Predictive factors of biliary complications after bariatric surgery.

BACKGROUND: Obesity and rapid weight loss are risk factors for gallstone development. Bariatric surgery and significant postoperative weight loss are associated with postoperative biliary complications. OBJECTIVE: We aim to identify predictive factors of biliary complications after bariatric surgery. SETTING: University hospital. METHODS: After Institutional Review Board approval, charts at a single institution were reviewed to identify patients with biliary complications after bariatric surgery from 2005 to 2012. Data collected included baseline patients demographic characteristics, periop

Post-operative psychosocial predictors of outcome in bariatric surgery.

Although there are several recent reviews of the pre-operative factors that influence treatment outcome for bariatric surgery, commensurate efforts to identify and review the predictive validity of post-operative variables are lacking. This review describes the post-operative psychosocial predictors of weight loss in bariatric surgery. Results suggest empirical support for post-operative binge eating, uncontrolled eating/grazing, and presence of a depressive disorder as negative predictors of weight loss outcomes; whereas, adherence to dietary and physical activity guidelines emerged as positi

The Night Eating Syndrome (NES) in Bariatric Surgery Patients.

The night eating syndrome (NES) has been included into the Diagnostic and Statistical Manual of Mental Disorders 5 as an example of an 'other-specified feeding or eating disorder'. The prevalence of NES has found to be higher in obese populations than in the general population and seems to rise with increasing body mass index. Recent studies suggest a prevalence of 2%-20% in bariatric surgery samples. Given that the core feature of this eating disorder may involve a shift in the circadian pattern of eating that disrupts sleep, and not the ingestion of objectively large amounts of food, it is a

Biochemical consequences of bariatric surgery for extreme clinical obesity.

Obesity, defined as a body mass index over 30 kg/m(2) for adults, poses a major healthcare challenge with important economic, personal and social consequences. Although public health measures, lifestyle change and pharmacological therapies have an important role in the management of obesity, patients with established morbid obesity (body mass index over 40 kg/m(2)) may also require bariatric surgery. Bariatric or metabolic surgery is associated with effective and enduring weight loss but is also known to improve glucose homeostasis, blood pressure and dyslipidaemia. Patients who have bariatric

Pregnancy as a risk factor for undertreatment after bariatric surgery.

A pregnant woman presented at the emergency department with severe nausea and vomiting at 20 weeks of gestational age; she was known with gastric banding. Advanced imaging studies were avoided of fear to harm the fetus. The patient continued to vomit and at 23 weeks intrauterine fetal death was noted. The symptoms did not resolve after delivery and CT scan demonstrated slippage of the gastric band over the pylorus resulting in a high digestive obstruction as the cause of hyperemesis and finally resulting necrosis of the vasa brevia. The gastric band was laparoscopically removed along with the

Obesity: long-term management after bariatric surgery.

For many patients, bariatric surgery results in enduring weight loss and comorbidity resolution, but it is not without risk of complications. Complications necessitating possible surgical intervention include small bowel obstruction, internal hernia, intussusception, anastomotic ulcer, and anastomotic leak. Medical complications include dumping syndrome; osteoporosis; substance abuse; nephrolithiasis; high suicide rates; and nutritional deficiencies, some of which can result in neurologic disorders. Patients need to take daily multivitamins with minerals (containing iron, copper thiamine, and

Imaging of bariatric surgery: normal anatomy and postoperative complications.

Obesity is a disease that has reached epidemic proportions in the United States and around the world. During the past 2 decades, bariatric surgery has become an increasingly popular form of treatment for morbid obesity. The most common bariatric procedures performed include laparoscopic Roux-en-Y gastric bypass, laparoscopic adjustable gastric banding, and laparoscopic sleeve gastrectomy. Fluoroscopic upper gastrointestinal examinations and abdominal computed tomography (CT) are the major imaging tests used to evaluate patients after these various forms of bariatric surgery. The purpose of thi

[Long-term implication of bariatric surgery: beyond deficiencies].

Bariatric surgery frequently leads to several nutritional deficiencies that are well reported in the literature. However, numerous other aspects of the patients' life are concerned by bariatric interventions, so that the follow-up of these patients after bariatric surgery does not only encompass a mere correction of the well-known nutritional deficiencies. Only a thorough knowledge of all possible consequences of bariatric interventions permits an adequate follow-up of these patients. Overtly addressing all these issues with the patients before performing a bariatric intervention is the best w

Patients at risk for trace element deficiencies: bariatric surgery.

Obesity is a worldwide epidemic associated with diseases such as diabetes mellitus and cardiovascular disease. Current methods for weight loss are not very effective, particularly for those with morbid obesity. Surgical therapy may be recommended for those with a BMI≥40kg/m(2), or BMI≥35kg/m(2) with co-morbidities. This therapy can produce significant weight loss and improve/resolve co-morbidities including hypertension and hyperlipidemia. Yet successes may be tempered by adverse effects on trace element absorption and status. A PubMed literature search identified studies from January 1980 to

Complications from micronutrient deficiency following bariatric surgery.

We report a case of clinically significant micronutrient deficiencies following biliary pancreatic diversion (BPD) surgery. Our patient was admitted to hospital six years after BPD surgery following a low impact humeral fracture complicated by postoperative wound infection. On presentation she complained of a widespread rash and loss of night vision. Laboratory testing confirmed hypoalbuminaemia, deficiencies of vitamins A, E and D and of the trace elements copper, zinc and selenium. Bone densitometry confirmed osteoporosis. The skin rash was thought to be due to zinc deficiency and improved w

Bipolar disorder symptoms in patients seeking bariatric surgery.

BACKGROUND: Mood disorders are common among patients seeking bariatric surgery although little data exist regarding the prevalence of bipolar symptoms in this population and how they influence surgical outcomes. Our aim was to describe baseline rates of bipolar symptoms and their relationship to psychological factors and completing surgery in a sample of adults seeking bariatric surgery at an academic medical center. METHODS: We retrospectively reviewed the relationship of bipolar symptoms to demographic characteristics, baseline weight, psychological factors, and bariatric surgery completion

Safety of Veress needle insertion in laparoscopic bariatric surgery.

BACKGROUND: Creating the pneumoperitoneum is the first surgical procedure in laparoscopic abdominal surgery. Morbid obesity is a risk factor for iatrogenic injuries because of the considerable thickness of the abdominal wall. The aim of this study was to assess the feasibility and the incidence of complications when using Veress needles (VN) in obese patients undergoing bariatric surgery. METHODS: Between March 2004 and December 2010, a retrospective analysis was performed on 139 obese patients (mean body mass index=45.94 kg/m). Blind VN insertion followed by optical trocar insertion was the

Lifestyle management for enhancing outcomes after bariatric surgery.

Bariatric surgery has been safe and effective for treatment of severe obesity and comorbidities like type 2 diabetes mellitus (T2D). Nonetheless, weight loss and health outcomes vary considerably across individuals. Although the factors associated with outcomes are not fully understood, postoperative weight loss following any type of bariatric surgery is largely dependent on the extent to which patients can make and sustain changes in eating and activity. Therefore, lifestyle management including diet, exercise, and behavior modification is critical to helping patients achieve long-term weight

Pregnancy after bariatric surgery--a review of benefits and risks.

BACKGROUND: When other weight loss attempts have failed, bariatric surgery offers a successful alternative for obesity. Since operations are performed during women's reproductive years, the number of pregnant women with prior bariatric surgery is increasing. Bariatric surgery results in restriction of food intake and/or malabsorption leading to weight loss, but may induce a risk for malnutrition and pregnancy complications. METHOD: Systematically conducted review addressing pregnancy after bariatric surgery using the PubMed and Cochrane databases. MAIN OUTCOME MEASURES: Birthweight, gestatio

[How to identify the risk for osteoporosis after bariatric surgery].

Gastric bypass often reduces the protein and calcium consumption and causes deficiencies in vitamin D with secondary hyperparathyroidism. As a consequence, it is a risk for osteoporosis. It is therefore necessary to regularly evaluate protein consumption and the levels of vitamin D and PTH. In case of deficiency, supplements must be given in order to obtain PTH level < 50 ng/l. The interpretation of the results of the bone densitometry is difficult during rapid weight loss due to the presence of technical artifacts related to the reduction of the subcutaneous fat thickness, while from the seco

Early complications in bariatric surgery: incidence, diagnosis and treatment.

CONTEXT: Bariatric surgery has proven to be the most effective method of treating severe obesity. Nevertheless, the acceptance of bariatric surgery is still questioned. The surgical complications observed in the early postoperative period following surgeries performed to treat severe obesity are similar to those associated with other major surgeries of the gastrointestinal tract. However, given the more frequent occurrence of medical comorbidities, these patients require special attention in the early postoperative follow-up. Early diagnosis and appropriate treatment of these complications are

Bariatric surgery and its effects on the skin and skin diseases.

In light of the increasing prevalence of obesity worldwide, the popularity of bariatric surgery is on the rise. As with any other invasive procedure, these surgeries, especially with the obesity risk factor, carry the risk of direct cutaneous complications following the penetration and manipulation of tissues. In addition, bariatric surgery has an effect on skin structure and function. It also appears to be affiliated with several dermatoses. Some of these represent preexisting diseases the course of which is altered by the procedure, such as psoriasis. On the other hand, other skin disorders
