Disparities in Reporting of Violent Events by Patients at a US Children's Hospital.

BACKGROUND: Violence toward health care workers is increasing, yet patient risk factors for having a reported violent event (VE) are unknown. Understanding patient risk factors associated with reported VEs is needed to inform equitable approaches to VE reporting and guide resource delivery. OBJECTIVE: To identify patient and hospitalization characteristics associated with reported VEs at a US children's hospital. METHODS: This case-control retrospective study included children aged 5 to 18 that were hospitalized between January 1, 2019 and December 31, 2023 at a 390-bed children's hospital.

Five-Year Outcomes of a School-Based Personality-Focused Prevention Program on Adolescent Substance Use Disorder: A Cluster Randomized Trial.

OBJECTIVE: Rates of substance use disorders (SUDs) remain significantly above national targets for health promotion and disease prevention in Canada and the United States. This study investigated the 5-year SUD outcomes following a selective drug and alcohol prevention program targeting personality risk factors for adolescent substance misuse. METHODS: The Co-Venture trial is a cluster randomized trial involving 31 high schools in the greater Montreal area that agreed to conduct annual health behavior surveys for 5 years on the entire 7th grade cohort of assenting students enrolled at the sch

The effects of mindfulness and cognitive strategy interventions on core symptoms in children with ADHD: A randomized controlled trial.

Psychological interventions based on mindfulness and cognition have shown promise for treating ADHD, yet their application in children remains understudied. This study aimed to investigate the effectiveness and sustainability of mindfulness and cognitive strategy interventions in reducing core symptoms of ADHD in children. A randomized controlled trial (RCT) was conducted involving 70 children with ADHD aged 6-12, who were randomly assigned to one of three groups: a mindfulness-based cognitive intervention group (M + CB, n = 22), a cognitive-based mindfulness intervention group (C + MB, n = 23

Identification of Transdiagnostic Childhood Externalizing Pathology Within an Electronic Medical Records Database and Application to the Analysis of Rare Copy Number Variation.

Externalizing traits and behaviors are broadly defined by impairments in self-regulation and impulse control that typically begin in childhood and adolescence. Externalizing behaviors, traits, and symptoms span a range of traditional psychiatric diagnostic categories. In this study, we sought to generate an algorithm that could reliably identify transdiagnostic childhood-onset externalizing cases and controls within a university hospital electronic health record (EHR) database. Within the Vanderbilt University Medical Center (VUMC) EHR, our algorithm identified cases with a clinician-validated

Etiopathogenesis in the Development of Borderline Personality Characteristics in Children and Adolescents.

The validity and clinical significance of the characteristics of borderline personality disorder (BPD) in adolescents are increasingly being recognized. The persistence of BPD characteristics in adolescence is high and is associated with negative interpersonal, academic, professional, and financial outcomes. In the literature, BPD characteristics observed in children and adolescents are explained with psychodynamic theories, developmental models, and evolutionary approaches. Emotional dysregulation, interpersonal dysfunction, impulsivity, and self-harming behavior, negative life events, temper

A tool for Dravet syndrome-associated neuropsychiatric comorbidities evaluation (DANCE).

BACKGROUND: Dravet syndrome (DS) is a rare and severe form of epilepsy that begins in infancy, which is primarily caused by pathogenic variants in the SCN1A gene. DS is characterized by prolonged and frequent drug-resistant seizures, as well as developmental delays and behavioral problems. The identification of these comorbidities is based on clinical interview and relies on healthcare professionals (HCPs) experience. METHODS: We assembled a group of expert HCPs and caregivers to create a screening checklist for assessing DS-Associated Neuropsychiatric Comorbidities (DANC). The checklist incl

Joint and Limb Loading during Gait in Adults with ACL Reconstruction: Comparison between Single-Step and Cumulative Load Metrics.

PURPOSE: Individuals with anterior cruciate ligament reconstruction (ACLR) generally exhibit limb underloading behaviors during walking, but most research focuses on per-step comparisons. Cumulative loading metrics offer unique insight into joint loading as magnitude, duration, and total steps are considered, but few studies have evaluated if cumulative loads are altered post-ACLR. Here, we evaluated if underloading behaviors are apparent in ACLR limbs when using cumulative load metrics and how load metrics change in response to walking speed modifications. METHODS: Treadmill walking biomecha

Striatal Transcriptome and Interactome Analysis of Shank3-overexpressing Mice Reveals the Connectivity between Shank3 and mTORC1 Signaling.

Mania causes symptoms of hyperactivity, impulsivity, elevated mood, reduced anxiety and decreased need for sleep, which suggests that the dysfunction of the striatum, a critical component of the brain motor and reward system, can be causally associated with mania. However, detailed molecular pathophysiology underlying the striatal dysfunction in mania remains largely unknown. In this study, we aimed to identify the molecular pathways showing alterations in the striatum of SH3 and multiple ankyrin repeat domains 3 (Shank3)-overexpressing transgenic (TG) mice that display manic-like behaviors. Behavioural addiction-A rising tide?

The term 'addiction' was traditionally used in relation to centrally active substances, such as cocaine, alcohol, or nicotine. Addiction is not a unitary construct but rather incorporates a number of features, such as repetitive engagement in behaviours that are rewarding (at least initially), loss of control (spiralling engagement over time), persistence despite untoward functional consequences, and physical dependence (evidenced by withdrawal symptoms when intake of the substance diminishes). It has been suggested that certain psychiatric disorders characterized by maladaptive, repetitive be

The association between repetitive behaviours, impulsivity and hyperactivity in people with intellectual disability.

BACKGROUND: There is a need for assessments of psychological difference and disorder in people who have more severe intellectual disability (ID). Hyperactivity and impulsivity are two behavioural domains of importance as they are correlated with self-injury and aggression and this alludes to a shared cognitive correlate of compromised behavioural inhibition. Additionally, compromised behavioural inhibition is demonstrably related to repetitive behaviour and the latter might be expected to be associated with impulsivity and hyperactivity. METHODS: The Activity Questionnaire (TAQ) was developed

Cognitive-behavioural therapy for individuals with bulimia nervosa and a co-occurring substance use disorder.

A significant percentage of individuals with bulimia nervosa (BN) also can be diagnosed with a co-occurring substance use disorder (SUD). Although studies have addressed the frequency of overlap between the disorders, etiology and shared personality traits, limited research is available about the treatment of these comorbid patients. Adapting cognitive-behaviour therapy (CBT) to serve as an integrated treatment for patients with both BN and a SUD is a viable option, as studies of CBT suggest that this form of treatment is efficacious for both disorders independently. The shared strategies in C

"I swear it's not Tourette's!" - it is time to rethink coprolalia in Tourette syndrome.

Coprolalia is a relatively rare type of complex vocal tic in people with Tourette syndrome (TS). In the general population, however, it is believed that coprolalia is the most typical and common symptom resulting in the stereotype of TS as the "swearing disease". Swearing, however, also occurs as a "normal" expression of negative emotions and thus occurs more often in people with impulse control problems. It is believed that there is a continuum reaching from voluntary to involuntary swearing. Besides coprolalia, in TS, coprophenomena may also occur in the form of motor tics, called copropraxi

Combined abdominal and transcranial photobiomodulation is associated with fecal microbiome restructuring: a systems microbiology pre-post case report.

Photobiomodulation (PBM) has been proposed as a non-invasive strategy capable of modulating mitochondrial, inflammatory, vascular, and gut-brain axis-related pathways. However, its potential effects on fecal microbiome ecology in neurobehavioral conditions remain poorly characterized. This case report describes paired fecal microbiome findings before and after a 90-day combined abdominal and transcranial PBM protocol in a 6-year-old female with autism spectrum disorder and chronic constipation. PBM was delivered daily using a 1064-nm near-infrared LED device with 54 mW/cm2 irradiance, applied

Sensory inputs from the oral region to the cerebral cortex in behaving rats: an analysis of unit responses in cortical somatosensory and taste areas during ingestive behavior.

1. The responses of 90 cortical neurons in the somatosensory and gustatory areas were recorded with chronically implanted fine wires in freely moving Wistar rats. The responses were analyzed mainly while the animals were freely licking solutions and eating dry pellets. Cortical neurons were classified into several groups according to their response properties. 2. "Mechanosensitive" neurons (n = 20) showed rhythmic phasic activity in different phases of the licking cycle, depending on the location of their receptive field in the peripheral orofacial region. 3. "Movement-related" neurons (n = 27

In vitro associative conditioning of Hermissenda: cumulative depolarization of type B photoreceptors and short-term associative behavioral changes.

Cumulative depolarization of Hermissenda type B photoreceptors, a short-term neural correlate of associative learning, was produced by simulating associative training in the isolated nervous system (in vitro conditioning). This simulation entailed stimulation and recording from three classes of neurons normally affected by the associative training procedure: a type B photoreceptor, the silent/excitatory (S/E) optic ganglion cell, and a statocyst caudal hair cell. Exposure of the isolated nervous system to five simultaneous pairings of light and current-induced impulse activity of the caudal ha

White matter signatures of addiction diverge across opioids, cannabis and nicotine and predict impulsive choice in cannabis dependence.

White matter (WM) microstructure is implicated in the neurobiology of substance use disorders (SUDs), yet direct comparisons across substance-dependence syndromes remain scarce. It is also unclear whether WM abnormalities relate to impulsive decision-making and substance use severity. We performed a cross-sectional, dual-centre diffusion MRI study and quantified WM microstructure using diffusion tensor imaging and neurite orientation dispersion and density imaging in 87 individuals with SUDs (48 with cannabis dependence (CUD), 16 with opioid dependence (OUD), and 23 with nicotine dependence (N

Classification and discrimination of emotion dysregulation disorders using machine learning.

Attention-deficit/hyperactivity (ADHD), bipolar (BD) and borderline personality (BPD) disorders are severe psychiatric illnesses often presenting with overlapping emotion dysregulation symptoms. To date, it is unknown whether these disorders share a continuum on emotion dysregulation. To address this, we employed machine learning (ML) algorithms on psychological and environmental features from 232 adults to predict and discriminate these disorders. We recruited 92 patients diagnosed with either ADHD, BD, or BPD. Considering heritability, we recruited 67 genetically unrelated ADHD, BD and BPD o
