Imaginal, sensory, and cognitive experience in spontaneous recovery from alcoholism.

Although alcoholism is often regarded as an intractable disorder that requires intensive treatment, studies of the natural history of alcoholism indicate that unaided, spontaneous recovery may be the most common pathway to remission from alcoholism. Negative environmental consequences of alcoholic drinking have been invoked to explain spontaneous recovery, but a more compelling reason for sudden changes in drinking behavior concerns shifts in the personal meanings surrounding alcohol use. Extensive interviews in a multimodal format were conducted with two groups of alcoholics: one group compri…

[Clinical variants of neuropsychological disorders during remission in chronic alcoholism].

To study the psychopathological characteristics of remissions in alcoholism, 142 patients aged 32 to 45 years were examined. The anxious hypochondriac, neurasthenia-like, asthenic-depressive, anxious suspicious, anxious depressive, excitable, euphoric, apathetic, dysphoric syndromal variants are described. Correlation of the indicated syndromal variants with the other clinical characteristics of alcoholism made it possible to distinguish a favourable and relatively unfavourable type of remission, providing a possibility of differentiating between the treatment and rehabilitation measures in pa…

Alcoholism, hyponatraemia and central neurological damage: more than pontine myelinolysis?

Three patients with alcoholism and severe hyponatraemia are described. Permanent neurological damage occurred in each case with cerebral, cerebellar or pontine damage from infarction or haemorrhage following correction of the biochemical disturbance. No patient developed Central Pontine Myelinolysis (CPM), the condition usually associated with profound hyponatraemia and its correction.

Oxidative metabolism in muscle mitochondria from patients with chronic alcoholism.

We investigated the effect of long-term ethanol intake on the function of skeletal muscle mitochondria from 30 human alcoholics. Mitochondrial studies included (1) oxidative phosphorylation, (2) the activity of the individual complexes of the respiratory chain, and (3) the cytochrome content. State 3 and state 4 oxidation rates with glutamate-malate, succinate, tetramethyl-p-phenylenediamine-ascorbate, and acetylcarnitine-malate, the activity of the individual complexes of the respiratory chain, and the concentration of mitochondrial cytochromes were comparable in control subjects and alcoholi

[Characteristics of chronic alcoholism in patients with sensitive premorbid states].

Overall 69 patients with sensitive traits of character not reaching the degree of psychopathy, suffering from stage II chronic alcoholism were examined on a clinical basis. In the given patients' group, alcoholism manifested itself atypically. The initial lack of the protective nauseous and vomiting reflex and high tolerance, relative preservation of the quantitative control, no amnesia during drunkenness, temporal break between the formation of compulsive addiction and the abstinent syndrome, manifestation of compulsive addiction only in a state of alcoholic intoxication were recorded. At the…

Central pontine myelinolysis associated with low potassium levels in alcoholism.

Two patients with chronic alcohol abuse and central pontine myelinolysis are described. One developed a Korsakoff syndrome 2 days before admission to our hospital and the other showed signs of a incipient delirium without Korsakoff syndrome. Diagnosis of incipient central pontine myelinolysis was based on acute brain-stem dysfunction, serum electrolyte disturbances, malnutrition with vitamin B1 (thiamine), B6 (pyridoxine) and B12 (cyanocobalamin) deficiency in combination with typical neuroradiological findings. Hypokalaemia but no disturbance in serum sodium levels was found in both patients.

MR imaging of white matter lesions in uncomplicated chronic alcoholism.

Chronic alcoholics may have CNS lesions, such as microvasculitis or glial, neural, and myelin degeneration, as documented in postmortem studies on subjects who had Wernicke encephalopathy, corpus callosum degeneration, or central pontine myelinolysis. One may also expect the presence of early white matter disease in patients who do not have neurologic complications of alcoholism. Thirty-five chronic alcoholics (Diagnostic and Statistical Manual III criteria) and 35 normal control subjects were studied by means of magnetic resonance (MR) imaging. Subjects greater than 60 years old, or those wit…

Attitudes of mental health professionals toward alcoholism recognition and treatment.

The attitudes of 100 urban mental health professionals toward alcoholism diagnosis and treatment were assessed using a 23-item questionnaire. A significant number of those sampled, physicians more than other professional groups, looked upon alcoholism as a disease in its own right. Physicians, attendings more than residents, subscribed to a more medical history-taking model. While the majority of those surveyed would advise complete sobriety, a large number would not. Likewise, despite a large percentage of our sample who indicated familiarity with alcohol-related concepts, a significant numbe…

Detection of blood benzodiazepines in injured people. Relationship with alcoholism.

In 2021 accident victims (household, work and road accidents) the qualitative test for benzodiazepines in the serum by the EMIT method was positive in 9.6% of cases, including 3.2% of subjects who also had a blood alcohol level greater than 0.10 milligram. Benzodiazepines were detected with a particularly high frequency in household accidents and road accidents and rarely in work accidents. These results raise the problem of the role of benzodiazepines as a possible risk factor for accidents, particularly road accidents.

Treatment of alcoholism in Kuwait: a prospective follow-up study.

The outcome of routine drug-supportive psychotherapy treatment of 100 patients with alcohol-related problem in Kuwait's only psychiatric hospital was evaluated in four 6 month's intervals. Patients were assessed on 9 post-treatment questions reflecting improvement or deterioration. Correlation of follow-up changes to pre-treatment data shows that non-Kuwaitis do better than Kuwaitis in resuming employment and social reintegration. Patients on chronic benzodiazepine prescription were less likely to become abstinent, had poor social re-integration and more re-admissions. Good relationships with

Treatment of alcoholism: the role of the primary care physician.

The primary care physician plays an important role in early identification of the alcoholic, who may seek treatment for a problem other than alcoholism. Confrontation begins with a complete history and ends with an invitation to the patient to participate in definitive treatment. During detoxification the patient can be helped to establish a long-term relationship with the physician and with Alcoholics Anonymous.

Alcoholism and alcoholic liver injury: new diagnostic and prognostic tests.

Recently developed tests that measure levels of alpha-amino-n-butyric acid (AANB) and serum glutamic dehydrogenase (GDH) may improve screening for early detection of heavy drinking and liver injury, respectively. With these tests, a "three-level" approach to the problem is now possible: (1) detection of heavy drinking on the basis of a biochemical marker (such as AANB); (2) detection of liver injury (necrosis and inflammation) on the basis of serum liver tests (such as GDH); and (3) detection of alcoholics in whom cirrhosis is prone to develop by the screening of liver biopsy specimens for pre

Metabolic Acidosis in Chronic Alcoholism: When It's Not Just Diabetic Ketoacidosis.

Metabolic acidosis with ketosis is a medical emergency. Due to divergent management strategies, prompt differentiation is required between diabetic ketoacidosis (DKA), alcoholic ketoacidosis (AKA), and euglycemic diabetic ketoacidosis (euDKA). We present the case of a 28-year-old woman with heavy alcohol use who presented with severe metabolic acidosis, ketosis, and hyperglycemia who was initially treated for DKA. Unexpected lab discrepancies in the course of treatment including hypoglycemia despite dextrose infusion raised suspicion for AKA. Elevated C-peptide, normal HbA1c, and absent anti-i
