PURPOSE: To determine the incidence of bacteremia following lacrimal-duct probing in children, and to evaluate the potential need for antibiotic prophylaxis in children who are at an increased risk of infectious endocarditis (IE).
METHODS: In a prospective study beginning in October 1994, 40 consecutive children requiring lacrimal-duct probing performed by a single pediatric ophthalmologist received preoperative lacrimal and blood cultures followed by postoperative probe-induced transient bacteremia that was defined as a negative preoperative blood culture, followed by a positive postoperativ…

Bacterial infections, especially cholangitis, are still common complications after liver transplantation (LTx). During recent years, multiresistant enterococci have become a nosocomial problem in transplant units. The present prospective study on 26 patients, including 24 patients with chronic liver disease, demonstrated that enterococci were the predominant micro-organism involved in post-LTx bacterial infections. They were cultured in the feces and in other sites of 10 out of 13 (77%) patients who underwent extensive examinations. Ampicillin-resistant Enterococcus faecium strains were isolat

Since 1973, when doxycycline was introduced as preoperative prophylaxis in elective colorectal surgery at Malmö General Hospital, Sweden, there has been an unchanged and low rate (8% to 12%) of septic complications in colonic surgery. For treatment of postoperative infections, ampicillin and cefuroxime have been used since 1973 and 1980, respectively. The sensitivity of Escherichia coli to these three antibiotics used for prophylaxis and treatment was followed for five years (1981-1985). Only minor changes were observed during the period. A lower frequency of antibiotic resistance was found in

The point of this study was to analyze the possible benefits of perioperative antibiotic prophylaxis in patients from whom oral cavity or throat tumors are removed. The criteria used to judge the efficacy of each treatment included the clinical course of the treatment, the bacterial colonization of the surgical area as well as the growth of bacteria during the postoperative phase. 50 patients were chosen and grouped according to their surgical treatment: laryngectomy (n = 20), partial laryngectomy (n = 22) or tongue, floor of the mouth, soft palate, gum or base of the tongue partial resection …

A prospective, double-blind study was performed to evaluate the comparative efficacy of single- and multiple-dose antimicrobial prophylaxis for preventing infection in high-risk patients undergoing cesarean section. One hundred fifty-eight patients were randomly assigned to receive either a single perioperative dose of mezlocillin, three doses of mezlocillin or three doses of cefoxitin. The incidence of endometritis was 5.9%, 4.0% and 4.0%, respectively. The incidence of febrile morbidity was 5.9%, 2.0% and 6.1%, respectively. These differences are not statistically significant. BACKGROUND: While surgical antibiotic prophylaxis (SAP) is a cornerstone of preventing surgical site infections, adherence to clinical guidelines determines its effectiveness. In Spain, nationwide data on SAP appropriateness are scarce. This study evaluated prescribing practices across Spanish hospitals to assess alignment with national standards and identify factors associated with inappropriate antibiotic use.
METHODS: We conducted a multicentre, cross-sectional, point-prevalence survey (PPS) in hospitals nationwide in Spain. Adult and paediatric patients undergoing elective or urgent surge…

BACKGROUND: Patients who receive piperacillin-tazobactam when undergoing pancreatoduodenectomy have reduced postoperative surgical site infection, pancreatic fistula, and sequelae of surgical site infection compared with patients who received cefoxitin, supporting broad-spectrum penicillin prophylaxis as standard care. However, evidence-based practices can take years to integrate widely, and the rate of clinical uptake after pragmatic surgical trials is undefined. Our objective was to evaluate broad-spectrum antimicrobial use in pancreatoduodenectomy after the dissemination of level I evidence

Infective endocarditis (IE) remains a severe condition associated with significant morbidity and mortality. Dental procedures involving manipulation of gingival tissue or the periapical region may cause transient bacteremia, making prevention strategies at the dental-medical interface clinically important. Current guidance emphasizes selective antibiotic prophylaxis (AP) for patients at the highest cardiac risk while prioritizing long-term oral health maintenance. This scoping review synthesizes contemporary evidence (2021-2025) and major international guidelines to clarify the relationship be…

Surgical site infections (SSI) remain among the most common post-operative complications following gynecological and obstetric surgeries. Pre-operative antibiotic prophylaxis plays a key role in preventing SSIs, with intravenous administration serving as the conventional standard. Recent interest has turned toward oral antibiotic prophylaxis as a more convenient and cost-effective alternative. Hence, this prospective comparative study included 221 women undergoing elective gynecological or obstetric procedures, randomly assigned to receive either oral antibiotics (Group A) or intravenous antib…

BACKGROUND: Surgical site infections (SSI) remain a significant concern following Type III open fractures. Recent studies have suggested that narrow-spectrum antibiotics like cefazolin (CEZ) may offer comparable efficacy to broader-spectrum combinations in preventing SSI, with potential cost benefits. However, the cost-effectiveness of different prophylactic regimens in Type III open fractures in China has not been comprehensively evaluated. METHODS: A decision-tree model was constructed for a hypothetical cohort of 1,000 adult patients with Type III open fractures in the Chinese healthcare se

BACKGROUND: Extended post antibiotics do not reduce infectious complications after traumatic facial injuries or elective clean-contaminated head and neck operation. However, the optimal duration of antibiotic prophylaxis following operative repair of traumatic neck aerodigestive injuries remains uncertain. This study compared outcomes between perioperative (≤24 hours) and extended (>24 hours) antibiotic prophylaxis and hypothesized that extended prophylaxis would not reduce local infectious complications.
METHODS: Using a Level I and II university-based trauma system registry, patients underg…

BACKGROUND: Gunshot-induced open fractures (ballistic OFs) present significant challenges in trauma management due to high risk of infection and lack of standardized antibiotic prophylaxis protocols. This study evaluates the implementation of an institutional antibiotic prophylaxis protocol for ballistic OFs at a Level I trauma center, assessing its impact on infection rates and protocol adherence.
METHODS: A retrospective analysis of patient records was conducted, including all cases of ballistic OFs treated between June 7, 2021, and March 31, 2024. Patients were stratified into pre-implemen…

Introduction and Objective: Postoperative infection and sepsis account for the most common complications following percutaneous nephrolithotomy (PCNL), as high as 14% in low-risk patients. Although the American Urological Association (AUA) recommends perioperative antibiotics for 24 hours or less for PCNL, practice patterns vary regarding duration of antibiotic therapy. We aimed to compare the efficacy of 24-hour antibiotic coverage vs short-course protocol of antibiotic prophylaxis for PCNL. Materials and Methods: Low-risk patients with a sterile preoperative urine culture undergoing PCNL wer…

Background and study aims Single-operator peroral cholangioscopy (SOC) has gained increasing attention in modern biliary and pancreatic therapy and diagnosis. This procedure has shown higher rates of infectious complications than conventional endoscopic retrograde cholangiopancreatography (ERCP); therefore, many guidelines recommend antibiotic prophylaxis (AP). However, whether AP administration decreases infectious or overall adverse events (AEs) has been little studied. We aimed to study whether AP affects post-procedure infectious or overall AEs in ERCP with SOC. Patients and methods We col…

BACKGROUND: Periprosthetic joint infection (PJI) is a common source of failure following elbow arthroplasty. Perioperative prophylactic antibiotics are considered standard of care. However, there are no data regarding the comparative efficacy of various antibiotics in the prevention of PJI for elbow arthroplasty. Previous studies in shoulder, hip, and knee arthroplasty have demonstrated higher rates of PJI with administration of non-cefazolin antibiotics. The elbow has higher rates of PJI than other joints. Therefore, this study evaluated whether perioperative antibiotic choice affects rates o

BACKGROUND: In the United States, it has been common practice to recommend that dentists provide antibiotic prophylaxis (AP) before invasive dental procedures (IDPs) to prevent late periprosthetic joint infections (LPJIs) in patients who have prosthetic arthroplasties despite lack of evidence for a causal relationship between IDP and LPJI and a lack of evidence for AP efficacy.
METHODS: A recent study quantified the IDP incidence over the 15-month period prior to LPJI hospital admissions in the United Kingdom for which dental records were available. A case-crossover analysis compared IDP inci…
