INTRODUCTION: Cholecystoenteric fistula (CEF) is rare in biliary tract surgery, and cholecystogastric fistula (CGF) is the rarest form of CEF. Although open cholecystectomy with the closure of the fistula is the gold standard treatment for nonobstructing biliary-enteric fistulas, the optimal treatment for CGF has not been established. Laparoscopic and endoscopic cooperative surgery (LECS), a minimally invasive surgery for gastric submucosal tumors, reportedly helps achieve favorable postoperative outcomes. This report presents a case wherein CGF was treated with LECS.

Spontaneous cholecystocutaneous fistula is an exceptionally unusual complication of chronic calculous cholecystitis now. The remarkable drop in incidence is probably associated with the introduction of antimicrobial therapy and early surgical management of biliary tract disease. We report a case of spontaneous cholecystocutaneous fistula in a patient who presented with an abscess in the right upper quadrant.

Our patient presented with dysuria and pneumaturia without any prior urologic instrumentation. History included choledocolithiasis requiring endoscopic retrograde cholangiopancreatography and biliary stenting. Imaging showed a large bladder stone. The patient was taken to surgery and found to have diverticulitis. The sigmoid was resected and the bladder was found to have a small fistula tract. The bladder was opened and a large calculus was identified and extracted. The stone was opened and found to contain a biliary stent. Although biliary stenting is generally considered safe, migration can

The cholecystocolonic fistula (CCF) is an atypical variant of biliary disease, and it is the second most common intestinal fistula after cholecystoduodenal fistula. Intraoperative diagnosis is frequent, which implies challenging surgical management, especially in patients, often aged, with comorbidities. The rarity of this condition, atypical and various presentation, diagnostic and management complexity, makes it a unique surgical entity. We report our experience of an 84-year-old man with a history of chronic cholecystitis who presented with nonspecific symptoms. The imaging tests aroused th

Cholecystocolonic fistula is an unusual complication of biliary tract disease. Many of the signs and symptoms of these fistulas are nonspecific, so the diagnosis is often not suspected preoperatively. It is important to make the diagnosis then to prevent fecal contamination when the fistula is divided. We recently encountered a patient who, while undergoing laparoscopic cholecystectomy, was found to have a fistula between the gallbladder and the proximal transverse colon. Important features in the management of this case are (1) maintaining a high index of suspicion for the presence of this co

Biliary fistulae occurred in 5 out of 8 patients in both groups after primary reconstructive surgery. Biliary stricture was observed in 3 cases due to external biliary drainage failure followed by peritonitis. 7 patients underwent reconstructive surgery with positi …

Endoscopic management of biliary diseases in patients with surgically altered anatomy can be challenging because the altered anatomy makes it difficult to insert an endoscope into the biliary orifice. Even if insertion is feasible, the worse maneuverability of the endoscope and the restriction in available devices and techniques could complicate the procedure. Recently, endoscopic ultrasound-guided antegrade intervention (EUS-AG) has been reported as a useful management method for biliary diseases, especially in patients with surgically altered anatomy. In EUS-AG, the biliary disease is manage

Pyogenic liver abscesses are commonly caused by biliary tract infections. We report here a case of liver abscess developed secondary to a biliary-enteric fistula. A 83 year old diabetic woman was admitted because of sepsis due to Klebsiella pneumoniae and E. Coli and with upper right quadrant pain. Six months before admission, a laparoscopic cholecystectomy was performed. The abdominal sonography showed a liver abscess associated with an important aerobilia. The Magnetic Resonance Cholangiography showed a choledocho-colic fistula with an important inflammatory background. There was no evidence

BACKGROUND: Pancreaticogastrostomy has been known as a method of reconstruction after pancreaticoduodenectomy for almost 60 years. According to some published reports, the pancreatic fistula rate for pancreaticogastrostomy is low. The purpose of this study is to present the results of pancreaticogastrostomy after pancreaticoduodenectomy at one department of our university hospital.
METHODS: From 1994 to 2004, 159 patients underwent pancreaticoduodenectomy. Whipple procedures were performed in 125 cases and 34 underwent pylorus-preserving modification. In all of them, pancreaticogastrostomy wa

BACKGROUND: Comparative studies of the bile microbiota in different biliary obstructive infections remain limited. This study aims to characterize bile microbiota and investigate differences in microbial profiles across various biliary obstructive diseases.
METHODS: This study included patients with biliary obstructive diseases admitted to Jinling Hospital and Suqian First Hospital. The cohort consisted of individuals with benign biliary disorders, malignant biliary obstruction, and biliary obstruction secondary to severe acute pancreatitis (SAP) or intestinal fistulas. A total of 133 bile sa

Three cases of gallstone ileus are reported and the literature reviewed. There seems recently to be a trend towards conservative surgery aiming at relief of the obstruction only. Definitive surgery to deal with a biliary-enteric fistula, should be undertaken primarily only in fit patients; and should be considered as a second stage procedure only in those patients who develop biliary complications.

Cholecystoenteric fistulas are a complication of biliary disease, having an estimated incidence in the range of 0.1 - 0.5 percent. Roughly 8-27 percent of these are of the cholecystocolonic type,1 occurring most commonly at the hepatic flexure. An 82-year-old male presented to our tertiary hospital emergency department and was diagnosed with a cholecystocolonic fistula (CCF) by CT imaging. Surgical evaluation confirmed the CT diagnosis with subsequent removal of the fistula and gallbladder, which was curative.
