A novel graded-stiffness footwear device for heel ulcer prevention and treatment: a finite element-based study.

Diabetic heel ulceration is a serious, destructive, and costly complication of diabetes. In this study, a novel "graded-stiffness" offloading method was proposed. This method consists of heel support with multi-increasing levels of stiffness materials, to prevent and treat heel ulcers. A three-dimensional finite element model of the heel was used to evaluate the novel "graded-stiffness" orthotic device compared to two existing solutions: (1) an insole with a hole under the active ulcer and (2) an insole with a hole filled with a soft material (elastic modulus of 15 kPa). Volumetric exposure ev

TCP/PLGA composite scaffold loaded rapamycin in situ enhances lumbar fusion by regulating osteoblast and osteoclast activity.

The purpose of this study was to develop a novel β-tricalcium phosphate (TCP)/poly (D,L-lactic-co-glycolic acid) (PLGA) composite scaffold loaded with rapamycin that can regulate the activity of osteoblasts and osteoclasts for lumbar fusion. The TCP/PLGA composite scaffold was fabricated by cryogenic three-dimensional printing techniques and then loaded with rapamycin in situ. The structural surface morphology of the composite scaffold was tested with scanning electron microscope. To evaluate the biocompatibility of the composite scaffold in vitro, bone marrow mesenchymal stem cells (BMSCs) we

Heat Sterilization Effects on Polymeric, FDM-Optimized Orthopedic Cutting Guide for Surgical Procedures.

Improvements in software for image analysis have enabled advances in both medical and engineering industries, including the use of medical analysis tools to recreate internal parts of the human body accurately. A research analysis found that FDM-sourced elements have shown viability for a customized and reliable approach in the orthopedics field. Three-dimensional printing has allowed enhanced accuracy of preoperative planning, leading to reduced surgery times, fewer unnecessary tissue perforations, and fewer healing complications. Furthermore, using custom tools chosen for each procedure has

Biodegradable Antimicrobial Agent/Analgesic/Bone Morphogenetic Protein-Loaded Nanofibrous Fixators for Bone Fracture Repair.

PURPOSE: Postoperative infection and pain management are of great concern to orthopedic surgeons. Although there are several protocols available to deal with these aspects, they are fraught with complications, such as cartilage damage, cardiovascular and neurological intoxication, and systemic adverse responses. Therefore, it is necessary to develop safe and effective perioperative protocols. In the current study, antimicrobial agents/analgesics/growth factor-embedded biodegradable hybrid fixators (polycaprolactone fixator + poly[lactide-co-glycolide] sheath-core structured nanofibers) for bon

Repeated transcatheter aortic valve implantation for the treatment of a degenerated transcatheter aortic valve implantation valve (valve-in-valve technique): a case report.

BACKGROUND: Valve-in-valve transcatheter aortic valve implantation (TAVI) has emerged as a competent alternative for the treatment of degenerated bioprosthetic valves after surgical aortic valve replacement, or during TAVI procedure as a bailout option. Herein, we report a rare case of a self-expandable Medtronic Evolut R valve into a failing Medtronic CoreValve, with the use of modern pre-TAVI imaging screening, suggesting the proper procedural design steps for so complicated implantations. CASE SUMMARY: A frail 78-year-old woman with a degenerated Medtronic Core Valve 26 mm bioprosthesis, i

Surgical treatment of orbital hypertelorism: Historical evolution and development prospects.

Orbital hypertelorism (OR.H) is defined as an abnormal increase in the distance between the two orbits secondary to a skeletal anomaly, and it occurs in association with numerous congenital craniofacial malformations. Since its description by Greig in 1924, OR.H and the associated corrective procedures have captivated many surgeons. Here we present a discussion of the historical evolution of surgery for OR.H and highlight its future prospects. In the mid-twentieth century, only cover-up techniques simulating approximation of the eyes via an optical illusion were used, such as frontonasal skin

Functional and histological assessment of 3D-printed helmet and hearing protection devices in preventing blast-induced auditory injury in Chinchillas.

Blast-induced auditory injury remains a persistent concern in many high-risk occupational environments, especially military service members and veterans. Although helmets and hearing protection devices (HPDs) have independently demonstrated efficacy in mitigating acoustic injuries, the synergistic protective effect of their combined use against blast overpressure (BOP) has not been comprehensively evaluated. The present study investigated the auditory protective performance of a custom-engineered 3D-printed helmet, both independently and in conjunction with HPDs, in a chinchilla model subjecte

3D printed carboxymethyl cellulose scaffolds for autologous growth factors delivery in wound healing.

This work aims to use carboxymethyl cellulose (CMC) as main structural and functional component of 3D printed scaffolds for healing of diabetic wounds. Differently from previous inks involving small contents in CMC, herein sterile (steam-heated) concentrated CMC solely dispersions (10-20%w/v) were screened regarding printability and fidelity properties. CMC (15%w/v)-citric acid inks showed excellent self-healing rheological properties and stability during storage. CMC scaffolds loaded with platelet rich plasma (PRP) sustained the release of relevant growth factors. CMC scaffolds both with and

Modulation of nose-to-brain delivery of a P-glycoprotein (MDR1) substrate model drug (quinidine) in rats.

During the last decades several new drug formulations were developed to target the central nervous system (CNS) from the nasal cavity. However, in these studies less attention was paid to the possible drug-drug interactions in case of multi-drug therapy. In our pilot study first we compared a nasal solution and a nasal gel to demonstrate their distribution in the nasal cavity (3D printed rat skull model and histology). Due to the aspiration induced high mortality at administration of nasal solution the study was continued only with the gel formulation of quinidine. The aim of our experiments w

3D printed complex tissue construct using stem cell-laden decellularized extracellular matrix bioinks for cardiac repair.

Stem cell therapy is a promising therapeutic method for the treatment of ischemic heart diseases; however, some challenges prohibit the efficacy after cell delivery due to hostile microenvironment of the injured myocardium. 3D printed pre-vascularized stem cell patch can enhance the therapeutic efficacy for cardiac repair through promotion of rapid vascularization after patch transplantation. In this study, stem cell-laden decellularized extracellular matrix bioinks are used in 3D printing of pre-vascularized and functional multi-material structures. The printed structure composed of spatial p

Mandibular distraction combined with orthognathic techniques for the correction of severe adult mandibular hypoplasia.

BACKGROUND: Mandibular hypoplasia may result from congenital deformities or trauma or infection during the early stage of facial skeleton development. Deficiencies in the growth of the mandible can not only cause various degrees of facial deformity but also affect breathing and occlusal function. Here, we report our experiences with mandibular distraction combined with orthognathic surgical techniques for the treatment of severe adult mandibular hypoplasia. METHODS: Cephalometric analysis was conducted in all patients for quantitative evaluation. A computer-assisted surgical simulation was pr

Fully Fiber-Integrated 3D-Printed Probes for Plug-and-Play Endoscopic Optical Coherence Tomography.

Miniature probes extend optical coherence tomography (OCT) into lumens and other confined spaces, but conventional implementations often rely on multiple distal components, fiber processing, and probe-specific interferometer matching. Here, a fully fiber-integrated OCT (F2I-OCT) architecture is demonstrated in which two-photon microfabrication defines not only the terminal imaging optic, but the complete distal optical and interferometric architecture within a single fiber-mounted element. Beam expansion, side-view redirection, common-path reference generation, and terminal imaging are physica

Left bundle branch pacing in anatomically repaired congenitally corrected transposition of the great arteries: a case report.

BACKGROUND: There are currently different approaches to conduction system pacing in unrepaired congenitally corrected transposition of the graet arteries (CCTGA) patients. However, these therapies are not feasible when patients have undergone anatomic repair. CASE SUMMARY: We describe our approach and considerations for left bundle branch (LBB) area pacing in a 12-year-old patient with CCTGA after double switch operation using Rastelli repair. Three-dimensional reconstructions derived from cardiac CT were both printed as a dry model and uploaded in a virtual reality environment to enhance ana

Plant-Inspired Elastic-Hydraulic Tactile Sensing Enables Quantitative Stiffness Estimation in Soft Robots.

Soft robots require tactile sensors capable of quantifying the mechanical properties of unknown objects during manipulation, yet most existing approaches are fragile, costly, or limited in dynamic range. Here, we present a model-guided plant-inspired hydraulic tactile sensor in which contact-induced deformation of a compliant elastomer generates a measurable pressure change in an embedded liquid-filled channel. By combining pressure and deformation measurements with an analytical elastic-hydraulic contact model, the effective Young's modulus of the contacted spherical object can be inferred wi
