BACKGROUND: Sickle cell anemia (SCA) increases the rate of maternal and fetal complications. This pilot study was designed to compare the maternal and fetal outcomes of spinal versus general anesthesia (GA) for parturients with SCA undergoing cesarean delivery.
METHODS: Forty parturients with known SCA scheduled for elective Cesarean delivery were randomized into spinal anesthesia (n = 20) and GA groups (n = 20). Perioperative hemodynamic parameters were recorded. Postpartum complications were followed up. Opioid consumption was calculated. Blood loss during surgery and the number of patients

BACKGROUND: Transanal minimally invasive surgery (TAMIS) for rectal tumors has been introduced as an alternative approach to transanal endoscopic microsurgery (TEM). TEM has some limitations, such as the need for special equipment, expensive cost, and steep learning curve. In this study, we address the technical feasibility of TAMIS under spinal anesthesia and its short-term postoperative outcomes.
METHODS: From July 2011 to September 2012, 25 consecutive patients with middle or upper third rectal masses underwent TAMIS. Tumors were located 6-17 cm from the anal verge. After spinal anesthesia

This study was performed to compare the anesthetic efficacy and safety of three local anesthetic agents: racemic bupivacaine and its two isomers: ropivacaine and levobupivacaine, in patients undergoing lower abdominal surgery. One hundred-twenty patients, ASA I-III, were randomized to receive an intrathecal injection of one of three local anesthetic solutions. Group A (n = 40) received 3 ml of isobaric bupivacaine 5 mg/ml (15 mg). Group B (n = 40) received 3 ml of isobaric ropivacaine 5 mg/ml (15 mg). Group C (n = 40) received 3 ml of isobaric levobupivacaine 5 mg/ml (15 mg). The onset and dur

UNLABELLED: We report a case of fatal gas embolism during transurethral incision of the bladder neck under spinal anesthesia in a 76-yr-old man. We confirmed the diagnosis of venous gas embolism by aspiration of frothy blood through the double-lumen central venous catheter and by observation on the transesophageal echocardiogram of a massive gas embolism in the right atrium and right ventricle with obstruction of right ventricle outflow. This report is presented in an attempt to remind anesthesiologists of this unusual but potentially fatal complication that may occur during common transurethr

BACKGROUND: Hypertonic saline improves organ perfusion and patient survival during hemorrhagic shock because it expands plasma volume and increases tissue oxygenation. Its beneficial results have been reported in patients suffering from hypotension during spinal anesthesia. The purpose of this study was to compare the influence between prehydration with 3% hypertonic saline and with isotonic lactated Ringer's solution on the hemodynamic changes and serum electrolyte concentrations in patients undergoing spinal anesthesia.
METHODS: Sixty ASA class I patients scheduled for herniorrhapy under sp

We examined the usefulness of 10 degrees head-down tilt for hypotension after spinal block. Two different investigations were performed, one employing head-down tilt after arterial blood pressure had decreased (n = 40), and the other using a prophylactic tilt (n = 50). When the head-down tilt was applied to treat hypotension after spinal block (n = 40), arterial blood pressure increased only in patients whose systemic blood pressure decreased more than 30% from the control (severe hypotension group, n = 11). However, even in the severe hypotension group, systolic blood pressure did not increas

Most of the lower leg surgeries are done under spinal anesthesia. This 53 year old male had undergone a surgery for his left hip fracture previously and was scheduled for the removal of the screw. Spinal anesthesia was administered and sensory block was obtained up to T8. After the removal of the screw, he coughed weakly for several times. Suddenly ECG monitor showed severe bradycardia and hypotension was observed. He complained of chest pain, dyspnoea and went into shock. Immediately after giving atropine 0.5 mg iv, ephedrine 8 mg x 2 was necessary to raise his heart rate. About 3 minutes lat

Background Hypotension following spinal anesthesia is common and may adversely affect end-organ perfusion. Perfusion index (PI), derived non-invasively from pulse oximetry, reflects peripheral vascular tone and may help identify patients at risk of post-spinal hypotension. This study aimed to determine the diagnostic threshold of baseline PI for predicting hypotension following spinal anesthesia in adults undergoing elective below-umbilical surgery and to assess its diagnostic performance. Methods This prospective observational diagnostic accuracy study included adult patients aged 18-65 years

The Sharon operation is useful for recurrent sigmoid volvulus in high-risk patients because of its minimal invasiveness and short operative time. We report the case of a patient with recurrent sigmoid volvulus who was at high risk during general anesthesia and successfully underwent the Sharon operation under spinal anesthesia. This case suggests that spinal anesthesia may be a feasible option for selected patients undergoing this procedure.

The rising prevalence of obesity, including among pregnant patients, presents challenges for anesthetic management during cesarean sections. We report the case of a parturient with class III obesity (BMI > 60) undergoing elective cesarean section successfully managed with single-shot spinal anesthesia. This case report supports the feasibility of single-shot spinal anesthesia in parturients with class III obesity and emphasizes the importance of rapid recognition and management of potential complications.

INTRODUCTION: Chloroprocaine is a rapid, short-acting spinal anesthetic that may facilitate quicker postoperative recovery in total knee arthroplasty (TKA). However, limited literature exists regarding its use in patients undergoing same-day discharge (SDD) TKA. This study evaluated the clinical outcomes and safety of chloroprocaine compared with those of mepivacaine.
METHODS: This retrospective study of 178 patients who underwent primary TKA at a single ambulatory surgery center from March 2022 to June 2023 compared chloroprocaine (n = 114) and mepivacaine (n = 64) spinal anesthesia. Surgica
