A review of surgical procedures designed to increase the functional height of the resorbed alveolar ridge.

By raising the alveolar ridge, relatively or absolutely, in partially or totally edentulous patients, preprosthetic surgery makes possible an improvement in the bed for the denture. Raising the height of the ridge, relatively, by deepening the sulci surrounding the jaw, can be achieved by vestibuloplasty and grafts to the floor of the mouth. However, for these procedures it is essential that there still be sufficient bone (at least 20 mm in the mandible). Long term observations show that the main problem is a reduction in the vestibular depth initially obtained by surgery. The most durable res

[Implant-borne prosthesis for an edentulous maxilla with a large maxillectomy defect--a step-wise therapy concept].

Due to the great variety of maxillectomy defects standardized restorative treatment protocols are rarely documented. For the practitioner it is from highest importance to consider the specific defect morphology and, even more important, the individual needs of affected patients. The presented case shows the planning and realization of the restoration of a maxilla with an extended resection defect following tumor surgery. Because the demanding patient remained edentulous after extraction of the residual teeth for periodontal reasons, an implant retained obturator prosthesis was inserted. Follow

Prevalence, diameter and course of the maxillary intraosseous vascular canal with relation to sinus augmentation procedure: a radiographic study.

The aim of the study was to characterize the prevalence, diameter and course of intraosseous anastomosis between the posterior superior alveolar artery and the infraorbital artery (bony canal) involved in the sinus floor augmentation procedure. Data from 208 sinuses were analyzed from reconstructed computed tomography (CT) images. The presence of the intraosseous anastomosis in the lateral antral wall was detected using sagittal plane sections, in addition, the intraosseous course and the diameter of the bony canal were examined. The bony canal was identified in 114 (55%) of the 208 maxillary

Dentofacial rehabilitation by osteodistraction, augmentation and implantation despite osteogenesis imperfecta.

Osteogenesis imperfecta-- a heritable systemic disorder characterized by enhanced bone fragility-- is frequently associated with a Class III malocclusion and distinct dental disorders. This patient, suffering from a late form of osteogenesis imperfecta, displayed early loss of teeth and severe maxillary hypoplasia. Bone grafting of the alveolar ridge was assumed not to guarantee a neutral basal relation before dental implantation. Due to the risk of atypical fractures conventional orthognathic surgery was excluded in the atrophic maxilla. In contrast to a conventional Le Fort I osteotomy, oste

Sinus Elevation Through Transcrestal Window Approach and Delayed Implant Placement in 1- to 2-mm Residual Alveolar Bone: A Case Report.

Sinus augmentation with a lateral window technique and delayed implant placement are the traditional treatment schedules when the residual alveolar bone height in atrophic posterior maxilla is less than 5 mm. This article presents a transcrestal window approach (TWA) to replace a missing first molar in an atrophic posterior maxillary ridge with 1 to 2 mm of residual bone height (RBH). Transcrestal sinus augmentation and delayed implant placement were performed. The antral pseudocyst that was located in the nasal wall of the maxillary sinus disappeared in the cone-beam computer tomography image

Use of cell-based approaches in maxillary sinus augmentation procedures.

The dimension of alveolar ridge is decreased by bone atrophy and pneumatization of the maxillary sinus after loss of teeth in the posterior maxilla, and sinus augmentation procedures are performed to create bone quantity and quality to ensure the placement of dental implants.Various osteoconductive materials have been used to augment the sinus floor, but these materials are cell-free and require more time for bone healing. Attempts have been made to apply a cell-based approach that uses mesenchymal stem cells combined with an osteoconductive scaffold. Adult stem cells that can be derived from

Does alveolar ridge preservation reduce the need for sinus floor elevation: A comparative study to spontaneous healing.

INTRODUCTION: In cases of atrophy in the maxillary posterior region, characterized by reduced vertical bone volume, implant placement becomes challenging. Augmentation procedures like sinus lifts are often needed to address insufficient bone volume. This study aims to explore if alveolar ridge preservation, using a bovine bone substitute and a porcine collagen membrane, significantly decreases the need for sinus lifts compared to natural wound healing after tooth extraction. MATERIALS AND METHODS: In this comparative clinical study, 40 patients requiring a total of 53 extractions were assigne

Polyphosphate-crosslinked collagen scaffolds for hemostasis and alveolar bone regeneration after tooth extraction.

UNLABELLED: Post-extraction bleeding and alveolar bone resorption are the two frequently encountered complications after tooth extraction that result in poor healing and rehabilitation difficulties. The present study covalently bonded polyphosphate onto a collagen scaffold (P-CS) by crosslinking. The P-CS demonstrated improved hemostatic property in a healthy rat model and an anticoagulant-treated rat model. This improvement is attributed to the increase in hydrophilicity, increased thrombin generation, platelet activation and stimulation of the intrinsic coagulation pathway. In addition, the

Patient-Related Risk Factors for Maxillary Sinus Augmentation Procedures: A Systematic Literature Review.

The posterior maxilla has traditionally presented a challenge for successful placement of dental implants due to a combination of poor bone quality, ridge atrophy, and pneumatization of the sinus floor following tooth extraction. However, with the successful and predictable surgical outcomes reported in the literature, more clinicians and patients are choosing an implant-supported restoration in the edentulous posterior maxilla. Consequently, sinus elevation and augmentation have gained more popularity. Extensive research has been conducted on types of bone graft materials and implants, less-i

Impact of Bone Particle Size on Alveolar Ridge Preservation in Periodontally Compromised Molar Extraction Sites.

INTRODUCTION AND AIMS: The clinical efficacy of alveolar ridge preservation (ARP) is well-documented in the literature. However, there is a lack of evidence regarding ARP in the posterior area and in periodontally compromised sites. There is no clinical decision-making guideline on the choice of bone substitute particle size for ARP in periodontally compromised molar sockets. This study aimed to compare the radiographic outcomes and implant feasibility of two deproteinized bovine bone mineral (DBBM) particle sizes for alveolar ridge preservation (ARP) in periodontally compromised molar extract
