Helicobacter pylori infection and serum pepsinogen I concentration in peptic ulcer patients: effect of bacterial eradication.

OBJECTIVES: In order to test the hypothesis that H. pylori infections in the gastric antrum increase pepsinogen I release, fasting serum pepsinogen I concentrations were compared in peptic ulcer patients with and without H. pylori infection. A randomized prospective study was performed to determine whether the increased serum pepsinogen I concentrations associated with H. pylori infection respond to treatment that eradicates H. pylori. METHODS: Fasting serum pepsinogen I concentrations were measured by RIA in 736 patients with endoscopically and histologically confirmed benign peptic ulcer wi

Esomeprazole 20 mg maintains symptom control in endoscopy-negative gastro-oesophageal reflux disease: a controlled trial of 'on-demand' therapy for 6 months.

BACKGROUND: Most patients with gastro-oesophageal reflux disease (GERD), regardless of endoscopic status, suffer symptomatic relapse within … zole 20 mg or placebo on demand (maximum of one dose per day) for a further 6 months. Use of rescue antacids was permitted. RESULTS: All 342 patients (191 males), aged 19-79 (mean 49) years, were evaluable in the intention-to-treat analysis. The proportion of patients who discontinued treatment due to insufficient control of heartburn was significantly hi …

Urinary catheter blockage depends on urine pH, calcium and rate of flow.

Urinary catheters tend to block when biofilm from urease-producing organisms build up on the catheter surface. This is a locally-occurring p … . Other avoidable risk factors include: excess dietary calcium from certain protein supplements and antacids; excess dietary magnesium from certain beverages and antacids; alkali from effervescent tablets; excess dietary citrate from some fruit juices and cordials; intermittent dehydration from alcohol ingestion. Less tractable risk factors include infecti …

Nonprescription H2-receptor antagonists.

OBJECTIVE: This article reviews the use of nonprescription histamine2-receptor antagonists (H2RAs) in the management of secretory disorders of the upper gastrointestinal tract. DATA SOURCES: The scientific and consumer literature pertaining to the over-the-counter availability and therapeutic uses of H2RAs was reviewed. STUDY SELECTION: References related to safety, efficacy, comparative efficacy with antacids, and economic impact of H2RA therapy were emphasized. Additional importance was placed on information describing the pharmacist's role in nonprescription H2RA therapy. DATA EXTRACTION…

The role and limitations of H2-receptor antagonists in the treatment of gastro-oesophageal reflux disease.

Gastro-oesophageal reflux disease (GERD) occurs in up to 44% of adults in the USA. Most individuals do not seek medical help, self-medicating with antacids. Manifestations of GERD range from symptoms without oesophagitis, which constitute the bulk of patients who self-medicate, to active oesophagitis and then to complications such as stricture and ulceration. It is the more severe cases who tend to come to the gastroenterologist, but it must be remembered that reflux symptoms are probably around 5-10 times more common than actual oesophagitis. Since acid in the refluxate is responsible for the…

The therapeutic strategy for peptic ulcer disease.

Therapy of acid/peptic disease has evolved since the 1970s with development of: (i) more accurate endoscopes which permit precise examination and documentation of upper gastrointestinal lesions; and (ii) the histamine H2-receptor antagonists. As well, refined standards for clinical investigation have contributed to the clinical study of acid/peptide diseases. Initially, ulcer diseases were considered to be principally secondary to increased 'aggressive' factors (acid, pepsin) and the therapeutic focus was directed at antacids, the progressive evolution of additional histamine H2-receptor antag…

[Prevention and treatment of keloids].

Keloids are pathological scars extending beyond the initial wound's natural borders. The condition occurs as a result of impaired wound healing with excessive collagen deposition. Keloid scars frequently recur, rarely regress, and have a tendency to grow over time. They are aesthetically disfiguring and can be devastating for patients, both physically and emotionally. This review finds that prevention and early intervention are essential for good results. Treatment is often challenging. There are no standardized management guidelines currently available but a combinational therapeutic approach

Complications of metabolic acidosis and alkalinizing therapy in chronic kidney disease patients: a clinician-directed organ-specific primer.

Chronic kidney disease is prevalent, affecting more than one in ten adults. In this population, metabolic acidosis is considered a key underlying pathophysiological feature, tying together bone mineral disorders, sarcopenia, insulin resistance, vascular calcification, pro-inflammatory and pro-thrombotic states. This review aims to address the paucity of literature on alkalinizing agents, a promising treatment option that has known adverse effects.
