Serial Tc-99m MAG3 renography evaluating the recovery of acute kidney injury associated with minimal change nephrotic syndrome.

Acute kidney injury (AKI) is a well-recognized complication of minimal change nephrotic syndrome (MCNS). Previous reports support the concept that AKI associated with MCNS is reversible; however, information regarding the hemodynamic basis of AKI in MCNS is insufficient. We herein describe a case of AKI in a man with MCNS. In this case, monitoring the longitudinal changes in renal perfusion using serial Tc-99m-MAG3 renal scanning was beneficial for evaluating the pathophysiological background associated with the development of AKI. The potential impact of serial renal scanning in MCNS patients

Association of Clinical Findings With Complications in the Cubitus Varus Deformity After Supracondylar Fracture.

INTRODUCTION: Cubitus varus deformity is primarily a cosmetic complaint that causes some early and late complications. However, no studies have reported the cubitus varus deformity regarding the frequency of complications, relationship to the degree of deformity, and period from the occurrence of the initial injury. METHODS: Overall, 83 patients with cubitus varus deformity were examined. The differences in the humerus-elbow-wrist angle (∆HEW-A), tilting angle (∆TA), and internal rotation angle (∆IRA) between the affected and normal sides were measured to determine varus and extension and int

[Application of picture archiving and communication systems in lateral wedge osteotomy for cubitus varus deformity in teenagers].

OBJECTIVE: To investigate effectiveness of picture archiving and communication systems (PACS) in lateral wedge osteotomy for cubitus varus deformity in teenagers. METHODS: A clinical data of 16 teenagers with cubitus varus deformity between July 2014 and July 2016 was retrospectively analyzed. All patients were treated with lateral wedge osteotomy and fixed with plate. Before operation, the osteotomy design (the osteotomy angle and length) was done in the PACS, including the carrying angle of healthy limb and the varus angle of affected side. There were 10 males and 6 females, with an average

Patient specific instrumentation for corrective osteotomy in case of posttraumatic cubitus varus in children.

Malunion in cubitus varus most often results from inadequate supracondylar fracture reduction or from secondary displacement. Treatment of cubitus varus needs an accurate preoperative planning to obtain a good functional and esthetical outcome. Planning based on conventional radiology is source of inaccuracy and clinical results are variable. Developments of computer-assisted orthopaedic surgery (CAOS) and of patient specific instruments (PSI) have made accurate three dimensional (3D) preoperative simulation possible. This original technique based on 3D-osteotomy planning and using PSI was dev

Long-term follow-up of vascular reconstructions after supracondylar humerus fracture with vascular lesion in childhood.

INTRODUCTION: Supracondylar humerus fractures in childhood present with a pulseless but well-perfused hand in 2.6% of cases and with limb-threatening ischaemia in <1%. Conservative treatment is widely used in non-limb-threatening ischaemia, in particular if the child is very young (<2.5 years). It has been sufficiently proven that conservative treatment may retard growth. The aim of our study was to determine long-term patency rates after surgical reconstruction and growth impairment, if any, after surgical vascular reconstruction. PATIENTS AND METHODS: Between June 1990 and June 2004, 12 chi

Surgical treatment of displaced olecranon fractures by tension band wiring technique.

Forty-five displaced olecranon fractures including 14 accompanying dislocated radial heads and seven radial head fractures were treated over a 13-year period by the tension band wiring technique. The use of supplemental internal fixation when necessary allows excellent results with the use of this technique, even in the presence of severe comminution or radial head dislocation. Primary silicone radial head implants fractured in all three patients in which they were used, necessitating repeat surgery in two patients to date. While loss of motion in terminal extension was a common aftermath of d

Supracondylar fractures of the humerus in children. Analysis at maturity of fifty-three patients treated conservatively.

From a pool of 131 supracondylar fractures of the humerus in 131 patients who were treated conservatively, all of which healed in an average time of 4.5 weeks without complications related to the treatment itself, the cases of fifty-three patients were reviewed at maturity. The average age at follow-up was twenty-six years. Nine patients had unimportant limitation of elbow motion, and slight atrophy of the musculature of the arm or forearm, or of both, was present in six patients. Arm-length discrepancy was never observed. The carrying angle remained at about the same value that had been prese

Far-advanced primary hyperparathyroidism in an 18-year-old young man.

Advanced debilitating primary hyperparathyroidism has become a clinical rarity as the condition is diagnosed at an asymptomatic stage in more patients. An 18-year-old young man is described who was referred because of an expanding facial mass. Evaluation revealed the classic musculoskeletal findings of severe primary hyperparathyroidism, as originally reported in the early part of the century.

A Safe Medial Column Screw Strategy for Transcondylar Humerus Fractures in the Elderly: CT-Based Analysis of Screw Diameter and Insertion Trajectory.

BACKGROUND: Double-plate fixation is widely used for transcondylar fractures of the distal humerus in elderly patients. Recently, fixation using a medial column screw (MCS) without a medial plate has been reported. However, anatomical data regarding optimal screw diameter and insertion trajectories are scarce. This study aimed to clarify the anatomical basis of MCS fixation using CT-based morphologic analysis. METHODS: We retrospectively analyzed CT data from 92 humeri (51 male, 41 female) obtained between 2012 and 2023. A virtual cylinder was placed along the medial column, and the maximum d

Biomechanical evaluation of the effect of varying humeral implant length in total elbow replacement: a finite element analysis.

BACKGROUND: Total elbow arthroplasty (TEA) is increasingly used for expanding indications but complications like aseptic loosening and periprosthetic fractures persist. Our objective is to examine the biomechanical behavior of the total elbow implant in response to varying implant lengths by investigating the stresses and the stress shielding effect in the bone-implant assembly using finite element (FE) modeling. METHODS: A fourth-generation synthetic humerus sawbone and its corresponding digital model were used in this study. Total elbow implants are laser scanned to obtain the 3D implant mo

Osteoblastogenesis and osteolysis in the Zucker Diabetic Sprague Dawley rat humerus head.

The endocrinology of type 2 diabetes (T2D) and its predisposing factors have been studied extensively while its skeletal effects have received negligible research despite this being a global disease. The cellular and molecular association between proximal humeral fractures and T2D has not been fully elucidated. We aimed to study bone cell quantities and immunolabel osteogenic and antiosteogenic cytokines. The study used 12-week-old rats (23 males) consisting of 8 Sprague Dawley (SD) and 15 Zucker Diabetic Sprague Dawley (ZDSD). Weekly mass measurements were taken while fasting blood glucose le…

Correction of Pediatric Cubitus Varus by Centralization of the Distal Humeral Fragment: A Surgical Technique to Avoid Lateral Condylar Prominence.

BACKGROUND: Cubitus varus is a well-recognized late complication of supracondylar humerus fractures in children. Various osteotomies have been described to correct this deformity, but each has disadvantages. The purpose of this study was to investigate the outcomes of a surgical technique, lateral closing wedge osteotomy combined with centralization of the distal humeral fragment, in cubitus varus. Meanwhile, the occurrence of postoperative lateral condylar prominence was observed. METHODS: From January 2015 to December 2019, 36 pediatric cubitus varus deformity patients who were treated with

Clinical translation of a mesenchymal stromal cell-based therapy developed in a large animal model and two case studies of the treatment of atrophic pseudoarthrosis.

Pseudoarthrosis is a relatively frequent complication of fractures, in which the lack of mechanical stability and biological stimuli results in the failure of bone union, most frequently in humerus and tibia. Treatment of recalcitrant pseudoarthrosis relies on the achievement of satisfactory mechanical stability combined with adequate local biology. Herein we present two cases of atrophic pseudoarthrosis that received a tissue-engineering product (TEP) composed of autologous bone marrow-derived mesenchymal stromal cells (BM-MSC) combined with deantigenized trabecular bone particles from a tiss

Sports after shoulder arthroplasty: a comparative analysis of hemiarthroplasty and reverse total shoulder replacement.

BACKGROUND: Traditionally, fewer postoperative sport restrictions are imposed on hemiarthroplasty (HHA) patients on than reverse total shoulder arthroplasty (RTSA) patients. However, functional outcomes have been shown to be superior in RTSA. No direct comparison of RTSA vs HHA has been done on rates of return to sports in patients with glenohumeral arthritis and rotator cuff dysfunction, proximal humeral fractures, or rheumatoid arthritis. METHODS: This is a retrospective review of consecutive RTSA and HHA patients collected from our institution's shoulder arthroplasty registry. All patients…

Radial head prosthesis in complex elbow dislocations: effect of oversizing and comparison with ORIF.

PURPOSE: Elbow dislocations with complex elbow instability (CEI) and unstable radial head fractures require reconstruction by open reduction and internal fixation (ORIF) if possible or alternatively by a radial head prosthesis. The aim of this study was to determine the differential outcome of both strategies and to investigate the contribution of prosthesis-related radiographic factors such as oversizing on clinical outcome. METHODS: A total of 53 patients underwent ligament and coronoid refixation, and radial head reconstruction by ORIF (n = 18; group 1) or by monopolar modular prosthesis (

The impact of a patient's engagement in their health on the magnitude of limitations and experience following upper limb fractures.

AIMS: Patient engagement in adaptive health behaviours and interactions with their healthcare ecosystem can be measured using self-reported instruments, such as the Patient Activation Measure (PAM-13) and the Effective Consumer Scale (ECS-17). Few studies have investigated the influence of patient engagement on limitations (patient-reported outcome measures (PROMs)) and patient-reported experience measures (PREMs). First, we assessed whether patient engagement (PAM-13, ECS-17) within two to four weeks of an upper limb fracture was associated with limitations (the Quick Disabilities of the Arm,

Antero-lateral minimally invasive plate osteosynthesis (MIPO) with the radial nerve exploration for extra-articular distal-third diaphyseal fractures of the humerus.

INTRODUCTION: Traditional open reduction and internal fixation (ORIF) of extra-articular distal humerus fractures has a risk of iatrogenic radial nerve injury, extensive soft tissue stripping, and long incision scar. We performed an antero-lateral minimally invasive plate osteosynthesis (MIPO) technique with the radial nerve exploration for distal-third diaphyseal fractures of the humerus and evaluated clinical and radiographic outcomes through this respective study. METHODS: From April 2010 to June 2016, 28 cases of extra-articular distal-third diaphyseal fractures were treated with an anter

Third degree open fractures and traumatic sub-/total amputations of the upper extremity: Outcome and relevance of the Mangled Extremity Severity Score.

INTRODUCTION: Third degree open fractures and traumatic sub-/total amputations of the upper extremity represent severe injuries and are associated with a high rate of functional impairment of the affected extremity. More than 20 years ago, the Mangled Extremity Severity Score (MESS) was introduced to predict amputation following severe lower extremity trauma. However, there have been few studies evaluating MESS in connection with the mangled upper limb. MATERIAL AND METHODS: A retrospective medical chart review was performed of all patients diagnosed with the aforementioned fractures of the u
