Circadian variation of paroxysmal atrial fibrillation.

BACKGROUND: Circadian variation in the incidence of acute cardiovascular events is well known but has not been extensively investigated in paroxysmal atrial fibrillation, although the significance of this arrhythmia is growing in our society with the increasing number of aged people. METHODS AND RESULTS: We detected 150 patients with paroxysmal atrial fibrillation in a drug-free state from among 25,500 consecutive Holter recordings. To determine whether the onset, maintenance, and termination of paroxysmal atrial fibrillation were random events, we analyzed the total recorded duration of arrh…

Radiofrequency catheter ablation for atrial fibrillation.

Until recently, catheter-based radiofrequency ablation for atrial fibrillation was limited to palliative approaches of either atrioventricular node ablation or modification. It is now recognized that at least a proportion of patients with paroxysmal atrial fibrillation may be suitable for curative ablation of an underlying single arrhythmogenic focus. With the intense interest in this area, a catheter-based cure involving endocardial linear lesion creation for patients with chronic or paroxysmal atrial fibrillation may not be far in the future.

Nonpharmacologic strategies for treating atrial fibrillation.

Nonpharmacologic tools to treat atrial fibrillation (AF) are direct current cardioversion, radiofrequency (RF) current catheter ablation, antiarrhythmic surgery, pacing, and atrial defibrillation. In patients with sustained AF, when no cause can be found for AF or when the associated disease is mild, an attempt should be made to restore sinus rhythm. Electrical cardioversion by synchronized direct current shock can be attempted when drugs have failed and is the first choice in acutely ill patients. Virtually all patients should be anticoagulated. Temporary pacing should be available in patient…

Atrial fibrillation, anticoagulation, and electroconvulsive therapy.

We describe our experience with six patients with atrial fibrillation (AF) during electroconvulsive therapy (ECT). In four, we observed episodic or persistent conversion of AF to normal sinus rhythm (NSR). Four patients, three with cardioversion and one without, received anticoagulation. In published case reports, ECT was successfully performed in three patients with AF without anticoagulation and in three patients with AF who received anticoagulation. In addition, 18 patients received ECT while taking concomitant anticoagulation therapy for reasons other than AF. Despite the potential risk of…

Persistent atrial fibrillation following electrical injury.

Supraventricular arrhythmias are seen following non-fatal electrical injuries. They are believed to be transient, but this has never been confirmed. We report a case of fixed atrial fibrillation that has persisted for 20 years after an electrical injury at work.

Hemodynamics of idiopathic paroxysmal atrial fibrillation.

The hemodynamics of induced atrial fibrillation (AF) was investigated in 15 patients (ages 58 +/- 11 years) with paroxysmal AF presenting without organic heart disease or hypertension. A hemodynamic study was performed both during sinus rhythm and after the induction of AF. The mean heart rate increased from 73 +/- 11 to 128 +/- 18 beats/min (P < 0.001) after AF. Systolic and mean aortic pressures did not significantly change, and diastolic aortic pressure increased (78 +/- 11 vs 89 +/- 12 mmHg, P < 0.01). Left ventricular end-diastolic pressure decreased during AF (9 +/- 3 vs 6 +/- 2.6 mmHg, …

[Antiarrhythmic treatments of permanent atrial fibrillation].

The treatment of chronic atrial fibrillation has 3 different objectives: the control of the ventricular rate, the attempts to restore sinus rhythm, and the prevention of arrhythmia recurrences. Digitalis compounds, beta-adrenergic antagonists and calcium-channel blocking agents can be used to achieve reduction of the ventricular response, in recent-onset arrhythmia with rapid heart rate, and for long-term rate control in patients who cannot be converted to sinus rhythm. In some of them, a combination of 2 of these drugs is needed to slow the ventricular rate, at rest and on exertion. Sinus rhy…

[Antithrombotic treatment of permanent atrial fibrillation].

Whether idiopathic or associated with an underlying heart disease, complete arrhythmia induced by atrial fibrillation ranks first among the causes of arterial embolism. It has been recognized that 30 to 35% of subjects with arrhythmia develop a transient or permanent ischaemic cerebral accident, and it is therefore rational to assess the value of systematic antithrombotic treatment in the prevention of such accidents. It is agreed by all that anticoagulants are necessary when atrial fibrillation occurs in the course of cardiac valve diseases, notably those of rheumatismal origin. Conversely, t…

Flecainide in quinidine-resistant atrial fibrillation.

Flecainide (100 mg twice daily) was used for prevention of paroxysmal atrial fibrillation (PAF) in 52 patients with frequent symptomatic attacks that were resistant or intolerant to quinidine (600-900 mg/day). Underlying heart disease was present in only 8 cases and left ventricular ejection fraction was always greater than 30%. No patient had had a myocardial infarction. Vagally induced PAF was clinically documented in 35 patients. Amiodarone, previously used and ineffective, was combined with flecainide in 33 patients. After 1-5.8 years of follow-up, complete disappearance of PAF was observe…

[Embolic complications of chronic atrial fibrillation].

Systemic embolism secondary to chronic atrial fibrillation usually affect the cerebral circulation. The risk of a cerebrovascular accident in patients with chronic atrial fibrillation, irrespective of the aetiology, is 1.8 to 7.5 times that of the general population. The embolic risk is 18 times greater in patients with atrial fibrillation related to the rheumatic heart disease. The risk of patients under 60 years of age with idiopathic atrial fibrillation does not seem to be different to that of the general population. The risk of early recurrence of embolism in the first 30 days ranges from …

[Isolated so-called "alone" atrial fibrillation].

A 49-year-old man with paroxysmal atrial fibrillation was studied in hospital for detection of the cause of arrhythmia and was qualified into the group of the so called alone atrial fibrillation, with healthy cardiovascular system. He was treated successfully with amiodarone++. This type of atrial fibrillation occurs in only 6% of cases of atrial fibrillation, but is possible occurrence should be taken into consideration in differential diagnosis.

Atrial fibrillation following methylprednisolone pulse therapy.

Two children, one with renal manifestations of systemic lupus erythematosus and the other with idiopathic nephrotic syndrome, were treated with methylprednisolone pulses. Neither had previous evidence of underlying cardiac disease. Within 24 h of pulse therapy, they complained of palpitations and developed atrial fibrillation which reversed spontaneously or after anti-arrhythmic therapy. Subsequent serial electro- and echocardiograms were normal. We propose that the arrhythmias were a complication of steroid pulse therapy.

[Atrial fibrillation in apparently healthy persons].

The author reports about three young patients with chronic atrial fibrillation. No data about organic disease of the cardiovascular system have been established by the detailed clinical and paraclinical study. All three patients were influenced by the treatment with cardiac glucosides, panangin and quinidine-they recovered the sinus rhythm. After recovering the sinus rhythm and followed up for one year, no data about organic disease of the cardiovascular system had been established.

Embolic complications in paroxysmal atrial fibrillation.

The incidence of embolic complications among 426 patients with initial paroxysmal atrial fibrillation (PAF) was analysed. A distinct clustering of emboli was seen at the time of onset of PAF. After transition to chronic atrial fibrillation (CAF), which developed in 141 patients (33.1%), the incidence of emboli was seen to rise to a new level several times higher than the incidence level for patients with PAF. Also in this group a distinct clustering of emboli was seen during the first year after transition to CAF. On this background it is suggested that patients with PAF may benefit from treat…
