Background/Objectives: Benign paroxysmal positional vertigo (BPPV) is the most prevalent vestibular disorder encountered in clinical settings and is highly responsive to evidence-based diagnostic and therapeutic interventions. However, variations in practice patterns among physician specialties can compromise timely diagnosis and effective treatment. Understanding these variations is essential for improving clinical outcomes and standardizing care. This study aimed to assess the diagnostic and treatment practices for BPPV among Ear, Nose, and Throat (ENT) specialists, neurologists, general pra…

We evaluated the short-term efficacy of Epley, Semont, and sham maneuvers for resolving posterior canal benign paroxysmal positional vertigo (BPPV) in a prospective multicenter randomized double-blind controlled study. Subjects were randomly divided into three groups: Epley (36 patients), Semont (32 patients), and sham (Epley maneuver for the unaffected side, 31 patients). Out of 14 institutes which participated in this study, 5 institutes had previous experience of the Epley but not the Semont maneuver and the other 9 had previous experience of both maneuvers. Each maneuver was repeated twice…

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is a common vestibular disorder characterized by brief episodes of vertigo triggered by changes in head position and is associated with displaced otoconia in the inner ear. Emerging evidence suggests that metabolic conditions, such as vitamin D deficiency, osteoporosis, and obesity, increase the risk of BPPV recurrence. This study investigated the associations between BPPV recurrence and these metabolic conditions using a large population-based dataset.
METHODS: A nested case-control study was conducted using data from the Korean Nationa…

Background/Objectives: Vitamin D deficiency has been associated with the occurrence and recurrence of benign paroxysmal positional vertigo (BPPV), but its influence on the immediate success of canalith repositioning maneuvers remains uncertain. This study investigated the relationship between serum vitamin D levels and the first-attempt success of the Epley maneuver in patients with posterior canal BPPV (PC-BPPV). Methods: This prospective observational study included 140 consecutive adults with objectively confirmed unilateral PC-BPPV. Serum 25-hydroxyvitamin D levels were measured at present…

Benign paroxysmal positional vertigo (BPPV) is amongst the commonest causes of dizziness and falls in older adults. Diagnosing and treating BPPV can reduce falls, and thereby reduce fall-related morbidity and mortality. Recent World Falls Guidelines recommend formal assessment for BPPV in older adults at risk of falling, but only if they report vertigo. However, this recommendation ignores the data that (i) many older adults with BPPV experience dizziness as vague unsteadiness (rather than vertigo), and (ii) others may experience no symptoms of dizziness at all. BPPV without vertigo is due to …

INTRODUCTION: Benign paroxysmal positional vertigo (BPPV) is a common diagnosis for dizziness patients admitting to emergency department and for initial diagnosis, cardiac causes of dizziness should be excluded at admittance. Electrocardiography (ECG) is a simple method to detect cardiac arrhythmias for these patients. Tp-e interval and Tp-e/QTc ratio are transmural repolarization parameters and shown to be strongly related to ventricular arrhythmias. With this study, we aim to investigate ventricular repolarization parameters like Tp-e interval and Tp-e/QTc ratio which can be easily evaluated…

Benign paroxysmal positional vertigo (BPPV) occurs in 14.5% of patients with spinal cord injury (SCI) and may require intervention on intensive care unit (ICU). A 61-year-old man was admitted to a spinal injury ICU with a traumatic C3 complete SCI following a mountain bike accident. Ventilated but stable he complained of severe dizziness on rolling, during personal cares, which lasted for 40 s. Clinical examination was limited due to the injury and ventilation. Subjective questioning, visio-ocular control and a modified Dix-Hallpike and roll tests confirmed a right posterior canalithiasis BPPV…

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment.
CASE PRESENTATION: We describe the case of a 52-year-old French woman p…

BACKGROUND: As the prevalent cause of dizziness, benign paroxysmal positional vertigo (BPPV) is increasingly considered a major public health concern due to its high recurrence rate and persistent symptoms. Growing evidence suggests a biologically plausible link between dietary factors and BPPV progression. However, current research on the role of diet in BPPV has predominantly focused on individual nutrients and disease onset, with limited evidence regarding the impact of overall dietary patterns on post-treatment clinical outcomes, especially in Asian populations. Furthermore, the potential …

BACKGROUND: Residual dizziness (RD) is a common and debilitating symptom following successful canalith repositioning procedure (CRP) for benign paroxysmal positional vertigo (BPPV). Currently, no effective pharmacological treatment exists for this condition. Our previous work identified low serum 25-hydroxyvitamin D [25(OH)D] as an independent risk factor for RD. This study therefore evaluated whether 8-week supplementation with Caltrate D was associated with improvement in RD.
METHODS: Female patients with primary BPPV who achieved successful CRP but still complained of RD at day 7 post-CRP …
