[A case of advanced gastric cancer with direct invasion of the transverse colon responding to paclitaxel/5'-DFUR combined therapy].

A 50-year-old man was diagnosed with non-resectable scirrhous gastric cancer of antrum accompanied with colon ileus due to direct invasion of the transverse colon. As the ileus improved after cecostomy, chemotherapy with TS-1/cisplatin(CDDP) was performed. Because of no response, 4 cycles of paclitaxel (PTX)/doxifluridine (5'-DFUR) therapy was performed as second-line chemotherapy. Since the stenosis of transverse colon dilated completely and the tumor disappeared, we performed total gastrectomy and right hemicolectomy, and could resect completely. Though 2 cycles of PTX/5'-DFUR therapy was pe

Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

Advanced oxidation processes for 2,4-dichlorophenoxyacetic acid in aqueous media: evidence gaps, mechanistic insights and emerging directions.

The herbicide 2,4-dichlorophenoxyacetic acid (2,4-D) remains a relevant aquatic contaminant because parent-compound disappearance does not imply mineralization, detoxification or treatment readiness. This systematic review synthesizes recent evidence on advanced oxidation processes (AOPs) and closely related chemically coupled routes for aqueous 2,4-D treatment. Records from Scopus, Web of Science and Google Scholar within a documented 2020-2025 review window were screened using explicit eligibility criteria and assessed with an evidence-completeness rubric covering key reporting domains for c

Targeting the PI3K/AKT/mTOR signaling pathway in prostate cancer: Molecular dysregulation, therapeutic advances, and future directions.

Prostate cancer (PCa) is among the most common malignancies and remains a leading cause of cancer-related mortality in men worldwide. One of the main drivers is the dysregulation of the downstream signalling machinery. The PI3K, AKT, and mTOR signalling pathways play a pivotal role in cellular sustenance, growth, metabolism, and proliferation. In prostate cancer this pathway is generally altered due to the mutation or deletion of the PTEN (phosphatase and tensin homolog) gene and unnecessary activities of some components of PI3K, AKT, or mTOR. Extracellular communication of the androgen recept

Emerging target antigen landscape for CAR T-cell therapy in solid tumors: current advances and future directions.

Chimeric antigen receptor (CAR) T-cell therapy has achieved remarkable clinical success in hematologic malignancies but remains largely ineffective in solid tumors due to antigen heterogeneity, immune evasion, and dose-limiting toxicities. A central challenge is the identification of optimal target antigens that balance tumor specificity with therapeutic efficacy. In this review, we define the emerging antigenic landscape for CAR T-cell therapy in solid tumors through integrative curation and systems-level analysis. We reviewed 58 candidate targets spanning tumor-associated surface molecules,

Unveiling Advances in Membrane Materials for CO2 Separation and Direct Air Capture (DAC): From Membrane Design to Applications.

The increase in carbon dioxide (CO2) concentration in the atmosphere has resulted in adverse and irreversible effects in terms of climate change and global warming. To limit the temperature rise to less than 2 °C by the end of this century, it is urgent to reduce the CO2 concentration in the atmosphere. Direct air capture (DAC) is considered a carbon-negative emission technology which could efficiently remove CO2 from air. Membrane gas separation is a promising technology for CO2 capture, owing to its higher energy efficiency, greater scale-up ability, and smaller carbon footprints compared wi

Medical treatment for diabetic acute kidney disease from 2012 to 2024: advances, prescription trends, and future directions.

Diabetic acute kidney disease (AKD) represents a pivotal transitional phase between acute kidney injury (AKI) and chronic kidney disease (CKD), carrying a high risk of progression to end-stage kidney disease. Effective medical management is crucial during this phase. Renin-angiotensin-aldosterone system inhibitors remain the foundation of therapy, and newer agents have expanded the armamentarium. Sodium-glucose cotransporter-2 inhibitors and glucagon-like peptide-1 receptor agonists have been shown to confer significant nephroprotective and cardioprotective effects, including decreased risks o

The Impact of Preinjury Advanced Directives Among Critically Ill Trauma Patients With Disseminated Cancer Undergoing Withdrawal of Care.

INTRODUCTION: Advanced directives (ADs) clarify patients' end-of-life preferences while preventing escalations of care against their wishes. This is especially important for critically ill trauma patients, who may be unable to communicate their preferences. National Comprehensive Cancer Network guidelines recommend all patients with disseminated cancer should discuss ADs after diagnosis. However, the impact of ADs in trauma patients with disseminated cancer who undergo withdrawal of life-sustaining treatment remains unclear. This study aimed to evaluate this relationship, hypothesizing patient

Cell-Based Therapies for Cardiac and Vascular Regeneration in Cardiovascular Disease: Recent Advances, Translational Barriers, and Future Directions.

Cardiovascular diseases (CVDs) remain the foremost cause of death globally, responsible for 19.2 million deaths and 437 million disability-adjusted life years in 2023, with prevalent cases having more than doubled since 1990. No approved therapy restores myocardium lost to infarction. The adult heart replaces cardiomyocytes at approximately 1% per year in young adults, declining to about 0.45% per year with ageing, far below what is needed to recover the more than one billion cells destroyed by a large myocardial infarction. Cell-based regenerative strategies have been investigated for more th

Direct viral invasion and tumor-like pulmonary nodules: A fatal case of mpox in a patient with advanced HIV disease.

While mpox is typically a self-limiting zoonosis, individuals with advanced human immunodeficiency virus type 1 (HIV-1) infection are at increased risk for severe visceral complications and high mortality. We report a fatal case of fulminant mpox pneumonia in a 38-year-old male with advanced HIV-1 Infection and severe immunosuppression (CD4+ T-cell count <100 cells/µL). The patient initially presented with characteristic cutaneous lesions but rapidly progressed to dyspnea and respiratory failure. Serial chest imaging revealed diffuse, solid perivascular nodules and patchy consolidations were h

Genome-scale CRISPR screens identify PTGES3 as a direct modulator of androgen receptor function in advanced prostate cancer.

The androgen receptor (AR) is a critical driver of prostate cancer (PCa). Here, to study regulators of AR protein levels and oncogenic activity, we developed a live-cell quantitative endogenous AR fluorescent reporter. Leveraging this AR reporter, we performed genome-scale CRISPRi flow cytometry sorting screens to systematically identify genes that modulate AR protein levels. We identified and validated known AR protein regulators, including HOXB13 and GATA2, and also unexpected top hits including PTGES3-a poorly characterized gene in PCa. PTGES3 repression resulted in loss of AR protein, cell

Intranasal Drug Delivery Technology in the Treatment of Central Nervous System Diseases: Challenges, Advances, and Future Research Directions.

As population aging becomes an increasingly critical global issue, the incidence of central nervous system (CNS) diseases, including Alzheimer's disease (AD), Parkinson's disease (PD), and stroke, has risen sharply. However, the blood-brain barrier (BBB) presents a significant obstacle to the effective treatment of these CNS disorders, limiting the ability of therapeutic agents to reach the brain. In this context, intranasal drug delivery, which bypasses the BBB, has attracted considerable attention in recent years. By utilizing pathways such as the olfactory and trigeminal nerves, intranasal

Current Advances and Future Directions of Pluripotent Stem Cells-Derived Engineered Heart Tissue for Treatment of Cardiovascular Diseases.

Cardiovascular diseases resulting from myocardial infarction (MI) remain a leading cause of death worldwide, imposing a substantial burden on global health systems. Current MI treatments, primarily pharmacological and surgical, do not regenerate lost myocardium, leaving patients at high risk for heart failure. Engineered heart tissue (EHT) offers a promising solution for MI and related cardiac conditions by replenishing myocardial loss. However, challenges like immune rejection, inadequate vascularization, limited mechanical strength, and incomplete tissue maturation hinder clinical applicatio

Determinants of outcomes and advances in CD19-directed chimeric antigen receptor therapy for B-cell acute lymphoblastic leukemia.

Relapsed and refractory B-cell acute lymphoblastic leukemia (B-ALL) is an aggressive B-cell neoplasm associated with poor outcomes. Conventional multiagent chemotherapy and bispecific antibody therapy may induce remission; however, relapse rates remain high and overall survival is poor. Chimeric antigen receptor T-cell (CAR-T) therapy provides durable, deep complete remission, and long-term cures in relapsed and refractory B-ALL. However, with this new treatment modality, 10%-30% of patients do not achieve remission, and over 50% experience relapse after therapy. Currently, there are two appro
