temporal lobes, and basal ganglia and pons. The lesions showed watershed distribution in 13 patients. When the patients with and without MR imaging findings were compared, there was a statistically significant difference regarding visual disturbances, depression of consciousness, and seizure …

OBJECT: The aim of this study was to evaluate whether the level of a spinal lesion is associated with variations in anomalous brain developm … upper-level spinal lesions demonstrated more qualitative abnormalities in the midbrain and tectum, pons, and splenium, although not in the cerebellum, compared with images obtained in children with lower-level spinal lesions. A facial nerve study using transcranial magnetic paired stimulation.

OBJECTIVE: The purpose of this study was to evaluate the refractory period of the facial nerve by transcranial magnetic paired stimulation (TMPS) with as short interstimulus interval (ISI) as possible. Also, by applying TMPS, the long latency response obtained at the same time was recorded and its neurophysiological characteristics were studied. METHODS: (Experiment 1) The subjects comprised of 30 normal volunteers and 19 patients with Bell's palsy. The experiments were carried out using two sets of Magstim model 200 and Bistim modules, a large coil measuring 90 mm in diameter with a maximal …

Neural correlates of laughter and humour.

Although laughter and humour have been constituents of humanity for thousands if not millions of years, their systematic study has begun onl … gh the motor cortex and pyramidal tract to the ventral brainstem. These systems and the laughter response appear to be coordinated by a laughter-coordinating centre in the dorsal upper pons. Analyses of the cerebral correlates of humour have been impeded by a lack of consensus among psychologists on exactly what humour is, and of what essential compone …

Role of nitric oxide in hypoxia-induced hyperventilation and hypothermia: participation of the locus coeruleus.

Hypoxia elicits hyperventilation and hypothermia, but the mechanisms involved are not well understood. The nitric oxide (NO) pathway is involved in hypoxia-induced hypothermia and hyperventilation, and works as a neuromodulator in the central nervous system, including the locus coeruleus (LC), which is a noradrenergic nucleus in the pons. The LC plays a role in a number of stress-induced responses, but its participation in the control of breathing and thermoregulation is unclear. Thus, in the present study, we tested the hypothesis that LC plays a role in the hypoxia-induced hypothermia and hy…

Cavernous malformations of the brainstem: experience with 100 patients.

OBJECT: In this study the authors review surgical experience with cavernous malformations of the brainstem (CMBs) in an attempt to define more clearly the natural history, indications, and risks of surgical management of these lesions. METHODS: The authors retrospectively reviewed the cases of 100 patients (38 males and 62 females; mean age 37 years) harboring 103 lesions at treated a single institution between 1984 and 1997. Clinical histories, radiographs, pathology records, and operative reports were evaluated. The brainstem lesions were distributed as follows: pons in 39 patients, medulla…

The potassium channel subunit KV3.1b is localized to somatic and axonal membranes of specific populations of CNS neurons.

Potassium channels play major roles in the regulation of many aspects of neuronal excitability. These channels are particularly well suited for such multiplicity of roles since there is a large diversity of channel types. This diversity contributes to the ability of specific neurons (and possibly different regions of the same neuron) to respond uniquely to a given input. Neuronal integration depends on the local response of spatially segregated inputs to the cell and the communication of these integration centers with the axon. Therefore, the functional implications of a given set of K+ channe…

[Cerebral ultrasound diagnosis in brain abnormalities].

Gray scale ultrasonography of the brain was performed in 93 infants with cerebral malformations. The most common malformation was the Chiari-malformation (56 children), characterized by the caudal displacement of the cerebellar vermis into the foramen magnum. The rest of the cerebellum, pons and medulla oblongata were displaced caudally and dysplastic. All children with Chiari-syndrome showed more or less severe hydrocephalus. The Dandy-Walker-malformation (3 children) was characterized by a huge retrocerebellar cyst communicating with the fourth ventricle. The cerebellar vermis was dysplastic…
