Eight patients with severe tetanus experienced episodes of marked cardiovascular instability. During these unstable periods, cardiac output and vascular resistance varied independently. Beta blocking and low doses of ganglionic blocking agents failed to control cardiovascular instability even when combined. High doses of ganglionic blockers stabilized hemodynamics for a short period, but caused the elevation of transaminases. For replantation surgery, continuous brachial plexus block was employed using supraclavicular or axillary approach. We present a patient who developed a high central neural block after stellate-ganglion-blockade. The underlying pathophysiology was assessed via sonographic imaging. The term "atypical facial pain" refers to a pain disease and should not be used for pain of peripheral origin. As a rule atypical facial pain is induced by a peripheral tissue trauma not necessarily associated with nerve lesions. The intensity of pain does not correlate with the measurable magnitude of the trauma. If treatment is possible, it will be the more successful the earlier it is started. Therapy comprises transcutaneous electric nerve stimulation, sympathetic nerve block, drug therapy and psychotherapy.
