Cutaneous eruptions from ibrutinib resembling epidermal growth factor receptor inhibitor-induced dermatologic adverse events.

BACKGROUND: Ibrutinib is an oral inhibitor of Bruton tyrosine kinase that is approved by the United States Food and Drug Administration for several lymphoproliferative disorders and chronic graft-versus-host disease. OBJECTIVE: To characterize cutaneous eruptions arising from ibrutinib and highlight overlap with epidermal growth factor receptor (EGFR) inhibitor-induced dermatologic adverse events. METHODS: Single-center retrospective cohort of patients referred to the Skin Toxicities Program for treatment of cutaneous eruptions while taking ibrutinib. RESULTS: Among 19 patients, cutaneous e

Practical management of cutaneous reactions to the methylphenidate transdermal system: recommendations from a dermatology expert panel consensus meeting.

BACKGROUND: Psychostimulants remain the most-used medications for attention-deficit/hyperactivity disorder (ADHD). The methylphenidate transdermal system (MTS) is the first stimulant patch dosage formulation to be approved by the US Food and Drug Administration for the treatment of the symptoms of ADHD in children aged 6 to 12 years. The MTS patch is approved to be applied once daily to the hip and worn for 9 hours. While cutaneous reactions may occur with any formulation of medication, they are more likely with transdermal administration. OBJECTIVE: The purpose of this commentary was to desc

IL-18 has IL-12-independent effects in delayed-type hypersensitivity: studies in cell-mediated crescentic glomerulonephritis.

IL-18 (formerly known as IFN-gamma-inducing factor) enhances Th1 responses via effects that are thought to be dependent on and synergistic with IL-12. The potential for IL-18 to exert IL-12-independent effects in delayed-type hypersensitivity (DTH) responses was studied in a model of Th1-directed, DTH-mediated crescentic glomerulonephritis induced by planting an Ag in glomeruli of sensitized mice as well as in cutaneous DTH. Sensitized genetically normal (IL-12(+/+)) mice developed proteinuria and crescentic glomerulonephritis with a glomerular influx of DTH effectors (CD4(+) T cells, macropha

Interface-Engineered Nanocarriers for Translational and Patient-Centric Topical Therapy in Psoriasis.

Psoriasis is a chronic, immune-mediated dermatological disorder characterized by keratinocyte hyperproliferation and persistent inflammation, representing a significant therapeutic challenge. Conventional topical therapies are often limited by inadequate skin penetration, poor drug stability, and systemic toxicity, necessitating the development of advanced drug delivery platforms. Recent progress in colloid and interface science has enabled the design of nanocarrier systems including solid lipid nanoparticles, nanostructured lipid carriers, nanoemulsions, and liposomes that optimize drug-skin

Effective Management of Mosaic Epidermolytic Ichthyosis in a Pediatric Patient With Urea 30% Cream: A Case Report.

Epidermolytic ichthyosis (EI) is a rare autosomal dominant keratinization disorder characterized by blistering at birth followed by progressive hyperkeratosis. Treatment is primarily supportive, involving emollients, keratolytics, and occasionally systemic agents. We report a pediatric case of clinically and histologically confirmed linear EI that showed marked improvement with topical urea 30% cream. Therapy led to substantial reduction in scaling, smoother skin texture, and improved comfort, with no adverse effects. This case highlights the potential role of high-concentration urea as a safe

Rosacea: New Concepts in Classification and Treatment.

Rosacea is a chronic inflammatory dermatosis mainly affecting the cheeks, nose, chin, and forehead. Rosacea is characterized by recurrent episodes of flushing or transient erythema, persistent erythema, phymatous changes, papules, pustules, and telangiectasia. The eyes may also be involved. Due to rosacea affecting the face, it has a profound negative impact on quality of life, self-esteem, and well-being. In addition to general skin care, there are several approved treatment options available for addressing these features, both topical and systemic. For some features, intense pulse light, las

Acne: What's New.

Acne vulgaris is one of the most prevalent skin conditions. Antibiotics, when considered, are most effective in combination with other therapies, and limited evidence suggests that submicrobial doses of antibiotics may improve acne without increasing the risk for antibiotic resistance. A small but significant risk for inflammatory bowel disease has also been identified in children treated with multiple courses of antibiotics. New topical agents are expanding therapeutic options for acne. Semin Cutan Med Surg 35(supp6):S114-S116.

Plasma and urine dimercaptopropanesulfonate concentrations after dermal application of transdermal DMPS (TD-DMPS).

2,3-Dimercaptopropane-1-sulfonate (DMPS) is a metal chelator approved in Europe for oral or intravenous use for heavy metal poisoning. Transdermally applied DMPS (TD-DMPS) is used by some alternative practitioners to treat autism, despite the absence of evidence for its efficacy. We found no literature evaluating the pharmacokinetics of the transdermal route of delivery or the ability of TD-DMPS to enhance urinary mercury elimination. We hypothesized that TD-DMPS is not absorbed. Eight adult volunteers underwent application of 1.5-3 drops/kg of TD-DMPS. Subjects provided 12-h urine collections

What is the role of field-directed therapy in the treatment of actinic keratosis? Part 2: Commonly used field-directed and lesion-directed therapies.

Actinic keratosis (AK) constitutes the initial epidermal lesion in a disease continuum that may potentially progress to invasive squamous cell carcinoma (SCC). A number of treatment options are available to clear lesions and thus reduce the risk for progression to SCC. Field-directed therapies are primarily used to clear multiple AKs and subclinical lesions. Part 1 of this review explaining the role of field-directed therapy for the treatment of AK discussed the unmet needs with current therapies and the investigational agents that are being developed to fill treatment gaps. Part 2 will mainly

[Evaluation of drug concentration in tissues after percutaneous administration of non-steroidal antirheumatics].

For some 10 years it has been usual practice to determine tissue drug concentrations following the application of topical antirheumatic agents. However, testing the measured drug concentrations for their therapeutic relevance continues to be difficult. An appropriate reference parameter is, for example, the IC50 of the inhibition of the prostaglandin synthesis. Moreover, tissue concentrations should be compared with those achieved following oral administration of clinically effective doses. Based upon practical experience with ibuprofen (CAS 15687-27-1) following both topical and oral administ

The effect of estradiol on the production of melatonin in postmenopausal women.

The influence of two routes of estradiol administration on pineal melatonin production in postmenopausal women was investigated. Both transdermal and oral estradiol treatment led to an increase as well as decrease of melatonin production in different patients. The reason why individuals respond either in a stimulatory or inhibitory manner is unknown and requires to be evaluated in further more extensive studies.

Life-threatening rocuronium-induced anaphylactic shock without cutaneous manifestations successfully reversed with sugammadex: a case report.

BACKGROUND: Recognition of rocuronium-induced anaphylaxis is often challenging, owing to its diverse clinical manifestations. Regarding treatment, several reports have described the efficacy of sugammadex, while conflicting reports have also been published. CASE: A 71-year-old man was scheduled to undergo split-thickness skin grafting surgery on his hip. During the induction of general anesthesia, the patient developed profound circulatory collapse without any cutaneous manifestations, which required 40 min of cardiopulmonary resuscitation. Later, the patient developed circulatory collapse ag

Successful Treatment of Primary Cutaneous Peripheral T-Cell Lymphoma Presenting Acquired Ichthyosis with Oral Bexarotene Monotherapy.

Acquired ichthyosis (AI) is a reactive cutaneous manifestation that can be associated with malignant hematological disease, including cutaneous T-cell lymphoma (CTCL). Since it is difficult to distinguish AI from ichthyosiform mycosis fungoides, to select the treatment for CTCL with ichthyosis-like appearance and to evaluate its efficacy is sometimes challenging. In this report, we describe a case of primary cutaneous peripheral T-cell lymphoma not otherwise specified presenting AI successfully treated with oral bexarotene. In the present case, the administration of oral bexarotene was not onl

Cutaneous zygomycosis.

The prevalence of cutaneous and soft tissue zygomycosis appears to have increased in recent years. We reviewed 78 case reports of cutaneous zygomycosis published from 2004 through 2008. Most patients with cutaneous zygomycosis have underlying conditions such as haematological malignancies, diabetes mellitus or solid organ transplantation, but a large proportion of them are immunocompetent. Trauma is the most common predisposing factor leading to zygomycosis in immunocompetent patients. If the patient is immunocompromised, the infection may disseminate. Cutaneous zygomycosis may be localized, m

Severe chickenpox after intranasal use of corticosteroids.

Two children were hospitalized for severe chickenpox after intranasal use of corticosteroids for chronic sinusitis. One had unusually extensive cutaneous disease with delayed progression of lesions, dehydration, and prolonged fever; the other had hemorrhagic cutaneous lesions, hepatitis, and pneumonitis. Both were treated with intravenous acyclovir infusion and recovered. Systemic or local immunosuppression after intranasal corticosteroid administration may have predisposed the children to severe varicella infection.

Management of Mycosis Fungoides with Topical Chlormethine/Mechlorethamine Gel: A Columbia University Cutaneous Lymphoma Center Experience.

Mycosis fungoides is a type of cutaneous T-cell lymphoma, which accounts for the majority of cases of cutaneous T-cell lymphoma. Mycosis fungoides can be classified as early-stage (IA-IIA) or late-stage (IIB or greater) disease. In early-stage mycosis fungoides, skin-directed therapies are commonly used to manage the disease. Chlormethine, or mechlorethamine, is a topical chemotherapeutic, which has been in use for over 60 years. In 2013, the US Food and Drug Administration approved chlormethine/mechlorethamine gel (Valchlor®) for treatment of stage IA and IB mycosis fungoides. Chlormethine/me

Oral and Topical Janus Kinase Inhibitors in Patients With Cutaneous T-Cell Lymphoma: A Real-World Single-Center Experience.

The JAK/STAT pathway is implicated in the pathogenesis of cutaneous T-cell lymphoma (CTCL), with early studies suggesting that JAK inhibitors (JAKi) may be used for treating CTCL. Patients with CTCL may have other indications for JAKi use, but data on JAKi agents in CTCL is limited due to warnings against their use in malignancy. We retrospectively characterized the experience of our tertiary cancer center with JAKi in patients seen for CTCL between the years 2011 and 2024. We identified 26 CTCL patients who received a JAKi: 7 received a JAKi prior to their CTCL diagnosis (topical (n = 5) or o

[Acute renal failure following mucosal administration of povidone iodine].

INTRODUCTION: Povidone iodine is a widely-used antiseptic agent, especially for cutaneous lesions. Despite its apparent innocuousness, some cases of acute renal failure are reportedly due to iodine toxicity. CASE: We report a case of an acute renal failure secondary to povidone iodine exposure in a 37-year-old woman. She underwent a hysteroscopy for diagnosis of primary sterility, and povidone iodine was used as the contrast agent. She developed acute renal failure with oliguria during the postoperative period. Treatment with diuretics and hemodialysis led to a favorable outcome and return of
