Diagnosis and Therapy of Anal Carcinoma Summary. The squamous cell carcinoma of the anorectum is rare and subdivided into perianal, anal canal and combined carcinomas. Persistent infection with a high-risk human papillomavirus (HPV) is believed to be the main cause for the development of anal cancer. Therefore, the incidence in high-risk individuals (e.g. immuno-compromised patients or patients living with HIV) is much higher than in the general population. Nevertheless, a nearly three-fold overall increase was observed within the last three decades. The diagnosis is often made by chance as an

BACKGROUND: In Japan, the standard treatment for squamous cell anal cancer(SCAC)has not been established. Herein, we report a case of SCAC that completely responded to chemoradiotherapy(CRT).
CASE: A woman in her 80s presented with anal pain and bleeding. Computed tomography revealed bilateral inguinal adenopathy and a tumor in the anal canal. Histopathological examination of endoscopic biopsies showed adenocarcinoma. Thus, she was diagnosed with anal canal adenocarcinoma and lymph node metastases:cT3, cN1a(No. 292), cM0, cStage Ⅲc(Japanese Classification of Colorectal, Appendiceal, and Anal

PURPOSE: Anal mucosal melanoma is an uncommon malignancy of the anal canal, with few large studies available to establish clear trends in the treatment modalities presently available. The primary goal of this study was to identify the patterns of care in the treatment of anal melanoma and secondarily to determine outcomes.
METHODS: This was a retrospective study performed utilizing the National Cancer Database (NCDB). A total of 787 patients diagnosed with anal melanoma between 2004 and 2014 were selected, of which 398 had staging information. The four treatment groups analyzed were surgical

Extramammary Paget's disease (EPD) is rare. We report a case of double anal canal cancers in a patient with a long history of perianal Paget's disease. The patient, a 68-year-old Japanese woman, refused surgery initially and was treated with electron beam therapy, which achieved remission. However, 6 years later, Paget's disease was found to be progressing again and double anal canal tumors were also detected in the proctos and external skin area. We performed abdominoperineal resection (Miles' operation) and lymph node dissection for the Paget's disease with double anal canal tumors. Immunohi

UNLABELLED: Although it is well established that the standard primary treatment for anal canal squamous cell carcinoma is chemoradiation, the strategy varies for recurrence cases.
CASE: A 76-year-old woman was diagnosed by biopsy as Stage II (T2N0M0) squamous cell cancer and treated with a total amount of 60 Gy pelvic radiation excluding the groin area, and oral chemotherapy of S-1 (60 mg/m2/day) for 4 weeks. Two years after initial therapy, she had a recurrence at the right inguinal lymph nodes. We resected her right inguinal lymph nodes and added 20 Gy photon radiations to both sides of the

A 74-year-old Japanese woman presented with a 3-month history of anal bleeding. Proctoscopy revealed an unusual polypoid lesion with focal pigmentation at the dentate line, which was histologically diagnosed as a malignant melanoma. Whole-body clinical and radiographic evaluations revealed no alternative primary source. Endoscopic ultrasonography (EUS) showed well-delineated hypoechoic tumors invading the muscularis propria, and magnetic resonance imaging (MRI) revealed regional lymphadenopathy. Following this evaluation, an abdominoperineal resection with regional lymphadenectomy was performe

A patient with an atypical presentation of cutaneous T-cell lymphoma is reported. For many years, there were perianal lesions only, which were initially diagnosed as a non-specific severe perianal dermatitis, possibly due to psoriasis. There was little response, however, to potent topical steroids. Further patch and plaque lesions then developed elsewhere and plaque stage mycosis fungoides was diagnosed, allowing effective treatment with superficial radiotherapy and PUVA to be given. The difficulties that may be encountered in the diagnosis of cutaneous T-cell lymphoma are discussed.

Heterotopic gastric epithelium occurs in all portions of the alimentary tract, but it is extremely rare in the rectum. The authors report the finding of a pedunculated anal polyp in a 9-year-old girl with a six-month history of rectal bleeding. Microscopically, the polyp consisted of epithelium of the fundic type. Both the age of the patient and the finding of heterotopic epithelium of the fundic type alone supports a congenital malformation and not an inflammatory condition.

A case of malignant anorectal melanoma is reported, which was treated by anorectal-sigmoid amputation, operation which did not prevent the occurrence, after 6 months, of metastases in the inguinal lymph nodes. On the basis of the reported case, the main etiological, histogenetic, prognostic and therapeutic aspects of these rare tumours are discussed.

Fewer than 100 cases of Paget's disease located in the perianal skin have been reported since the extramammary location was first described in 1893. Two patients seen and treated in the authors' institutions with disease limited to the epidermis and its adnexae are described to illustrate the usual presentation and pathobiology of the disease. A staging classification based on the disease pathology has been developed from the cases reported in the literature and correlated with appropriate surgical treatment. Stage I disease treated with wide local excision promises unlimited survival, whereas

A 59-year-old man had a pedunculate perianal tumor initially thought to be a condyloma acuminatum. Since it failed to respond to topical podophyllum resin, the growth was excised and found to be an apocrine gland tumor. Histological features included a duct extending from the surface epidermis to multiple tubules, which led to cystic spaces with papillary projections of columnar epithelium. Decapitation secretion and fibrous stromal proliferation were prominent. The clinical and histological similarities to other apocrine gland adenomas are briefly presented.
