Single nucleotide polymorphism in gene encoding transcription factor Prep1 is associated with HIV-1-associated dementia.

BACKGROUND: Infection with HIV-1 may result in severe cognitive and motor impairment, referred to as HIV-1-associated dementia (HAD). While its prevalence has dropped significantly in the era of combination antiretroviral therapy, milder neurocognitive disorders persist with a high prevalence. To identify additional therapeutic targets for treating HIV-associated neurocognitive disorders, several candidate gene polymorphisms have been evaluated, but few have been replicated across multiple studies. METHODS: We here tested 7 candidate gene polymorphisms for association with HAD in a case-contr

Comparing two measures of psychomotor performance in patients with HIV: the Coin Rotation Test and the Modified HIV Dementia Screen.

BACKGROUND: Psychomotor slowing is an early symptom of HIV-associated dementia. The Modified HIV Dementia Scale (MHDS) is a fast, reliable measure of HIV-associated dementia. However, literacy may influence the psychomotor speed subscale (MHDS-PS). Psychomotor speed might be more accurately assessed using the Coin Rotation Test (CRT). OBJECTIVE: This study examined the CRT's validity in assessing psychomotor speed in HIV+ patients by comparing it with the MHDS-PS. We also investigated whether demographic, education, or HIV-related variables were associated with either measure. METHODS: The s

The comorbidity of HIV-associated neurocognitive disorders and Alzheimer's disease: a foreseeable medical challenge in post-HAART era.

Although the introduction of highly active antiretroviral therapy (HAART) has led to a strong reduction of HIV-associated dementia (HAD) incidence, the prevalence of minor HIV-1-associated neurocognitive disorder (HAND) is rising among AIDS patients. HAART medication has shifted neuropathology from a subacute encephalitic condition to a subtle neurodegenerative process involving synaptic and dendritic degeneration, particularly of hippocampal neurons that are spared prior to HAART medication. Considerable neuroinflammation coupled with mononuclear phagocyte activation is present in HAART-medic

Cerebrospinal fluid HIV infection and pleocytosis: relation to systemic infection and antiretroviral treatment.

BACKGROUND: Central nervous system (CNS) exposure to HIV is a universal facet of systemic infection. Because of its proximity to and shared … arge differences between plasma and CSF VL. CSF HIV RNA correlated neither with the severity of the AIDS dementia complex (ADC) nor abnormal quantitative neurological performance, although these measures were associated with depression of CD4 counts. In subjects starting ART, those with lower CD4 counts had slower initial viral decay in CSF than in plasma. In all subje …

Ordered multiple-class ROC analysis with continuous measurements.

Receiver operating characteristic (ROC) curves have been useful in two-group classification problems. In three- and multiple-class diagnostic problems, an ROC surface or hyper-surface can be constructed. The volume under these surfaces can be used for inference using bootstrap techniques or U-statistics theory. In this article, ROC surfaces and hyper-surfaces are defined and their behaviour and utility in multi-group classification problems is investigated. The formulation of the problem is equivalent to what has previously been proposed in the general multi-category classification problem but

Neurocognitive disorders seen in HIV disease.

Despite dramatic improvements in the therapeutic management of clients with human immunodeficiency virus (HIV) disease, cognitive disorders still appear as clinical manifestations of the illness trajectory. The development of neuro-cognitive impairment is associated with high levels of HIV activity and the resultant severe degree of immunosuppression. Although many clinicians almost exclusively associate HIV-related cognitive dysfunction with HIV encephalopathy and dementia, the etiologic conditions are numerous and include not only HIV infection affecting the brain, but also infections and ne

HIV-1 gp120 Effects on Signal Transduction Processes and Cytokines: Increased src-Family Protein Tyrosine Kinase Activity.

Varying degrees of neurological dysfunction are observed in AIDS patients who develop AIDS dementia complex (ADC). Data from a large number of in vivo and in vitro rodent studies have suggested a role for the HIV envelope glycoprotein gp 120 in this process. These studies were initiated to clarify possible effects of recombinant gp120 on signal transduction systems and the synthesis of specific ADC-related cytokines in human neuroblastoma cells. Out results indicate that gp120 on signal transduction systems and the synthesis of specific ADC-related cytokines in human neuroblastoma cells. Our r…

Memory for verbal information in individuals with HIV-associated dementia complex. HNRC Group.

Patterns of memory performance were examined for 9 participants with HIV-associated dementia (HAD), 15 HIV-seropositive participants without dementia, and 15 HIV-seronegative controls. Episodic and semantic memory were assessed using the California Verbal Learning Test, the Boston Naming Test, and Verbal Fluency tests. The HAD group showed deficits in episodic memory, with relative sparing of semantic memory. In addition, results suggest a retrieval deficit in HAD rather than a deficit in retention of information. This pattern is consistent with the presence of a subcortical dementing process

[Neurological impairment in children with HIV infection].

We retrospectively checked 53 paediatric patients suffering from infection with human immunodeficiency virus (HIV) registered in our Centre between the years 1987 and 1993, and evaluated the appearance of HIV encephalopathy. We noted important neurological signs in eleven patients (20.7%) ten of whom had HIV infection via vertical transmission and one as a result of contamination from haemoderivatives. In this review we give a detailed description of neurological signs, the moment of onset of these signs and their possible relationship with the state of the HIV infection. We also analyzed the

Anaesthetic management of caesarean section in a patient with active recurrent genital herpes and AIDS-related dementia.

We report the anaesthetic management of a pregnant patient with multiple manifestations of HIV infection who underwent Caesarean section. A 30-yr-old, HIV-positive, Haitian woman presented with acute psychosis at 28 weeks' gestation. A diagnosis of HIV dementia complex was made and haloperidol therapy was started. Five days after admission the patient was found to be in labour and tocolytic therapy with terbutaline was commenced. A vaginal lesion compatible with herpes simplex virus was observed which was treated with acyclovir. After 3 days of tocolytic therapy there were no further signs of

The use of methylphenidate for cognitive decline associated with HIV disease.

OBJECTIVE: Complaints of cognitive changes are often expressed by patients at all stages of HIV infection. Such changes include decreased memory and attention span, diminished concentration, apathy, and "slowing." Methylphenidate (MPD) has been used in several clinical studies in men with late-stage HIV disease in an attempt to ameliorate these difficulties. The objectives of this review article are to review salient psychopharmacological characteristics of MPD and to describe the research and clinical literature supporting the use of MPD in patients at all stages of HIV infection. METHODS: S

Suicide related to human immunodeficiency virus infection in Stockholm.

During a 5-year period (July 1, 1985 to June 30, 1990), a systematic investigation of medicolegal autopsy cases with regard to the occurrence of human immunodeficiency virus (HIV) infection was carried out at the Department of Forensic Medicine in Stockholm, Sweden. Cases where a positive result was obtained from enzyme-linked immunosorbent assay and Western blotting were counted as HIV-positive (85 instances among 16,938 deaths, or 0.50%). Of these deaths, 21 (25%) were suicides. The number and proportion of suicides among HIV-positive homo- and bisexual males increased during the study perio

[Psychiatric morbidity in patients hospitalized for AIDS: experiences in a Quebec hospital].

Neuropsychiatric complications in AIDS patients are common and have been reported in many recent articles. The author reviewed the medical charts of 80 patients admitted for AIDS between April 1984 and April 1988 to determine the frequency and nature of psychiatric complications in these patients and to compare these with recent reports made by other authors. The most frequent complications requiring psychiatric intervention seem to be organic mental disorders and adjustment disorders. Practical conclusions are drawn.

Encephalopathy in AIDS patients: evaluation with MR imaging.

The presence and extent of encephalopathy were evaluated in 47 patients with AIDS or AIDS-related complex (ARC) by the use of MR imaging. Twenty-nine (62%) of the patients showed some form of white matter disease, exhibited as high signal intensity on T2-weighted images. Focal white matter lesions were seen in 23 (49%) of the patients, while a diffuse white matter process was observed in six patients (13%). Of the 29 patients who had white matter disease on MR scans, 17 (36%) had a suggestion of white matter involvement on an initial CT study. Meanwhile, 12 (26%) of the patients had a normal C

Can Biomarkers Advance HIV Research and Care in the Antiretroviral Therapy Era?

Despite achieving human immunodeficiency virus type 1 (HIV-1) RNA suppression below levels of detection and, for most, improved CD4+ T-cell counts, those aging with HIV experience excess low-level inflammation, hypercoagulability, and immune dysfunction (chronic inflammation), compared with demographically and behaviorally similar uninfected individuals. A host of biomarkers that are linked to chronic inflammation are also associated with HIV-associated non-AIDS-defining events, including cardiovascular disease, many forms of cancer, liver disease, renal disease, neurocognitive decline, and os

FACTORS AFFECTING THE FATAL OUTCOME IN HIV-INFECTED PATIENTS WITH ENCEPHALITIS.

Despite the successful use of ART up to 40-70% of HIV(+) individuals have neurologic complications caused both by the HIV itself and by the reactivation of OIs on the background of severe immunodeficiency. Nowadays, there are no universally recognized criteria that allow predicting the outcome of encephalitis caused by OIs in this category of patients. The aim of our study was to assess factors affecting the fatal outcome in HIV(+) patients with CNS involvement. Retrospectively we selected 53 HIV(+) patients with confirmed encephalitis due to OIs. Depending on the outcome of the disease, patie

Progressive multifocal leukoencephalopathy in an HIV-infected child.

A child with perinatally acquired HIV infection presented with acute neurologic deterioration. A cerebellar white matter lesion seen on CT and MRI later proved to be progressive multifocal leukoencephalopathy (PML) by histology. Although a recognized disease of HIV-infected adults, PML is certain to be seen with more frequency in HIV-infected children who are surviving longer as a result of improved medical care. Recognition of the clinical and radiographic manifestations is important because of the dismal prognosis.

Neuropathological syndromes in the course of full blown acquired immune deficiency syndrome (AIDS) in adults in Poland (1987-1995).

Morphological analysis of the brains from 100 cases of full blown AIDS patients observed in the course of 1987-1995 years was performed. The material comprised 96 males, 3 females and 1 infant, 11 months old. Early material consisted almost exclusively of homo- and bisexuals, while in the last years heterosexual drug addicts prevailed. Gross brain examination revealed focal changes in 25% of cases, most of them being connected either with opportunistic infections or primary proliferating malignancies. Brain atrophy with an evident regional differences was observed macroscopically in 35 cases.
