From 1978 until 1993 we diagnosed 167 cases of inhalative allergy to silk-filled bed quilts. The patients experienced asthma attacks predominantly during the night. Occupational exposure to silk materials was found in six cases. The mean age at first manifestation of symptoms (31 years, range 8 to 58) was relatively high, indicating an aggressive inhalation allergen. Rhinitis causes both clinical and social discomfort to patients, and in clinical practice is often underdiagnosed. We have examined a simple method for the assessment of a positive nasal provocation test to help in the diagnosis of rhinitis. In patients with histories suggestive of house dust mite (HDM) sensitivity and positive skin-prick tests or specific IgE to Dermatophagoides pteronyssinus, there was a fall in nasal inspiratory peak flow (NIPF) following nasal challenge with allergen. A metabolite of Sudan I (1-phenylazo-2-naphthol) was isolated after in vitro incubation with the supernatant of guinea pig liver homogenate (S-9). The metabolite was found to be 4'-hydroxy-1-phenylazo-2-naphthol by gas chromatography-mass spectrometry (GC-MS), and proton and carbon-13 nuclear magnetic resonance (NMR) analyses. It elicited positive reactions in guinea pigs sensitized to Sudan I. It was also shown to be allergenic. The results suggest that para-hydroxylation of the phenyl group of Sudan I may play an important rôle in its allergenicity.

Twenty-six pollen-allergic subjects participated in a double-blind, placebo-controlled trial of the protective effect of the calcium antagonist, verapamil, on allergen-provoked nasal symptoms. 79 children with classical collagenoses, streptococcaldiseases, arthralgia of unclear genesis and infection arthritis as well as juvenile rheumatoid arthritis and 25 normal children were submitted to an intracutaneous allergentest with various collecting extracts (legumens and cereals, house allergens, vegetables). Positive reactions to cereals in a high percentage were observed only in patients with juvenile rheumatoid arthritis and fewer also in children with unclear arthralgia and infection arthritis.

The allergenic potencies of samples of two commercial Alternaria tenuis extracts stored under various conditions were compared at 3 months intervals over a period of 1 year. Patch test reactions to 11 common contact allergens were studied after 24 h and 48 h occlusion with Finn Chambers in 390 patients. Concordant allergic results were found in 96 cases (74%). In 22 patients (17%), the reaction was positive only after 48 h and in 11 cases (8.5%) only after 24 h exposure.
