Systemic therapies for psoriasis: methotrexate, retinoids, and cyclosporine.

Despite the current use and ongoing development of the biological therapies 'traditional' systemic agents will continue to form a key part of the therapeutic armamentarium for patients with severe psoriasis. Long-term maintenance therapy with retinoids and methotrexate is cost-effective and, for many patients with psoriasis, life changing. Regular monitoring is required for both treatments, particularly methotrexate to prevent significant bone marrow suppression and hepatotoxicity. Ideally, cyclosporine should be used for short courses of 3 to 4 months duration, within which it provides excell

Feminization in hepatocellular carcinoma corrected by chemotherapy: a case report.

A 17-year-old boy with metastatic hepatocellular carcinoma and feminization became asymptomatic after administration of antineoplastic agents. Serum estradiol level, urine estrogens, and tumor estrogen receptors were changed after the treatment. Achievement of partial remission resulted in normalization of the serum sex hormone profile and disappearance of the feminization in spite of the presence of residual tumor. Thus, serum hormone levels alone cannot be used as tumor markers for control of therapy. This suggests that chemotherapy may have altered the biological behavior of the tumor, whic

Glucocorticoids in the management of systemic juvenile idiopathic arthritis.

Glucocorticoids have been the mainstay of treatment for many years in systemic-onset juvenile idiopathic arthritis (sJIA), causing important side effects and some difficulties in the management of this disease. Until the introduction of biologic agents, oral glucocorticoids were used to control fever and other systemic features for several months or even years if systemic manifestations persisted. Nowadays, clinicians have valid alternatives that have revolutionized the natural history of sJIA. Biologic agents, such as the interleukin-1 inhibitors anakinra and the more recent canakinumab, or t

Simulation and virtual reality in medical education and therapy: a protocol.

Continuing medical education has historically been provided primarily by didactic lectures, though adult learners prefer experiential or self-directed learning. Young physicians have extensive experience with computer-based or "video" games, priming them for medical education--and treating their patients--via new technologies. We report our use of standardized patients (SPs) to educate physicians on the diagnosis and treatment of biological and chemical warfare agent exposure. We trained professional actors to serve as SPs representing exposure to biological agents such as anthrax and smallpox

Emerging biological therapies for psoriatic arthritis: A systematic review.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory condition characterized by joint involvement, enthesitis, dactylitis, and skin psoriasis. The pathophysiology of PsA is complex, driven by dysregulated immune responses, making targeted therapies essential for managing symptoms. Despite the success of biologic disease-modifying anti-rheumatic drugs (bDMARDs) like tumor necrosis factor (TNF) inhibitors, many patients experience suboptimal responses or adverse effects, necessitating the development of new therapies. Emerging biologics targeting interleukin (IL)-17, IL-23, and Janus

Pyrrolnitrin and Hydrogen Cyanide Production by Pseudomonas chlororaphis Strain PA23 Exhibits Nematicidal and Repellent Activity against Caenorhabditis elegans.

Pseudomonas chlororaphis strain PA23 is a biocontrol agent able to suppress growth of the fungal pathogen Sclerotinia sclerotiorum. This bacterium produces an arsenal of exometabolites including pyrrolnitrin (PRN), phenazine (PHZ), hydrogen cyanide (HCN), and degradative enzymes. Production of these compounds is controlled at both the transcriptional and posttranscriptional levels by the Gac-Rsm system, RpoS, PsrA, and the Phz quorum-sensing system. Beyond pathogen-suppression, the success of a biocontrol agent is dependent upon its ability to establish itself in the environment where predatio

Novel Targeted Biological Agents for the Treatment of Atopic Dermatitis.

Atopic dermatitis (AD) is a common inflammatory dermatologic disease clinically characterized by intense itch, recurrent eczematous lesions, and a chronic or relapsing disease course. Mild-to-moderate AD can be controlled by using moisturizers and topical immunomodulators such as topical corticosteroids and calcineurin inhibitors. If topical therapies fail, phototherapy and systemic immunosuppressant therapies, such as ciclosporin, methotrexate, and azathioprine, can be considered. However, relapse and side effects could still occur. The pathogenesis of AD involves epidermal barrier dysfunctio

Exorchis sp. in the catfish Silurus asotus and Oncomelania hupensis in marshlands of Poyang Lake, China: A potential biological control tool for Schistosoma japonicum.

Oncomelania hupensis is the obligate intermediate host of Schistosoma japonicum, highlighting the medical importance of interrupting this unique and long-standing parasite-host interaction in controlling schistosomiasis transmission. It has been reported that a catfish trematode Exorchis sp. could have the potential to function as an effective anti-schistosomal agent in the snail host. However, the feasibility of this eco-friendly biological control strategy should be comprehensively investigated and evaluated in endemic areas for schistosomiasis. In this study, a field survey was conducted fr

Frequency of kdr mutations in the voltage-sensitive sodium channel (VSSC) gene in Aedes aegypti from Yogyakarta and implications for Wolbachia-infected mosquito trials.

BACKGROUND: In the inner city of Yogyakarta, Indonesia, insecticide resistance is expected in the main dengue vector, Aedes aegypti, because of the intensive local application of pyrethroid insecticides. However, detailed information about the nature of resistance in this species is required to assist the release of Wolbachia mosquitoes in a dengue control program, so that we can ensure that insecticide resistance in the strain of Ae. aegypti being released matches that of the background population. METHODS: High-resolution melt genotyping was used to screen for kdr mutations associated with

Understanding the biosynthesis, metabolic regulation, and anti-phytopathogen activity of 3,7-dihydroxytropolone in Pseudomonas spp.

The genus Pseudomonas is a prolific source of specialized metabolites with significant biological activities, including siderophores, antibiotics, and plant hormones. These molecules play pivotal roles in environmental interactions, influencing pathogenicity, inhibiting microorganisms, responding to nutrient limitation and abiotic challenges, and regulating plant growth. These properties mean that pseudomonads are suitable candidates as biological control agents against plant pathogens. Multiple transposon-based screens have identified a Pseudomonas biosynthetic gene cluster (BGC) associated w…

Biologic drugs in the treatment of polyarteritis nodosa and deficit of adenosine deaminase 2: A narrative review.

Polyarteritis nodosa (PAN) is a medium vessels vasculitis variously involving different organs and systems, sometimes with an aggressive course, leading to death or disability in a significant number of cases. First-line treatment usually relies on steroids and classical immunosuppressants, but a growing number of case reports and small case series shows the potential role of biologic drugs, mostly anti-tumor necrosis factor (TNF)-α agents, in inducing and maintaining remission in patients affected by PAN. Similarly, the recently described autoinflammatory disease named deficit of adenosine de

Dietary enrichment of Tyrophagus putrescentiae improves Amblyseius swirskii rearing efficiency: linking prey biochemistry to predator demography.

Optimizing nutrient transfer across trophic levels has become an important approach to enhance the biological efficiency and sustainability of predator-prey rearing systems supporting augmentative biological control. This study examined how dietary enrichment of the factitious prey Tyrophagus putrescentiae (Schrank) influences its biochemical composition and subsequent effects on the demographic performance of the predatory mite Amblyseius swirskii Athias-Henriot; one of the most widely used biocontrol agents in protected crops. Eight dry diets were tested, including a basal mixture (Diet 1: w

Morphological and genetic variation of the pine shoot tunnel beetle Placusa pinearum (Staphylinidae) in China.

Placusa pinearum, a newly described beetle species found living in pine shoot tunnels bored by the pine tip moth, Dioryctria rubella (Pyralidae), has potential as a vector to transport biological agents for controlling this moth pest of pine trees because of the high degree of niche overlap (co-occurrence) between them. In order to help determine the suitability of potential biological control vectors, it is useful to obtain knowledge concerning the intraspecific variation of the vector. We examined P. pinearum collected from 14 different geographical populations in China using morphological a

Specialty-Based Disparities in Biologic Use and Retention in Psoriatic Arthritis: A Nationwide Korean Claims Analysis.

AIM: Biologic agents play a pivotal role in controlling both articular and cutaneous manifestations of psoriatic arthritis (PsA). Although international guidelines emphasize the appropriate use of biologics, Korea lacks standardized treatment protocols tailored for PsA across specialties. We therefore aimed to examine specialty-specific prescription patterns and treatment retention using nationwide claims data. METHODS: Health Insurance Review and Assessment (HIRA) claims data from 2011 to 2022 were analyzed, including patients with PsA who initiated treatment with their first biologic agent.

Detection of atmospheric carbon dioxide with air-lasing-based coherent Raman spectroscopy.

Coherent anti-Stokes Raman scattering (CARS) is recognized as a powerful technique for chemical sensing, biological imaging, and combustion diagnostics, but it is rarely used in atmospheric detection due to complex multi-beam configuration and fine spatio-temporal control. Air lasing provides an attractive probe source for CARS, largely simplifying its implementation. Herein, we successfully detected atmospheric carbon dioxide (CO2) using air-lasing-based CARS spectroscopy and quantified its concentration to be 485.5 ppm and 502.0 ppm based on two CARS signals. The measured concentrations exhi
