Pathological gambling and hypersexuality due to dopaminergic treatment in Parkinson' disease.

INTRODUCTION: Prevalence of psychiatric disorders in patients suffering from Parkinson's disease varies from 12 to 90%. The most common disorder in the natural evolution of Parkinson's disease is depression. However, episodes of psychosis and hypomania are related to treatment with L-dopa and dopaminergic agents. Other recognized, although less frequent, psychiatric disorders are hypersexuality and development of certain addictive behaviors, which is compulsive gambling and overdosing of anti-Parkinson agents. CLINICAL CASE: A case is presented of a male patient diagnosed with Parkinson's Dis

Impact of Ramadan fasting on medical and psychiatric health.

Ramadan is a religious month dedicated to prayer, fasting and feasting. Recently, there has been an increased interest among healthcare providers regarding possible health-related complications as a consequence of religious fasting such as that seen during Ramadan. In , a 34-year-old female patient with a diagnosis of schizoaffective disorder, depressive type, was admitted for inpatient hospitalization to an inpatient psychiatric hospital in Buffalo, New York. The earliest date of initial diagnosis is unclear; however, the patient reports an increase in symptoms during her early twenties. Upon

Prevalence of mental disorders at admission to the penal justice system in emerging countries: a study from Chile.

BACKGROUND: Previous mental health surveys conducted in prisons within emerging countries recruited samples of all prisoners at any single point in time. However, this sampling strategy results in an overrepresentation of long-term prisoners as compared with those studies recruiting from all admissions over time. This study aimed to assess mental disorders in consecutively admitted prisoners soon after admission, in order to address service needs of people with short-term imprisonments and people at early stages of imprisonment. METHOD: Disorders were assessed in a sample of 229 male and 198

Epilepsy and concomitant obsessive-compulsive disorder.

People with epilepsy (PWE) often suffer psychiatric symptoms which can impact them more than seizures. Affective and psychotic disorders are well recognized as occurring more frequently in PWE than the general population. Less is known about obsessive-compulsive disorder (OCD) in PWE, despite it being as disabling and distressing. We sought to explore the association between epilepsy and OCD with casereports by identifying ten PWE and concomitant OCD. Demographics, seizure classification, neurological, surgical, psychiatric and psychological treatment as well as quality of life were examined.

Clinical correlates of loss of insight in bipolar depression.

INTRODUCTION: Affective state may influence insight, especially regarding mania. Nevertheless, studies have so far suggested that depression seems not to significantly impair insight. To the best of our knowledge, this study pioneers the evaluation of how insight variations in bipolar depression correlate with clinical variables. METHOD: A group of 165 bipolar patients, 52 of whom had depressive episodes according to DSM-5 criteria, were followed during a year. All patients underwent clinical assessment, and insight was evaluated through the Insight Scale for Affective Disorders (ISAD). Repea

Discrimination between schizophrenia and other psychotic conditions by clinician's difficulty in attunement: a reappraisal of the Praecox Feeling concept.

INTRODUCTION: In the 1940s, Henricus Cornelius Rümke introduced the concept of Praecox Feeling (PF), a multifaceted clinician's intuition about the nuclear essence of schizophrenia that may play a role in the diagnostic process. Many classical and contemporary psychopathologists have devoted attention to this concept and the issue of intuitive diagnosis of schizophrenia. However, so far very little empirical research was carried out on this topic. This study aimed at testing the hypothesis that the empathic failure described by Rümke as a major experiential dimension underlying the PF as measu

Symptom Profile of Psychiatric Patients With Psychosis or Psychotic Mood Disorder in Prison.

There is evidence that psychiatric patients with psychotic or manic disorders who are incarcerated suffer from the same symptoms as psychiatric patients who are treated in the community. There are also indications that their symptoms might be more severe. The aim of this study was to examine the severity of psychotic and manic symptoms, as well as to collect information about the emotional functioning of patients admitted to a prison psychiatric ward. Incarcerated patients with a diagnosis of psychotic or a manic disorder were examined with the Brief Psychiatric Rating Scale-Expanded (BPRS-E).

Clinical study of mianserin, imipramine and placebo in depression: blood level and MHPG correlations.

1. The clinical efficacy of mianserin, a tetracyclic compound, was compared with imipramine and placebo in 47 hospitalized depressed patients. 2. The clinical ratings showed that the three treatment groups improved equivalently during hospitalization. 3. Plasma levels of mianserin correlated with changes in the Hamilton Rating Scale for depression, total score and two of its factors (anxiety/somatization, and retardation) and the item 'late insomnia'. 4. Drowsiness was a more frequent side-effect among mianserin patients on day 4; no other side-effect distinguished the treatments. 5. No relati

Psychotic symptoms in combat-related posttraumatic stress disorder.

BACKGROUND: Posttraumatic stress disorder (PTSD) is known often to be comorbid with other anxiety, mood, and substance use disorders. Psychotic symptoms have also been noted in PTSD and have been reported to be more common in Hispanic veterans. However, the occurrence of psychotic symptoms, including the degree to which they are accounted for by comorbid disorders, have received limited systematic investigation. Our study objectives were to assess psychotic symptoms according to DSM-III-R criteria in patients with a primary diagnosis of combat-related PTSD and determine the associations of tho

Heidelberg Coping Scales for Delusions: psychometric evaluation of an expert rating instrument.

BACKGROUND: Coping is of substantial relevance in the treatment and course of psychiatric disorders. Standardized instruments to assess coping with psychotic symptoms, particularly delusions, are rare. The aim of this study was to develop and evaluate the psychometric properties of a new instrument to assess coping strategies in the context of delusional experiences: the Heidelberg Coping Scales for Delusions (HCSD). METHODS: Two hundred and twelve inpatients with schizophrenia spectrum disorders and affective disorders currently experiencing delusions were interviewed with the HCSD and other

A two-year longitudinal investigation of insula subregional cortical thickness and surface area in early psychosis.

BACKGROUND: Smaller insula volume is consistently observed in psychotic disorders; however, alterations in associated cortical features-thickness and surface area-are less clear. We examined insula thickness and surface area changes in early psychosis to determine whether alterations were a) present in the early stages of psychosis, b) specific to any of the insula's three cytoarchitectural subregions (agranular, dysgranular, and granular) and c) related to behaviors impacted in psychosis. METHODS: 63 non-affective early psychosis (EP) and 59 healthy comparison (HC) participants completed 2-4

A systematic review of relational-based therapies for the treatment of auditory hallucinations in patients with psychotic disorders.

BACKGROUND: Auditory hallucinations in patients with psychotic disorders may be very distressing. Unfortunately, a large proportion of individuals are resistant to pharmacological interventions and the gold-standard cognitive-behavioral therapy for psychosis offers at best modest effects. To improve therapeutic outcomes, several therapies have been created to establish a relationship between voice-hearers and their voices. With increasing literature, we conducted a systematic review of dialogical therapies and examined the evidence behind their efficacy. METHODS: A systematic search was perfo

Polysomnographic Parameters in Schizoaffective Disorder: A Systematic Review and Meta-Analysis.

BACKGROUND: Schizoaffective disorder (SZA) is a severe psychiatric condition characterized by overlapping affective and psychotic features; however, its sleep architecture remains poorly characterized. This systematic review and meta-analysis compared polysomnographic findings in drug-free patients with SZA with those of healthy controls (HC), patients with schizophrenia (SCZ), and patients with major depressive disorder (MDD). METHODS: An extensive database search identified 40 studies. Nine case-control studies including 79 patients with SZA, 88 patients with SCZ, 79 HC, and 131 patients wi

Linking peripheral low-grade inflammation and blood-cerebrospinal fluid barrier leakage in schizophrenia spectrum disorders - A retrospective analysis.

BACKGROUND: Social impairments and low-grade inflammation (LGI) are associated with psychotic disorders (e.g. schizophrenia spectrum disorders). Social impairments are important symptoms of the disease nonetheless a disturbed social interaction and inflammatory processes are further discussed as being part of the underlying pathophysiology, which is also characterized by blood-cerebrospinal fluid barrier (BCSFB) dysfunction. The relationship between social impairments, peripheral LGI (pLGI) and BCSFB permeability in psychotic disorders, however, is poorly understood. Therefore, we hypothesized
