COVID-19-induced psoriatic arthritis: a case report.

Coronavirus disease 2019 (COVID-19) is an infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) which may lead to uncontrolled immune activation and cytokine response in some. The pattern of pro-inflammatory cytokines is similar to that which has been observed to be involved in rheumatic diseases and target treatments. Viral arthritis is common with a wide variety in spectrum ranging from arthralgia to spurious and chronic arthritis. However, recent studies have demonstrated a correlation with endemic coronaviruses and increased risk of developing rheumatoid

Lest We Miss Hansen Arthritis: Case Series of Combo Articular Manifestations; One of Acute Lepra Reaction with Chronic Arthritis, and Another of Swollen Feet and Hand Syndrome besides Acute Lepra with Chronic Arthritis.

Hansen's disease is an infectious granulomatous disease with classical cutaneous and neurological profile and varied musculoskeletal manifestations. Poor awareness and low specificity of symptoms lead to a delay in the diagnosis of a treatable disease, culminating in worsening morbidity. A rare case series of Hansen's arthritis, showing complex, diverse, and rare combinations of musculoskeletal manifestations of acute lepra reaction over chronic arthritis, besides cutaneous and neurological manifestations, must be understood for early diagnosis and treatment to avoid debilitating complications

Reiter's disease.

Reiter's disease can no longer be considered a rare and relatively benign syndrome consisting of the clinical triad of urethritis, conjunctivitis and arthritis. It is, in fact, a post-infectious reactive arthritis that belongs to a new group of diseases now called seronegative spondyloarthropathies. These diseases are characterized by an extremely high association with the genetic marker, human leukocyte antigen B27. In its full course, the disease often is incomplete with its initial clinical manifestations and characterized by chronicity and progressive crippling recurrences.

Ultrasound in the study of soft tissue infections.

Musculoskeletal ultrasound is a safe and easily accessible imaging technique that allows the study of suspected infections of the musculoskeletal system. Although no finding is specific to these types of processes, certain synovial characteristics may suggest an infectious process rather than a non-infectious inflammatory condition. The role of ultrasound in this group of diseases is also to facilitate the extraction of synovial fluid for microbiological study. Ultrasound-guided synovial tissue biopsy is a minimally invasive, feasible, safe, and well-tolerated procedure that provides high-qual

Parvovirus infections in childhood.

Human parvovirus (HPV) infections have been associated with several clinical syndromes. The virus is the etiologic agent of erythema infectiosum (fifth disease) and the primary cause of aplastic crises in children with sickle cell disease and other hemolytic anemias. Some individuals with an acute symmetric polyarthritis were found to have a recent HPV infection. Although two stillborn infants with proved HPV infections have been identified, the exact relationship among HPV, intrauterine infection, and subsequent fetal damage requires further research.

Eye Disease in Patients with Rheumatic Diseases: A Retrospective Observational Cohort Study.

Data on eye diseases in rheumatic patients are limited. The aim of this study was to retrospectively assess the prevalence of ophthalmologic diseases in patients at a rheumatology outpatient clinic who also visited the ophthalmologic clinic. For this retrospective observational cohort study, a chart review was performed according to the STROBE guidelines. In this cohort, an ophthalmologic diagnosis was made in 26.9% of the 1529 rheumatic outpatients, whereas from a rheumatologic perspective, inflammatory non-infectious diagnoses dominated, at 71.7%. From an ophthalmologic perspective, diagnose

Infectious diseases affecting the musculoskeletal system in children and adolescents.

Many infectious organisms can lead to musculoskeletal symptoms in children. This article reviews recent studies on rheumatic fever, invasive group A streptococcal infections, septic infections of bones and soft tissues, reactive arthritides caused by viruses, fungi or bacteria, and Lyme disease. The search for etiologic organisms in chronic arthritis continues. Although Koch's postulates are not fulfilled in most cases of infectious arthropathies, progress has been made in diagnosing and treating these diseases.

Haemophilus parasuis (Glaesserella parasuis) as a Potential Driver of Molecular Mimicry and Inflammation in Rheumatoid Arthritis.

Background: Haemophilus parasuis (Hps; now Glaesserella parasuis) is an infectious agent that causes severe arthritis in swines and shares sequence similarity with residues 261-273 of collagen type 2 (Coll261-273), a possible autoantigen in rheumatoid arthritis (RA). Objectives/methods: We tested the presence of Hps sequencing 16S ribosomal RNA in crevicular fluid, synovial fluids, and tissues in patients with arthritis (RA and other peripheral arthritides) and in healthy controls. Moreover, we examined the cross-recognition of Hps by Coll261-273-specific T cells in HLA-DRB1*04pos RA patients,

Back pain following instillations of BCG for superficial bladder cancer is not a reactive complication: review of 30 Mycobacterium bovis BCG vertebral osteomyelitis cases.

Mycobacterium bovis Bacillus Calmette-Guérin (BCG) instillations are used in bladder cancer treatment. Adverse effects can occur. Osteoarticular complications are mainly reactive arthritis, but true infections have been described, such as vertebral osteomyelitis. We made a review of M. bovis BCG vertebral osteomyelitis after instillations for bladder cancer using PubMed search. We added three new French cases. Twenty-seven cases of BCG vertebral osteomyelitis had been reported on PubMed. Of the 30 cases, all were male, averaging 73.4 ± 8.7 years old. Median time between diagnosis and first and

Unexpected Hepatic Manifestations of Rheumatoid Arthritis.

Rheumatoid arthritis is an inflammatory condition typically affecting joint spaces. However, extra-articular manifestations are common including vascular and gastrointestinal tract involvement. This disease may also manifest as nodules, although these are typically found in the extremities and are very rarely seen in the liver. When a patient presents with liver nodules, an extensive work-up is typically completed to determine infectious, inflammatory, or malignant etiologies. We report hepatic findings of rheumatoid arthritis during an extensive inpatient investigation of liver lesions.

TNF-α antagonists beyond approved indications: stories of success and prospects for the future.

Tumour necrosis factor alpha (TNF-α) is a key molecule of the inflammatory response and data derived from studies in experimental animal models and humans suggest that TNF-α may be implicated in the pathogenesis of various autoimmune and non-infectious inflammatory conditions. Over the past decade pharmaceutical agents directed against TNF-α (infliximab, adalimumab and etanercept) have been widely and successfully employed for the management of rheumatoid arthritis (RA), ankylosing spondylitis (AS), psoriasis, psoriatic arthritis, juvenile idiopathic arthritis and inflammatory bowel disease, w

Post-infectious sequelae of travelers' diarrhea.

BACKGROUND: Travelers' diarrhea (TD) has generally been considered a self-limited disorder which resolves more quickly with expeditious and appropriate antibiotic therapy given bacteria are the most frequently identified cause. However, epidemiological, clinical, and basic science evidence identifying a number of chronic health conditions related to these infections has recently emerged which challenges this current paradigm. These include serious and potentially disabling enteric and extra-intestinal long-term complications. Among these are rheumatologic, neurologic, gastrointestinal, renal,

Orthopaedic infections.

Patients with orthopaedic infections may require admission to the intensive care unit (ICU). Necrotizing fasciitis and clostridial myonecrosis (gas gangrene) are serious soft tissue infections that may cause life-threatening complications. Patients suffering from infectious arthritis, osteomyelitis, or prosthetic joint infections may be seen in the ICU as a result of a previous injury, surgery, or delayed infectious processes. This article introduces the ICU nurse to the pathophysiology, clinical presentation, and management of a variety of orthopaedic infections.

[Cytology and histochemistry of synovial cells in various forms of arthritis].

Synovial effusions from various kinds of arthritis have been examined both morphologically and histochemically. These two methods give complementary evidence, and show (i) that there are two fundamentally different types of cell present in rheumatoid arthritis (ii) that the synovial fluid from septic arthritis can clearly be separated from that of all other forms of arthritis and (iii) that intracellular crystals can be demonstrated in gout. Laboratory methods also give information on the polysaccharide and fat content of synovial fluid, in particular on the lipid material in the cell free sup

Mycoplasma columbinum Isolated From a Racing Pigeon ( Columba livia ) With Arthritis.

A juvenile racing pigeon ( Columba livia ) was presented with drooping of the wing and inability to fly. On physical examination, the right shoulder joint was swollen. The pigeon was euthanatized and submitted for necropsy. An excessive amount of fibrin was present in the canalis triosseus with severe arthritis of the affected shoulder joint. A pure growth of Mycoplasma-like colonies was obtained on microbiological culture of the shoulder joint. A 16S ribosomal RNA gene-specific polymerase chain reaction assay was performed on the isolate and revealed 100% similarity with Mycoplasma columbinum

Tuberculous arthritis in a hemodialysis patient.

Tuberculosis (TB) is a relatively common infectious complication in the dialysis population. Most cases are extrapulmonary and pose a diagnostic dilemma for the clinician. Tuberculous arthritis is a rare form of extrapulmonary TB occurring in approximately 1% of cases in nonuremics. Only 1 case in a dialysis patient (not proven by positive synovial fluid culture) has been reported. We report here a culture-proven case of tuberculous arthritis in a hemodialysis patient which masqueraded as an acute septic arthritis. TB should be considered early as an etiologic agent along with synovial biopsy

Acute gouty arthritis as a manifestation of immune reconstitution inflammatory syndrome after initiation of antiretroviral therapy.

Immune reconstitution inflammatory syndrome (IRIS) in HIV-infected subjects initiating antiretroviral therapy most commonly involves new or worsening manifestations of previously subclinical or overt infectious diseases. Reports of non-infectious IRIS are much less common but represent important diagnostic and treatment challenges. We report on a 34-year-old HIV-infected male patient with no history of gout who developed acute gouty arthritis in a single joint one month after initiating highly active antiretroviral therapy.

Diagnostic potential of inflammatory markers in septic arthritis and periprosthetic joint infections: a clinical study with 719 patients.

BACKGROUND: The aim of this study was to investigate which markers in serum and in the synovial fluid have the highest diagnostic potential for predicting septic arthritis and periprosthetic joint infections (PJIs). The likelihood ratio assesses the diagnostic utility of a diagnostic test and the likelihood that a patient has a disease. METHODS: The levels of inflammatory markers in serum [white blood cells, C-reactive protein (CRPS)] and synovial fluid [synovial fluid white blood cell count (SFWBC), percentage of polymorphonuclear cells (%PMN), lactic acid, lactate dehydrogenase (LDH), gluco

Prevalence of antinuclear antibodies and rheumatoid factor titers in dogs with arthritis secondary to leishmaniosis (Leishmania infantum).

Dogs with infectious arthritis may occasionally exhibit positive serum antinuclear antibody (ANA) and rheumatoid factor (RF) titers; however, relevant data are sparse for arthritis secondary to canine leishmaniosis (CanL) caused by Leishmania infantum. We determined the prevalence of positive serum ANA and RF titers in dogs with arthritis secondary to CanL. Blood samples from adult, client-owned dogs with purulent arthritis secondary to CanL, without any comorbidities, were collected for diagnostic purposes. Serum ANA titers were measured by immunoperoxidase test and RF titers by the Rose-Waal

Polymicrobial polyarticular septic arthritis: a rare clinical entity.

Polymicrobial polyarticular septic arthritis is a rare clinical entity, with only a few cases having been reported to date. We report a case due to Streptococcus pyogenes and Staphylococcus aureus in an IVDU, complicated by fatal streptococcal toxic-shock syndrome, and review the current literature. We conclude that whenever polymicrobial polyarticular septic arthritis is diagnosed, a high index of suspicion should be maintained for the detection of locally destructive infectious processes as well as systemic complications, and that a high mortality rate should be expected.
