Trans-esophageal Echocardiographic Assessment of Left Atrial and Left Atrial Appendage Function in Atrial Fibrillation and Rheumatic Heart Disease.

INTRODUCTION: Rheumatic heart disease (RHD) is one of the most typical causes of atrial fibrillation in developing countries like India. The left atrial and left atrial appendage structure and function are deranged in atrial fibrillation and are a major source of thromboembolism. The goal of this study was to assess the left atrial and left atrial appendage function by transesophageal echocardiography in patients with atrial fibrillation and their comparison in patients with or without RHD. METHODS: A total of 172 consecutive patients with atrial fibrillation with or without RHD were subjecte…

Left Atrial Remodeling Assessed by Transthoracic Echocardiography Predicts Left Atrial Appendage Flow Velocity in Patients With Paroxysmal Atrial Fibrillation.

Atrial fibrillation (AF) is associated with an increased risk of stroke and other thromboembolic events. Left atrial (LA) thrombus formation is closely related to LA dysfunction, particularly to decreased LA appendage flow velocity (LAA-FV) in patients with AF. We estimated LAA-FV using parameters noninvasively obtained by transthoracic echocardiography (TTE) in patients with paroxysmal AF.Echocardiographic and clinical parameters were assessed in 190 patients with nonvalvular paroxysmal AF showing sinus heart rhythm during transesophageal echocardiography (TEE) and TTE.LAA-FV (60 ± 22 cm/s) s

Optimal Anticoagulation for Massive Left Atrial Appendage Thrombus in Atrial Fibrillation With a History of Gastrointestinal Bleeding.

Systemic embolism is a serious complication of atrial fibrillation (AF), most commonly originating from thrombi in the left atrial appendage (LAA). Thrombi that are large (≥15 mm) or exhibit mobility carry particularly high embolic risk, yet management can be challenging in patients with elevated bleeding risk. Advanced echocardiographic techniques now allow quantification of thrombus mobility, and dense spontaneous echo contrast (SEC) on transesophageal echocardiography has been identified as an independent predictor of thromboembolic events. We report the case of an 80-year-old woman with pa…

Arrhythmias of the Left Atrial Appendage: Approaches to the Definitive Management of Atrial Tachycardia from the LAA Stump.

The left atrial appendage (LAA) is now recognized as a significant contributor to arrhythmia and thromboembolism in patients with a history of atrial fibrillation. Thoracoscopic exclusion of the LAA is made possible with the AtriClip device. In this report, we describe the case of a 65-year-old man with history of multiple left atrial ablation procedures and LAA clipping. He developed a microreentrant atrial tachycardia originating from the anterior base of the LAA stump, underwent complete isolation of the LAA, and had subsequent resolution of arrhythmogenic activity from the residual LAA stu…

Case report: Epicardial ligation of the left atrial appendage in a patient with an inaccessible left atrial cavity.

It has been shown that endocardial occlusion of the left atrial appendage (LAA) is equally effective in preventing embolic events compared to oral anticoagulation in patients with nonvalvular atrial fibrillation. An 82-year-old female patient was admitted for LAA occlusion for repetitive GI bleeding. She had high CHADSVASC Score and HASBLED with long persistent atrial fibrillation with many comorbities. The preprocedural transesophageal echo revealed a great mass in the left atrium (Picture 1). Cardiac surgery was denied, we performed an epicardial only ligation for closing the LAA. For this a…

Percutaneous left atrial appendage occlusion for stroke prevention in patients with atrial fibrillation and contraindication for anticoagulation.

BACKGROUND: Stroke, as the third cause of death in developed countries, is a public health issue. Atrial fibrillation is an important cause of ischemic stroke and its prevention is efficient with oral anticoagulation. However, oral anticoagulation can be contraindicated because of hemorrhagic risk related to these treatments. Percutaneous left atrial appendage occlusion is a new alternative of oral anticoagulation for patients with atrial fibrillation and high risk of cardio-embolic stroke but contraindicated for oral anticoagulation. METHODS: We describe in this paper the procedure of left a…

Catheter closure of an atrial septal defect in anatomically corrected malposition with left juxtaposition of atrial appendages.

Anatomically corrected malposition is characterized by concordant atrioventricular and ventriculoarterial connection and parallel arrangement of arterial trunks. Left juxtaposition of the atrial appendages, a frequent association of this anomaly, alters the atrial septal plane. In this report, I describe the insertion of a device to close an atrial septal defect, as guided by transoesophageal echocardiography.

Predicting reduced left atrial appendage velocity from echocardiographic left atrial function parameters in patients with atrial fibrillation undergoing catheter ablation.

Decreased left atrial appendage velocity (LAAV) is considered a significant risk factor thrombus formation in the left atrial appendage (LAA). The aim of this study was to assess the role of echocardiographic left atrial (LA) function parameters in predicting LAAV in patients with persistent atrial fibrillation (AF) undergoing catheter ablation. We prospectively enrolled consecutive patients with persistent AF undergoing transesophageal echocardiography (TEE) directly before the first AF ablation in 2019-2022. Of the 150 patients enrolled in the study, 29.3% (n = 44) had reduced LAAV values de…

Outcomes of Left Atrial Appendage Occlusion in Patients With Cardiac Amyloidosis.

•Patients with cardiac amyloidosis (CA) and atrial fibrillation are at high risk for thromboembolic as well as bleeding complications. However, there is a paucity of data for left atrial appendage occlusion in these patients.•We found that while there was no significant difference in overall composite primary outcome (death, stroke, transient ischemic attack, and thromboembolic events) between patients with and without CA, those with CA had outcomes similar to patients without CA with intermediate to high CHA2DS2-VASc scores.•A propensity matched analysis similarly revealed significantly worse…

Left atrial appendage tear due to blunt trauma to the chest.

This case report presents the successful surgical management of a 34-year-old male who had blunt chest trauma resulting in a left atrial appendage tear after a traffic accident. He arrived at the emergency department 30 minutes post-injury in a gasping state, showing signs of hypovolemic shock, tachycardia (132 bpm), hypotension (blood pressure 88/48 mm Hg) and decreased breath sounds on the left side. The initial management included fluid resuscitation and the insertion of an intercostal drain, which drained 1.2 litres of blood. After the initial fluid resuscitation, haemodynamic stability wa…

The Case for Intracardiac Echo to Guide Left Atrial Appendage Closure.

Left atrial appendage closure (LAAC) has become a commonly used alternative to anticoagulation for stroke prevention in patients with atrial fibrillation. There is a growing interest in adopting a minimally invasive procedural approach using intracardiac echocardiography (ICE) and moderate sedation. In this article, we review the rational for and the data supporting ICE-guided LAAC and discuss the pros and cons of this approach.

Efficacy and Safety of a Dedicated Left Atrial Appendage Occlusion Protocol.

OBJECTIVES: The aim of this study is to present a dedicated left atrial appendage closure protocol, which could be of great interest in the approach of frail patients. BACKGROUND: Left atrial appendage (LAA) occlusion emerged as a promising therapeutic tool for stroke prevention, as most of the atrial fibrillation (AF) related strokes result from LAA thromboembolism, with an important residual risk even for anticoagulated patients. As an internationally recognized and scientifically-based protocol for the patients with higher comorbidities has not yet been defined for this procedure in terms …

Left atrial appendage occlusion with the Amulet device: to tug or not to tug?

PURPOSE: Left atrial appendage occlusion (LAAO) involves a "tug test," in which implanters pull on the device delivery cable to ensure stable occluder placement. The aim of this study was to evaluate the recommendation to perform the tug test, by comparing forces exerted on the device during deployment and subsequent tug test. A secondary objective was to simulate forces experienced on left atrial appendage tissue by placement of a 20-mm device. METHODS: The AMPLATZER™ Amulet™ device was used for occlusion. A force transducer recorded forces in the delivery cable during deployment and tug tes…

Early Dislodgment and Migration of a Left Atrial Appendage Closure Device.

A 68-year-old man underwent pulmonary vein isolation with cryoballoon combined with left atrial appendage closure using a LAmbre device. The device was dislodged and embolized early after implantation with no symptoms, and it was retrieved percutaneously. An early in-hospital check of the device position after implantation is important for early recognition of any possible device-related complication.
