[Gastroesophageal reflux disease].

The gastroesophageal reflux is a common clinical manifestation and it can be present under normal physiologic circumstances or may cause esophageal mucosal damage. This disease is currently believed to be ultifacorial in etiology, but the key in the dysfunction of the antireflux barrier, prolonged esophageal acid clearance and gastric emptying and lack in the esophageal mucosal resistance. The exposición repeatedly to clorhidric acid, pepsin and sometimes to bile salts and pancreatic enzymes may cause mucosal damage. The typical symptoms are heartburn and regurgitation, but the spectrum of man

The development of extrahepatic portal obstruction after undergoing multiple operations for a congenital dilatation of the bile duct: report of a case.

As a long-term complication after undergoing a cystenterostomy for a congenital dilatation of the bile duct, liver cirrhosis due to stenosis of the anastomosis or reflux cholangitis has been reported in conjunction with subsequent portal hypertension. We treated a 48-year-old Japanese woman who developed both portal hypertension and pancytopenia after undergoing multiple operations for a congenital dilatation of the bile duct. She underwent a Hassab's operation in July 1994, when an occlusion of the extrahepatic portal vein, which resulted in portal hypertension, was first noted; the liver was

Idiopathic spontaneous perforation of the common bile duct in an adult.

Spontaneous perforation of the common bile duct (SPCBD) is an extremely rare clinical entity in adults, typically idiopathic in origin, though potential causes include distal obstruction, reflux of pancreatic secretions, intramural vascular thrombosis, infection, or direct erosion by stones. We present a rare case of idiopathic SPCBD in an adult who had undergone a previous cholecystectomy. Imaging findings revealed biliary dilatation and a fluid collection extending to the paracolic gutter, posing a challenge in determining whether it was a collection related to pancreatitis or other cause. Symptomatic pancreatic duct stones in the disconnected bile duct: A case series.

BACKGROUND: Pancreaticobiliary maljunction (PBM) refers to the union of the pancreatic and biliary ducts outside of the duodenal wall. Patients are at increased risk of bile duct and gallbladder cancer, likely secondary to pancreatic juice refluxing into the biliary tree, and it is recommended that they undergo biliary diversion. METHODS: This is a case series of all patients in our institution with PBM and bilioenteric anastomosis who presented with symptomatic pancreatic duct stones in a disconnected bile duct. IRB approval was obtained prior to the initiation of the study. RESULTS: We des

In vivo monitoring of the gastrooesophageal system using optical fibre sensors.

In the present paper optical fibre sensors for the detection of foregut diseases are described, in particular, sensors for the detection of bile, carbon dioxide and pH. Bile-containing refluxes are measured by means of a sensor which uses bilirubin as natural marker. The sensor, which is already present on the market, has been clinically validated by various hospitals. The clinically relevant parameter is the exposure time of the stomach/oesophagus mucosa to the bile. When measured in the oesophagus, it has been shown to be closely correlated with the onset of Barrett's oesophagus or general o

Pathophysiology of gastro-oesophageal reflux disease.

Gastro-oesophageal reflux disease is a condition in which the reflux of gastric contents into the oesophagus provokes symptoms or complications and impairs quality of life. Typical symptoms of gastro-oesophageal reflux disease are heartburn and regurgitation but gastro-oesophageal reflux disease has also been related to extra-oesophageal manifestations, such as asthma, chronic cough and laryngitis. The pathogenesis of gastro-oesophageal reflux disease is multifactorial, involving transient lower oesophageal sphincter relaxations and other lower oesophageal sphincter pressure abnormalities. As

[An experimental study on pancreatitis associated with anomalous junction of the pancreaticobiliary ductal system with special reference to the activation of prophospholipase A2 in bile].

Congenital dilatation of the bile duct is often accompanied by recurrent pancreatitis. Reflux of pancreatic juices into the bile duct through anomalous junction and activation of pancreatic zymogens could be involved in its pathophysiological mechanism. In order to test this hypothesis, a rat model was designed according to Block's method with some modifications, and kinetics of activation of prophospholipase A2 (pro-PLA2) in bile was examined by measuring phospholipase A2 (PLA2) activity and by immunoblot analysis, using anti-rat pancreatic enzyme antibody, by which PLA2 could be separated fr

Acute pancreatitis: etiology and common pathogenesis.

Acute pancreatitis is an inflammatory disease of the pancreas. The etiology and pathogenesis of acute pancreatitis have been intensively investigated for centuries worldwide. Many causes of acute pancreatitis have been discovered, but the pathogenetic theories are controversial. The most common cause of acute pancreatitis is gallstone impacting the distal common bile-pancreatic duct. The majority of investigators accept that the main factors for acute billiary pancreatitis are pancreatic hyperstimulation and bile-pancreatic duct obstruction which increase pancreatic duct pressure and active tr

Pathophysiology and treatment of gastro-oesophageal reflux disease.

Gastro-oesophageal reflux occurs when the pressure barrier of the lower oesophageal sphincter (LOS) fails due to a low basal pressure (less than or equal to 6 mm Hg), sphincteric relaxations or a noncompensated increase in intragastric pressure. This reflux becomes pathological when it leads to symptoms severe enough for the patient to seek medical help or results in reflux oesophagitis or its complications. Damage to the oesophageal mucosa develops when the balance between aggressive and defensive factors is no longer in equilibrium. The main aggressive factor is acid-pepsin or alkaline bile

Reimplantation of the ampulla of Vater after total pancreatectomy for nesidioblastosis.

Total pancreatectomy is often required to control hypoglycemic seizures in infants with nesidioblastosis. If duodenal devascularization results, reconstruction is difficult because standard biliary-intestinal anastomoses may reflux, or, in infants, become strictured. Therefore, surgeons are loath to perform such procedures. To avoid these complications, the common bile duct and the ampulla of Vater were implanted into the reconstructed duodenum.

Pathophysiology Underlying Manifestation of Pancreaticobiliary Maljunction Throughout Life.

Pancreaticobiliary maljunction (PBM) is an anomaly in which the bile and pancreatic ducts join outside the area of action of the sphincter. PBM is typically accompanied by choledochal cysts (dilated PBM); however, cases of PBM without biliary dilatation (nondilated PBM) have also been reported. PBM manifests throughout life. The presentation remains unclear due to age-dependent differences in its pathophysiology. PBM symptoms may be attributed to biliary obstruction caused by substances that differ with age: calcium bilirubinate debris in neonates, protein plugs in children, and gallstones in

Mechanisms behind changes in gastric acid and bicarbonate outputs during the human interdigestive motility cycle.

Human gastric interdigestive acid and bicarbonate outputs vary cyclically in association with the migrating motor complex (MMC). These phenomena were studied in 26 healthy volunteers by constant-flow gastric perfusion, with continuous recording of pH and Pco2 in mixed gastric effluent and concomitant open-tip manometry of gastroduodenal motility. Stable acid and bicarbonate outputs were registered during less than 50% of the MMC cycle. Acid secretion started to increase 71 +/- 3% into the cycle, with maximum output during antral phase III. Bicarbonate output increased biphasically 1) 40 +/- 5%
