Hearing preservation surgery for acoustic neuroma.

Nowadays smaller acoustic tumors are being detected earlier. We started to remove these tumors with attempt of preservation of hearing in 1994. Since 1994, in an attempt to preserve hearing, the retrosigmoid approach was used. Our first results are presented and criteria of preoperative selection and postoperative evaluation are discussed. A fair chance to keep a useful hearing can be given to a patient with a small tumor, preferrably smaller than 1,5 cm and provided that the average hearing levels differ by no more than 30 dB from the other ear. In all the other cases the translabyrinthine ro

Treating the oral sequelae of an acoustic neuroma.

This article describes the treatment of oral complications that arose after surgery for the removal of an acoustic brain tumor. The authors contend that a thorough oral examination can pinpoint the cranial nerves that have been affected by the tumor or the surgery, and that dental appliances can offer special protection after surgery.

The posterior fontanelle: a neglected acoustic window.

The additional information, obtained when using the posterior fontanelle routinely as the second acoustic window, is illustrated in four infants. Three of them are full-term infants with hypoxic ischaemic encephalopathy. In newborn infants, who are still too unstable to be transported to the magnetic resonance unit, extensive damage in the occipital subcortical white matter and/or cortex can be visualised by performing cranial ultrasound through the posterior fontanelle.

[Sonomotor vestibular reflex in acoustic neuroma].

Despite recent advance in diagnosis and surgical techniques of acoustic neuroma, ideal criteria for hearing preservation attempt remain unsatisfactory. This study reports sonomotor vestibular reflex recorded pre and postoperatively in 20 patients with acoustic neuroma. Hearing preservation was attempted according to tumor size and preoperative pure tone audiometry results. The studied population was divided in 2 groups according to hearing preservation was attempted (group 1 n = 13, retrosigmoid approach) or not (group 2 n = 7, translabyrinthin approach). In accordance with postsurgical auditi

[Incidence of normal hearing in acoustic neuroma].

Unilateral or asymetrical sensorineural hearing loss, tinnitus and instability classically represent the main symptoms for the suspected diagnosis of acoustic neurinoma. In literature there are very few studies which refer about acoustic neurinoma in patients with normal hearing. In this article we report the results of a retrospective analysis of 155 acoustic neurinoma cases with normal hearing managed in our center in the last 7 years. These patients are classified in two groups: 1) with normal hearing at the time of diagnosis and 2) with at least a mild sensorineural hearing loss. Of this 1

Clinicopathologic growth factors of acoustic neuromas.

BACKGROUND Understanding the tumor growth rate is very important when considering strategies for the treatment of an acoustic neuroma, although the natural course of acoustic neuromas has not been reviewed in detail. METHODS The clinicopathologic features and the postoperative growth of tumors were evaluated in 32 patients with acoustic neuromas. This study was undertaken to assess the variability of the growth potential of cells within an acoustic tumor and to determine the relationships between the growth rate and the clinicopathologic characteristics of the patients with acoustic neuromas,

Expression of steroid receptors in acoustic neuroma.

The expression of steroid receptors has been investigated in an attempt to clarify the role of steroid hormones in the proliferation and progression of acoustic neuromas. Specimens of tumours taken during translabyrinthine surgery were tested for cytosolic (c) and nuclear (n) steroid receptors. Oestrogen and progesterone receptor levels were evaluated by enzymatic immuno-assay, while androgen receptor binding levels were detected by dextran-coated charcoal method in a single-step determination. In some cases, the six point Scatchard analysis of cytosolic and nuclear androgen receptor was also

[Solitary retinal astrocytoma with "acoustic shadowing"].

PATIENT: A polycyclic lesion slightly prominent and yellowish in colour with a mulberry-like surface was incidentally detected in the left eye of a 69-year-old white female. It was located in the nasal horizontal sector of the fundus in a distance of about 5.5 mm from the optic disk. It was surrounded by a bright halo. Retinal arterioles penetrated the lesion superficially and more deeply. The vitreous body was unremarkable. Visual acuity was 0.8 to 1.0 p with a hyperopia of 3.5 dpt. Intraocular pressure was 116 mm Hg. In both eyes partially confluent drusen of the retinal pigment epithelium w

Mood disorders associated with acoustic neuromas.

OBJECTIVE: The primary purpose of this article is the presentation of three cases of manic and mixed states associated with an acoustic neuroma, and review of the literature on psychiatric symptoms accompanied by the tumor. METHODS: The cases were identified from 830 consecutive inpatient psychiatric admissions over age fifty-five years. Patients were assessed using a Structured Clinical Interview (SCID-R), and met DSM-III-R criteria for the diagnosis of bipolar disorder. The psychopathology seen in acoustic neuroma patients and the pathophysiologic mechanisms proposed to explain them are rev

Hearing preservation in acoustic neuroma surgery.

Hearing conservation in acoustic tumour surgery remains controversial. There have been few previous reports in the British literature. The senior author has managed 24 patients by retrosigmoid surgery with the intention of preserving hearing during the last 9 years. The clinical features, surgical technique and results are discussed with respect to pre-operative selection criteria, and post-operative quality of hearing. Hearing preservation has been achieved in 11 (78.6%) of 14 patients with small or intracanalicular tumours and a mean minimum auditory threshold of 35 dB and 70% speech discrim

Frequency selectivity in patients with acoustic neuroma.

Frequency selectivity was compared in subjects with hearing loss due to acoustic neuroma and cochlear pathology, and normal listeners. A particular interest was the role of probe tone parameters on the shape of the tuning curve. Psychophysical tuning curves (PTCs) were measured for each of two equal energy 2000-Hz probe tones (10 dB SL/300 msec and 17 dB SL/60 msec), using simultaneous 1/3-octave narrow-band noise maskers centered at 1, 1.25, 1.6, 2.5, 3.15, and 4 kHz. The results showed that the critical masker levels obtained for impaired listeners were significantly greater than those from

The natural history of untreated acoustic neuromas.

The emergence of magnetic resonance imaging with gadolinium has dramatically enhanced our ability to accurately detect the presence of acoustic tumors as small as 2 mm in diameter. Early diagnosis and improved surgical techniques continue to reduce the morbidity associated with surgical removal of these lesions. There exists, however, a select group of patients in whom no treatment may be the most appropriate management. Since 1979, a total of 51 patients with radiographic evidence of an acoustic neuroma have been prospectively followed for tumor growth and progression of symptoms. Patients we

Translabyrinthine removal of large acoustic neuromas.

Several surgical approaches to the cerebellopontine angle and internal auditory canal have been developed for the removal of acoustic neuromas. The choice of an approach may be influenced by hearing levels and tumor size. We reviewed the records of the primary translabyrinthine removal of 167 large (> or = 4 cm) acoustic neuromas performed between 1982 and 1990. Patients ranged in age from 15 to 83 years, with a mean of 43 years (male, 49%; female, 51%). Total removal was achieved in 95%. The facial nerve was preserved anatomically intact in 91%. At follow-up (mean, 2.1 yr), facial nerve funct

Acoustic rhinometry in pre-school children.

Acoustic rhinometry was performed in 35 normal nose-breathing children between 3 and 6 years. The average cross-sectional areas at the nasal valve, at the anterior end of the turbinates, and in the nasopharynx were 0.34 +/- 0.06 cm2, 0.35 +/- 0.08 cm2 and 1.37 +/- 0.48 cm2 respectively. The average minimal cross-sectional area was 0.29 +/- 0.06 cm2. The minimal cross-sectional area was located at the nasal valve in 14 and at the anterior end of nasal turbinates in 21 of the 35 children. As would be expected, the cross-sectional areas at different sites of the nasal cavity increased with increa

Perceptual-acoustic relationships in spasmodic dysphonia.

Perceptual ratings were obtained from voice samples of 19 patients with adductor spasmodic dysphonia before and 1 week after unilateral treatment with Botulinum toxin. Five experienced listeners judged samples of sustained phonation using a seven-point equal-interval scale. The perceptual parameters assessed were overall severity, strain-strangled voice quality, and breathiness. Perceptual results were related to the standard deviation of fundamental frequency and the voice break factor, two acoustic parameters previously shown to be significantly improved following Botulinum toxin injection.

A review of acoustic tumors: 1983-1988.

This is a review of the English literature relative to acoustic tumors from 1983 through 1988. In January of 1984, a similar article was published in this journal which reviewed the literature from 1979 through 1982. We have selected what we consider the most important articles from the hundreds that were published on this subject. Many other significant papers are not discussed. The material is presented in 11 categories: diagnosis, histopathology, treatment, hearing preservation, facial nerve preservation and repair, von Recklinghausen's disease and bilateral tumors, nonacoustic cerebellopon
