Incidence and risk factors of acute transfusion reactions in patients admitted in intensive care unit on active monitoring after transfusion.

BACKGROUND AND OBJECTIVES: Transfusion reaction is often masked in critically ill patients because of underlying disease or certain pre-medications. The study was planned to know the incidence and determine risk factors associated with these acute transfusion reactions (ATRs) in intensive care (ICU) patients by actively monitoring the transfusion events. MATERIAL AND METHODS: In this prospective observational study, critically ill patients admitted to ICU and having an ICU stay of 24 h prior to transfusion were enrolled. Each enrolled patient was actively monitored after each transfusion of b

A comprehensive analysis of transfusion trends and patient outcomes in blood utilization practices at Jimma university medical center.

In resource-limited settings like Ethiopia, ensuring adequate blood availability and optimizing its use remain a significant challenge. Comprehensive studies on blood utilization practices are essential for minimizing unnecessary transfusions and enhancing transfusion services, yet such research remains limited. This study aimed to systematically evaluate the efficiency of blood utilization, analyse transfusion trends, and assess patient outcomes. A prospective cross-sectional study was conducted at the largest referral hospital in Southwest Ethiopia, from June 1 to August 30, 2023. Data were

Factor XI deficiency and aortic valve replacement: Perioperative management.

Severe factor XI deficiency (hemophilia C) is a rare coagulation disorder. A 73-year-old woman, a homozygote for factor XI deficiency, required aortic valve replacement. An initial dose of 15 U kg-1 of factor XI concentrate was administered preoperatively and on postoperative day 3. During surgery, concentrated red cells, fresh frozen plasma, platelets, tranexamic acid, and fibrinogen were transfused. Intraoperative bleeding and total chest drainage were minimal. Postoperatively, there was no need for further transfusions and no bleeding or thrombotic complications occurred. The patient was we

Posterior reversible encephalopathy syndrome following blood transfusion in a patient with factor X deficiency: Is it an unusual systemic manifestation of an adverse transfusion reaction?

Adverse neurological transfusion reactions including posterior reversible encephalopathy syndrome (PRES) following blood transfusion are rare. Our case an 18-year-female with known Factor X deficiency with menorrhagia developed severe hypertension, followed by generalised tonic clonic convulsions apparently after blood component transfusion. She had earlier received 4 units of red blood cells (RBC) for anaemia and 10 units of fresh frozen plasma (FFP) for menorrhagia (with prolonged PT and APTT) within short span of time at another hospital. There was no history of hypertension, convulsions, a…

Barriers to Effective Transfusion Practices in Limited-Resource Settings: From Infrastructure to Cultural Beliefs.

BACKGROUND: Surgery and anesthesia are indivisible parts of health care, but safe and timely care requires more than operating rooms and skilled providers. One vital component of a functional surgical system is reliable blood transfusion. While almost half of all blood is donated in high-income countries (HICs), over eighty percent of the global population lives outside of these countries. High-income countries have on average 30 donations per 1000 people, and the average age of transfusion recipient is over 65. Most low-income countries (LICs) have fewer than five donations per 1000 people, w

The management of blood safety in the presence of uncertain risk: a United kingdom perspective.

Millions of patients in the UK benefit from the use of both plasma derivatives and blood components that are seen as critical interventions … epidemiology of transfusion-transmitted infections, bacterial contamination of platelets, incorrect blood component transfusion, and variant Creutzfeldt-Jakob disease, require further attention. Risks such as these are often complex, and there is a difficult balance to be struck between donors/recipients' benefit and adequacy of blood supply. The introduction of any new safe …

Rotational ThromboElastometry-guided blood component administration versus standard of care in patients with Cirrhosis and coagulopathy undergoing Invasive ProcEdures (RECIPE): study protocol for a randomised controlled trial.

BACKGROUND: Patients with cirrhosis often undergo invasive procedures both for management of complications of their advanced liver disease, including treatment for hepatocellular carcinoma, as well as underlying comorbidities. Despite a current understanding that most patients with cirrhosis are in a rebalanced haemostatic state (despite abnormalities in conventional coagulation tests, namely INR and platelet count), patients with cirrhosis are still often given prophylactic blood components based on these conventional parameters, in an effort to reduce procedure-related bleeding. What is the Incidence of Early Antibody Formation After Resuscitation of Hemorrhage? A Comparison of RH(D)+ Whole Blood and RH(D)- Red Blood Cells in 3531 Trauma Patients.

OBJECTIVE: To evaluate the risk of red blood cell alloimmunization following transfusion of low-titer group O whole blood (LTOWB) compared with component therapy in severely injured trauma patients. SUMMARY BACKGROUND DATA: LTOWB is increasingly used for trauma resuscitation because of logistical advantages and potential hemostatic benefits. However, concerns remain regarding alloimmunization, particularly anti-D antibody formation in Rh(D)-negative patients exposed to Rh(D)-positive blood products. METHODS: A retrospective cohort study was conducted at a single Level I trauma center using t

Whole blood resuscitation is associated with decreased end-organ dysfunction in pediatric trauma patients.

INTRODUCTION: Low Titer Group O Whole Blood (WB) has emerged as a safe and efficient therapeutic agent for hemostatic resuscitation in injured children. The relationship between type of blood product transfused and incidence of organ dysfunction is not well elucidated. We hypothesize that WB transfusion is associated with decreased organ dysfunction compared with component therapy (CT). METHODS: This is a retrospective observational study of injured children ages 0 year to 17 years utilizing the TQIP database (2020-2022). Inclusion criteria were receipt of any blood product within 4 hours of

Transfusion issues in the cancer patient.

Blood transfusions are a lifesaving but transient therapy used to correct deficiencies of blood cells and coagulation factors that occur in cancer patients. Anemia can occur in cancer patients as a result of hemolysis, blood loss, or bone marrow failure. The blood component most commonly recommended for the treatment of anemia is packed red blood cells. Coagulation disorders are common with hemangiosarcoma and diffuse hepatic tumors. Fresh frozen plasma is used as a source for replacement coagulation factors for the treatment of disseminated intravascular coagulation or other cancer-associated

Use of group A thawed plasma in emergency transfusions at a pediatric quaternary care center.

BACKGROUND: Balanced plasma/red blood cell transfusions have shown survival benefit in emergency scenarios. To improve plasma availability, we implemented 5-day group A thawed plasma at our pediatric hospital in February 2021. STUDY DESIGN AND METHODS: We maintain thawed group A plasma units (5-day shelf-life) ready for immediate issue in the blood bank (since February 2021) and trauma code room (since August 2022). Group A plasma (un-titered) is issued for patients with unknown blood type during emergencies. We retrospectively reviewed records and laboratory results of recipients to assess s

Quality management of a massive transfusion protocol.

BACKGROUND: Massive transfusion is a response to massive uncontrolled hemorrhage. To be effective, it must be timely and address the patient's needs for blood volume, oxygen transport, and hemostasis. STUDY DESIGN AND METHODS: A review was performed on all activations of the massive transfusion protocol (MTP) in a hospital with large emergency medicine, trauma, and vascular surgery programs. Indications, transfused amounts, and outcomes were determined for each MTP event to determine appropriateness of MTP use. Results are presented as descriptive statistics, categorical associations, and sim

Overt immediate hemolytic transfusion reaction attributable to anti-Wr(a).

Wr(a) is a low-prevalence antigen. Anti-Wr(a) is a relatively common antibody present in approximately 1 in 100 healthy blood donors. Anti-Wr(a) is reported to cause different degrees of hemolysis in transfusion and in HDN, ranging from benign to severe. This report describes an acute overt hemolytic transfusion reaction in a patient whose serum contained anti-Wr(a) and who received a Wr(a+) RBC component.
