Baha solutions for patients with severe mixed hearing loss.

Patients with a mixed hearing loss present special challenges. The amplification demands of mixed hearing loss can drive powerful digital hearing aids to their limits and introduce distortion through saturation. Conversely, the Baha System effectively bypasses the conductive component and focuses on compensating for the sensorineural component of the hearing loss. Ten patients with a mixed hearing loss participated in the present study. Results indicate that Baha provided significant benefits (p < 0.01) over conventional air conduction hearing instruments across the dimensions of audibility, s

Predicting performance on the digits-in-noise test in noise and reverberation using the speech transmission index.

This study investigated the effects of noise and reverberation on recognition of Dutch digits-in-noise (DIN) test triplets. The findings demonstrate that the STIDIN, an adaptation to the standard speech transmission index (STI), provides a reliable, low‑error predictor of DIN test performance in conditions with noise and/or reverberation when only a single speech recognition threshold (SRT) measurement in noise is completed. To achieve this, twenty-four normal-hearing (NH) adults completed adaptive SRT measurements in noise-only, reverberation-only and combined noise and reverberation conditio

Frequency and Hearing Loss Phenotypes of OPA1 Variants in a Cohort of 18,475 Patients with Hearing Impairment.

BACKGROUND/OBJECTIVES: The OPA1 gene encodes a dynamin-related GTPase essential for mitochondrial fusion. Variants in OPA1 are a major cause of autosomal dominant optic atrophy (DOA). A subset of DOA patients exhibits hearing loss, often manifesting as auditory neuropathy spectrum disorder (ANSD). In this study, we aimed to describe the frequency of OPA1-related hearing loss in a large cohort of patients with hearing loss and to explore the genotype-phenotype correlations and appropriate interventions. METHODS: A total of 18,475 Japanese patients with hearing loss were recruited. Targeted mas

A Longitudinal Exploratory Case-Series Comparing Morphological Trajectories in Four Early-Implanted Greek-Cypriot Children and Eight Typically Hearing Peers.

Objectives: This paper explores whether deaf Greek-Cypriot (GC) children with cochlear implants (CIs) begin using the various forms of the definite article (DA) of the Greek language similarly to their typically hearing peers in terms of order of appearance and age. Furthermore, it explores if their MLUm growth correlates with that of typically developing peers. Methods: This is a longitudinal study with quantitative and qualitative analyses. The sample comprises four prelingually deaf GC children with CIs and eight typically hearing (TH) children with no developmental delays. Age and hearing

[Vestibular Assessment in Children].

Dizziness is a common symptom not only in adults but also in children, with a rising incidence in recent years. The prevalence of vestibular disorders in the pediatric population ranges between 0.4% and 15%, depending on the study. While both genders are equally affected in childhood, females are more frequently diagnosed after puberty, a trend persisting into old age. Migraine-associated vertigo, such as benign paroxysmal childhood vertigo (BPVC) and vestibular migraine, accounts for up to 50% of pediatric cases. These conditions manifest as sudden, short-lived dizziness episodes with rapid r

Age effect on inter-pulse interval selection for ECAP measurement.

INTRODUCTION: Age and duration of deafness significantly affect the auditory nerve (AN) health of cochlear implant (CI) users. In this study, we examine how age and duration of deafness impact Electrically Evoked Compound Action Potential (ECAP) amplitudes when the Inter-Pulse Interval (IPI) parameter in ECAP measurements is varied. METHODS: Our participants were thirty-one Mandarin-speaking CI users with Nurotron/CS-20A (19 males and 12 females, 33 ears, aged 6-66 yrs). We investigated eleven distinct IPI settings, ranging from 290 μs to 590 μs (IPI = 290, 330, 350, 390, 410, 450, 470, 510,

Does age protect against loss of tonotopy after acute deafness in adulthood?

The mammalian auditory system develops a topographical representation of sound frequencies along its pathways, also called tonotopy. In contrast, sensory deprivation during early development results in no or only rudimentary tonotopic organization. This study addresses two questions: (1) How robust is the central tonotopy when hearing fails in adulthood? (2) What role does age play at time of deafness? To address these questions, we deafened young and old adult rats with previously normal hearing. One month after deafening, both groups were unilaterally supplied with cochlear implants and elec

Inositol trisphosphate and ryanodine receptor signaling distinctly regulate neurite pathfinding in response to engineered micropatterned surfaces.

Micro and nanoscale patterning of surface features and biochemical cues have emerged as tools to precisely direct neurite growth into close proximity with next generation neural prosthesis electrodes. Biophysical cues can exert greater influence on neurite pathfinding compared to the more well studied biochemical cues; yet the signaling events underlying the ability of growth cones to respond to these microfeatures remain obscure. Intracellular Ca2+ signaling plays a critical role in how a growth cone senses and grows in response to various cues (biophysical features, repulsive peptides, chemo

New horizons in hearing conditions.

Hearing conditions such as hearing loss, tinnitus and hyperacusis are highly prevalent in the population and can severely impact communication and quality of life. Hearing is affected by multiple factors, including heredity, noise exposure, age, sex, ear disorders and lifestyle factors. Globally, hearing loss affects over 80% of adults aged 80 years and older, is often experienced in combination with other long-term health conditions and is a mid-life risk factor for dementia. To form a themed collection, we searched Age and Ageing for articles on hearing conditions published from 2000 onwards

Analysis of TMIE gene mutations including the first large deletion of exon 1 with autosomal recessive non-syndromic deafness.

BACKGROUND: Transmembrane inner ear (TMIE) protein is an essential component of the mechanotransduction complex. In collaboration with other components, TMIE aids the maintenance and function of the sensory hair cells. Autosomal recessive deafness-6 (DFNB6) is caused by mutated TMIE, a gene in the high genetic heterogeneity spectrum of deafness. Hearing loss has a significant impact on the global economy and the quality of life of affected persons, their families, and society. Here, three unrelated families with TMIE variants are presented. All three cases were found while studying the genetic

Integrating community health workers into a community hearing health collaborative to understand the social determinants of health in children with hearing loss.

BACKGROUND: Children with hearing loss (HL) require coordination of care to navigate medical and social services. Strong evidence supports the role of community health workers (CHWs) to identify and address social barriers. OBJECTIVE: The goal of this study was to evaluate the impact of integrating CHWs into the medical teams of children with HL and identify the social needs associated with their caregivers at a large urban hospital center. METHODS: A retrospective chart review was conducted for 30 children with HL whose caregivers enrolled in a CHW program between August 1, 2017 and Decembe

Haemophilus influenzae Type f Meningitis Complicated by Bilateral Subdural Empyema, Central Venous Thrombosis, and Bilateral Sensorineural Hearing Loss in an Immunocompetent 12-month-old.

Haemophilus influenzae is a gram-negative coccobacillus that colonizes the nasopharyngeal surface and upper respiratory tract of healthy individuals and includes six encapsulated serotypes as well as non-encapsulated, non-typeable strains. Since the widespread use of the Haemophilus influenzae type b (Hib) conjugate vaccine implemented in 1990, the majority of invasive illness now seen in the United States is secondary to capsular serotypes other than type b and non-typeable strains with the largest burden of disease affecting the extremes of age-infants and the elderly. We report a case of ac

Bartter syndrome: An infrequent tubulopathy of prenatal onset.

INTRODUCTION: Bartter syndrome (BS) is a rare inherited tubulopathy that has two presentation forms, the first one is a severe form of antenatal onset (neonatal Bartter) and the second one is a later on set form during the first years of life (classic Bartter). In the antenatal form, it manifests with fetal polyuria, polyhydramnios of early and severe onset, premature delivery, and intrauterine growth restriction. In the postnatal stage, it presents recurrent episodes of dehydration and electrolyte im balance that can compromise the survival of the patient. OBJECTIVE: To report a clinical cas

Comparing Parent and Teacher Ratings of Emotional and Behavioural Difficulties in 5-year old Children who are Deaf or Hard-of-Hearing.

Children who are deaf or hard of hearing (DHH) are at higher risk of developing mental health problems. This study reports on the parent and teacher ratings of emotional and behavioural difficulties (EBD) in 5-year old DHH children. It explores the similarities and differences between informants, and the risk and protective factors associated with parent and teacher-ratings of EBD. Parents and teachers of 224 DHH children completed questionnaires on children's EBD and functional auditory behaviour. Children completed standardised assessments of non-verbal cognitive and language abilities. On a

Ciliopathy: Usher Syndrome.

Ciliopathies are a group of disorders caused by a defect in ciliogenesis, ciliary protein trafficking. Because nearly every cell in the body (including the photoreceptors) contains cilia, defects in ciliary proteins typically affect multiple organ systems. Usher syndrome is the most common syndromic cause of retinitis pigmentosa (RP) and accounts for 10-20% of cases of RP Inheritance is autosomal recessive, and the retinal dystrophy is usually rod-cone dystrophy (Figs. 32.1 and 32.2). These patients have RP with sensorineural hearing loss (partial or complete) since birth; some may have vestib

Valproic acid promotes the neuronal differentiation of spiral ganglion neural stem cells with robust axonal growth.

Hearing loss occurs with the loss of hair cells of the cochlea and subsequent degeneration of spiral ganglion neurons (SGNs). Regeneration of SGNs is a potentially promising therapeutic approach to hearing loss in addition to the use of a cochlear implant (CI), because this device stimulates SGNs directly to restore hearing bypassing the missing hair cells. The presence of SGN-neural stem cells (NSCs) has been reported in adult human and mice. These cells have the potential to become SGNs and thus represent a cellular foundation for regeneration therapies for hearing loss. Valproic acid (VPA)

Challenges of modern medicine - geriatric otolaryngology or the advantages of cooperation between an otolaryngologist and a geriatrician.

Advanced age is often burdened with many deficits that are a consequence of the aging process, unfavorable lifestyle and multi-morbidity. They contribute to increased morbidity and disability of the older people. Laryngological problems often concern seniors, particularly progressing with age hearing impairment, dizziness, balance disorders, epistaxis, nasopharyngeal diseases, larynx, sinuses, and salivary glands disorders. They are not only a health threat, such as head and neck cancer, but they contribute to the deterioration of mobility, falls and injuries, depression, cognitive functions i
