Positron emission tomography in the asphyxiated term newborn: parasagittal impairment of cerebral blood flow.

Hypoxic-ischemic encephalopathy secondary to perinatal asphyxia in the term newborn is the most common recognized cause of the subsequent motor deficits often grouped under the rubric "cerebral palsy." In order to provide insight into the basic nature and pathogenesis of the brain injury in such infants, we studied regional cerebral blood flow (CBF) by positron emission tomography (PET) in 17 asphyxiated term infants during the acute period of illness. A consistent and apparently unifying abnormality was observed, namely, a relative decrease in CBF to parasagittal regions, generally symmetrica

Purine metabolites and lactate as parameters of hypoxia in the newborn infant.

In order to evaluate possible biochemical parameters of tissue hypoxia, arterial samples from 29 sick neonates were taken at 6-h intervals for lactate and blood gas analyses, as well as assays of purine metabolites in plasma. The state of the infant at each sampling was arbitrarily classified as severe, moderate, or mild on the basis of PO2, pH, FiO2, and transcutaneous PO2. Of the total of 289 samples, 72 were in the mild, 178 in the moderate, and 39 in the severe category. Elevated values for hypoxanthine (greater than 36 mumol/L), xanthine (greater than 11 mumol/L), and lactate (greater tha

Symmetric bithalamic and striatal hemorrhage following perinatal hypoxia in a term infant.

A full-term infant who demonstrated a prolonged period of obtundation following asphyxia at birth was found on cranial computed tomography (CT) to have hemorrhage limited exclusively to symmetric bithalamic and striatal areas. This pattern of discrete, symmetric nuclear hemorrhage has not so far been reported as a complication of birth asphyxia. It differs from the germinal matrix hemorrhage on one hand in having a later time of onset (between the 4th and 10th day of life). It is also distinct from the more common supratentorial parenchymal hemorrhages in full-term infants owing to its topogra

[Care of severely asphyctic preterm and term infants].

We present a complex monitoring procedure for preterm and term infants with severe perinatal asphyxia and/or shock. It includes arterial and central venous catheters and occasionally a pulmonary artery catheter. The monitoring system provides rapid information on a number of parameters which can be used to improve the quality of ventilation and drug treatment. Therapy induced side effects are more rapidly detected and corrected. Handling of the baby is reduced and thereby the otherwise resulting increase in blood pressure and ensuing danger of cerebral hemorrhage.

Naloxone reverses neonatal depression caused by fetal asphyxia.

Pregnant near-term rabbits were given an intravenous dose of saline or the opiate antagonist naloxone and then asphyxiated. The fetuses were delivered by cesarean section and evaluated for respiration, color, muscle tone, response to stimulation, and general activity at 1, 3, 5, 10, 15, and 30 minutes of age. The naloxone-treated pups had significantly better scores during the first 15 minutes after birth than the saline-treated pups. Naloxone did not adversely affect the scores of nonasphyxiated pups. These data suggest that endogenous opiates worsen the neonatal depression caused by intraute

[Severe cardiac arrhythmia secondary to perinatal asphyxia].

A complex case of heart arrhythmia, probably, secondary to perinatal asphyxia is described. It was not possible to confirm a diagnosis from among the most common types of arrhythmias that develop during the neonatal period, such as congenital AV blockage, supraventricular paroxysmal tachycardia or ventricular paroxysmal tachycardia because of the changing pattern of the pacemaker and the rhythm.

Gastric aspirate analysis in the newborn.

The value of gastric aspirate analysis in screening for perinatal infection is disputed. We have studied the cellularity and bacterial content of 231 aspirates from 105 normal, infected and asphyxiated infants, with the following conclusions: (1) A positive culture has no pathological significance. (2). Aspirate cellularity does not correlate with bacterial culture results. (3) An excess of polymorphonuclear leucocytes (PMNL) in aspirates collected at birth is a pathological but non-specific stress response. (4) After birth there is a physiological rise and fall in the aspirate PMNL content pa

Pharmacologic neuroprotective agents for the treatment of perinatal asphyxia in low-income and lower-middle-income countries: A systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Perinatal asphyxia (PA) is a major contributor to neonatal mortality and long-term neurodevelopmental impairment, particularly in low- and middle-income countries (LMICs), where the effectiveness of therapeutic hypothermia remains limited. Pharmacologic neuroprotective agents have shown potential as alternative treatments, but their efficacy in low-income and lower-middle-income countries (LILMICs) is not well established. This systematic review aimed to assess the effectiveness of pharmacologic interventions in neonates with PA in LILMICs. METHODS: A systematic search of PubMed,

The advantages and limitations of animal models for understanding acute neonatal brain injury.

Neonatal hypoxic-ischemic encephalopathy (HIE) remains a major cause of death and disability around the world. Therapeutic hypothermia is now established to improve outcomes in term and near-term infants in high-income countries, but even in this setting, many infants still survive with disability. To further improve outcomes, experimental models are needed to test new interventions before clinical translation. In the present review, we discuss the strengths and weaknesses of a range of near-term and term models and their utility for understanding the pathophysiology of HIE and to test potenti

Motor, cognitive and behavioural outcomes after neonatal hypoxic-ischaemic encephalopathy.

INTRODUCTION: The current neurodevelopmental status of patients with neonatal hypoxic-ischaemic encephalopathy (HIE) in Spain is unknown. Recent European studies highlight a shift of severe pathology towards mild motor disorders and emotional problems. The aim of this study was to analyse neurodevelopmental outcomes in a cohort of neonates with HIE at age 3 years. PATIENTS AND METHOD: Multicentre observational study of neonates born at 35 or more weeks of gestation with moderate to severe HIE in 2011-2013 in 12 hospitals in a large Spanish region (91 217 m2), with the recruitment extended thr
