Self-injection of barbiturates, benzodiazepines and other sedative-anxiolytics in baboons.

Self-injection of 12 sedative-anxiolytics was examined in baboons. Intravenous injections and initiation of a 3-h time-out were dependent upon completion of a fixed-ratio schedule requirement, permitting eight injections per day. Before testing each dose of drug, self-injection performance was established with cocaine. Subsequently, a test dose was substituted for cocaine. At some doses, all five of the benzodiazepines examined (alprazolam, bromazepam, chlordiazepoxide, lorazepam, triazolam) maintained rates (number of injections per day) of drug self-injection above vehicle control in each of…

Effects of barbiturates on GABA system: comparison to alcohol and benzodiazepines.

Central nervous system depressants, e.g. barbiturates, alcohol and benzodiazepines, have a wide spectrum of activity in humans and animals. Evidence accumulated suggests that some of the pharmacological actions exerted by these agents may be mediated through GABA system by mimicking GABAergic transmission. This proceeding briefly summarizes the evidence presented in our previous review (Yu S and Ho Ik: Alcohol 7: 261, 1990) as to how the GABA system plays a part in the barbiturate actions and the development of tolerance to and physical dependence on barbiturates. The comparisons of the effect…

Long-term therapeutic use of benzodiazepines. I. Effects of abrupt discontinuation.

We compared the effect of abrupt discontinuation of therapeutic doses of short half-life and long half-life benzodiazepines in 57 benzodiazepine-dependent patients (daily use, greater than 1 year). Despite the use of a mean daily dose of 14.1 mg of diazepam equivalents, there were notable residual symptoms of anxiety and depression present at intake (Hamilton Rating Scale for Anxiety score, 17.0; Hamilton Rating Scale for Depression score, 14.0). Benzodiazepine intake was stabilized for 3 weeks before double-blind assignment to placebo (n = 47), or continued benzodiazepine use (n = 10). Clinic…

Congenital malformations and maternal consumption of benzodiazepines: a case-control study.

This study assessed potential teratogenic properties of benzodiazepine (BZD) intake during early pregnancy. Four neonatal diagnoses of congenital malformations (embryopathy and fetopathy, unspecified; unspecified congenital malformations of the nervous system; cleft palate and cleft lip; congenital malformations of the urinary tract) were selected. The authors' previous clinical experience had shown these diagnoses to be characteristic of infants born to mothers with excessive intake of BZD in early pregnancy. The selected diagnoses were present in 25 of 10,646 liveborn infants (2.3 per 1000)

Benzodiazepines and the mammalian retina. II. Actions on retinal ganglion cells.

The effects of intravenously and iontophoretically applied benzodiazepines, midazolam and flurazepam and their receptor antagonist, flumazepil (RO 15 1788), on visually evoked and spontaneous activities of the mammalian retinal ganglion cells have been studied. Intravenously applied midazolam and flurazepam suppressed both light-evoked and spontaneous firing of rat optic tract fibres. They reduced both the sensitivity to light and the temporal resolution of the fibres. Flumazepil (RO-15-1788), on the other hand, enhanced both light-evoked and spontaneous firing of the optic tract fibres. It in…

However, there were no differences between placebo and the benzodiazepines as far as side effects or the quality of dreams and of morning awakening were concerned. The two drugs had an identical effect on sleep latency, but under midazolam the patients woke more frequently during the night and slightly earlier in the morni …

Rebound anxiety in panic disorder patients treated with shorter-acting benzodiazepines.

Rebound--the relative worsening of symptoms on discontinuation of treatment as compared to baseline symptoms--is distinguished from withdrawal. Case reports and a clinical study are presented to illustrate the management of patients with panic disorder who are taking short- and intermediate-acting benzodiazepines and are experiencing rebound anxiety. The authors present the results of switching over to clonazepam 48 patients with panic disorder who were experiencing rebound effects with alprazolam. Eighty-two percent (39) of the patients rated clonazepam as being "better" than alprazolam due t…

Brain mechanisms associated with therapeutic actions of benzodiazepines: focus on neurotransmitters.

The sedative, muscle relaxant, antianxiety, and anticonvulsant effects of benzodiazepines may involve several distinct mechanisms because dissociation among these actions can be demonstrated with various drugs. Neurotransmitters displaying prominent interactions with benzodiazepines include gamma-aminobutyric acid (GABA), glycine, norepinephrine, and serotonin.

Further evidence for GABA-ergic mechanisms in the action of benzodiazepines.

Drugs of the benzodiazepine series (clonazepam, lorazepam, diazepam and medazepam) increase the inhibitory processes in the cerebral cortex … ne and the decrease of the GABA level by thiosemicarbazide. It is inferred that the main effects of benzodiazepines are mediated by GABA and suggested that these agents increase the sensitivity of postsynaptic GABA-ergic receptors.

Recurrent Takotsubo Cardiomyopathy Due to Benzodiazepine Withdrawal.

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a rare cause of acutely decompensated heart failure among patients without pre-existing chronic heart failure. We present a case of recurrent TCM attributed to benzodiazepine withdrawal. CASE SUMMARY: A 40-year-old woman with prior TCM and reverse TCM was admitted for cardiogenic shock after having recently changed her benzodiazepine regimen. She was emergently cannulated for venoarterial extracorporeal membrane oxygenation; transthoracic echocardiography showed an ejection fraction of 10% to 15% with apical hypokinesis. Intra-aortic balloon pump

Identifying Benzodiazepine Abuse: A Report of 3 Cases.

Benzodiazepines are a class of tranquilizers that, like prescription opiates, are commonly subject to abuse. Abusers of benzodiazepines commonly malinger to sustain access to these prescriptions from their health care providers by creating and enhancing medically necessary conditions requiring treatment with benzodiazepines. Such behavior may involve displays of unrelenting extreme anxiety that are resolved only by the administration of benzodiazepines. Typical substance abuse behaviors also include noncompliance with treatment, lack of self-care, and lack/failure of counseling. Three cases un…

Flumazenil Use in Benzodiazepine-Induced Delirium Reversal.

Delirium is a frequent and serious complication in critically ill patients, associated with increased mortality, prolonged hospitalization, and long-term cognitive impairment. The hypoactive subtype is particularly underrecognized, missed in up to 75% of cases, and carries worse outcomes. Benzodiazepines (BZDs), though often used for sedation in the intensive care unit (ICU), can contribute to or exacerbate delirium due to their pharmacokinetics, especially in patients with renal or hepatic dysfunction. Flumazenil, a competitive benzodiazepine receptor antagonist, has been primarily utilized f…

Evoked craving in high-dose benzodiazepine users.

INTRODUCTION: Benzodiazepines (BDZs) are among the most abused substances worldwide, and high-dose BDZ abuse is considered a specific type of addiction. Cue reactivity (CR) is a hypersensitivity to motivational stimuli and, in substance use disorders, it increases craving and facilitates relapse, especially in chronic users. Virtual reality (VR) may be a viable technology to implement in CR paradigms. The general objective of this study is the implementation of a VR protocol to identify the causal relationship between the environmental features of a specific setting and craving responses in BD…

Impact of benzodiazepine use on the risk of occupational accidents.

Benzodiazepines (BZDs) are drugs commonly used for treating insomnia and anxiety. Although they are known to induce cognitive and psychomotor impairments, their effect on the risk of causing accidents at work remains understudied. The objective of this study is to estimate this risk by differentiating between the recommended use and overuse of these drugs (i.e., uninterrupted use for four months). The data come from the French National Health Data System, which provide a population composed of French people who had at least one work accident (WA) from 2017 to 2019 (approximately 2.5 million pe…

Benzodiazepine and Zolpidem-Induced Bradycardia: A Case Report.

Benzodiazepines and zolpidem are commonly used in suicide attempts. They have previously been thought to have very few side effects outside of central nervous system depression, and are considered relatively safe even in overdoses. Cardiovascular manifestations of these overdoses are exceedingly rare. We depict a case where an unknown woman appearing to be in her 50s presented to our emergency department somnolent after a large ingestion of alprazolam, clonazepam, and zolpidem. During the evaluation, the patient became bradycardic to 35 beats per minute on the monitor, which responded to atrop…
