Novel Approach for the Surgical Management of Nonunion After Minimally Invasive Bunionectomy Using Bone Harvesting and Bioabsorbable Screw Fixation: A Case Report.

Hallux valgus is a common forefoot deformity that can be surgically corrected with minimally invasive bunionectomy, which offers advantages such as smaller incisions, reduced soft tissue trauma, and shorter recovery. Despite these benefits, nonunion remains a challenging complication with limited consensus on optimal management. We report the case of a 42-year-old woman with situs inversus totalis who presented with painful left hallux abducto valgus unresponsive to conservative therapy. She underwent a minimally invasive bunionectomy, but at seven months, she continued to have persistent pain

Novel MRI-Based Pedicle Bone Quality Score Independently Predicts Pedicle Screw Loosening after Degenerative Lumbar Fusion Surgery.

UNLABELLED: Pedicle screw loosening after posterior lumbar fusion is associated with poor bone quality, which often determines screw pull-out strength, insertion torque, and vertebral body loading characteristics. Magnetic resonance imaging (MRI)-based vertebral bone quality (VBQ) score were associated with poor bone quality. Current evidence suggests that pedicle bone quality (PBQ) has a greater impact on screw stability. However, the correlation between MRI-based PBQ score and screw loosening has not been reported. PURPOSE: To introduce and evaluate an MRI-based PBQ score to determine its e

Pedicle Screw Pull-Out Strength Between the Jamshidi Needle and Pedicle Probe Techniques in an Osteoporotic Cancellous Bone Model: A Biomechanical Study.

Introduction Spinal fusion surgery with pedicle screws is commonly performed to stabilize the spine of osteoporotic patients. However, securing a strong screw fixation in osteoporotic bone presents significant challenges due to the reduced bone density. This study aimed to compare the biomechanical performance in an osteoporotic bone model of pedicle screws inserted using two different techniques, the Jamshidi needle technique and the pedicle probe technique, as well as the influence of tapping on both these techniques. The research sought to determine if the surgical device used in aiding ped

Prevention of Bone Cement Displacement in Kümmell Disease without Neurological Deficits through Treatment with a Novel Hollow Pedicle Screw Combined with Kyphoplasty.

OBJECTIVE: Displacement of bone cement following percutaneous vertebral augmentation for Kümmell disease (KD) presents a significant concern, resulting in increasing back pain and compromising daily activities. Unfortunately, current literature does not yet establish a validated and minimally invasive surgical intervention for this issue. This study aims to investigate the effects of a novel hollow pedicle screw combined with kyphoplasty (HPS-KP) in preventing bone cement displacement following simply percutaneous kyphoplasty for the management of KD. METHODS: A total of 22 patients (six male

Plate fixation optimization for distal femoral fractures with segmental bone loss: Defining the preferred screw distribution using finite element analysis.

OBJECTIVES: Distal femur fractures can exhibit extensive comminution, and open fractures may result in bone loss. These injuries are under high mechanical demands when stabilized with a lateral locked plate (LLP), and are at risk of non-union or implant failure. This study investigates the optimal LLP screw configuration for distal femur fractures with a large metadiaphyseal gap of 5 cm. METHODS: A finite element (FE) model, validated against experimentally measured strains and displacement, evaluated pull-out forces and stress concentration on typical implants under clinical conditions corre

Reinforcement strategy for medial open-wedge high tibial osteotomy: a finite element evaluation of the additional opposite screw technique and bone grafts.

BACKGROUND AND OBJECTIVE: bone grafts (bgs) and the opposite screw insertion technique are reported to enhance initial stability after medial open-wedge high tibial osteotomy (OWHTO); however, it is unclear how the general and local biomechanical stability of the proximal tibia is affected by these reinforcement strategies. In this study, we aimed to assess the biomechanical differences among different fixation configurations for OWHTO under two loading conditions using finite element analysis, and to assess the biomechanical contribution of an opposite screw insertion. METHODS: Models of the

[Improvement in the primary stability of the dynamic hip screw osteosynthesis in unstable, pertrochanteric femoral fractures of osteoporotic bones by a new glass ionomer cement].

Internal fixation of unstable intertrochanteric fractures of osteoporotic femurs often leads to complications, which are primarily caused by the mechanically weak bone. The aim of this study was to test whether a new glass-ionomeric cement could reinforce the weak trabecular bone of the femoral head and thus improve the preliminary stability of dynamic hip screws (DHS) used for internal fixation. As a standardized model, a intertrochanteric osteotomy was performed on eight pairs of human, female, cadaver femora; a bony defect was also created medially to diminish support of the femoral neck. Response to Letter to the Editor for "Femoral Bone Mineral Density Shows Stronger Correlation With Pedicle Screw Insertional Torque than Lumbar Bone Mineral Density or Hounsfield Units: A Retrospective in Vivo Study".

We thank the authors for their insightful comments regarding our study evaluating the relationship between bone mineral density (BMD), Hounsfield units (HU), and pedicle screw insertional torque. While trajectory-specific CT-based assessments represent an important and evolving approach to evaluating local bone characteristics, existing literature has generally demonstrated only moderate correlations between DEXA or conventional HU measurements and insertional torque. In our study, we extended prior work by evaluating both systemic and vertebra-specific metrics, including femoral and lumbar DEX

Clinical evaluation and finite element analysis of bone cement-augmented anterolateral screw fixation versus percutaneous bilateral pedicle screw fixation co-applied with oblique lumbar interbody fusion for single-level lumbar degenerative diseases with osteoporosis.

BACKGROUND: As the population ages, there is an increasing trend in patients with lumbar degenerative diseases (LDD) complicated by osteoporosis seeking lumbar fusion surgery. However, standardized strategies for minimally invasive surgical procedures among these populations still need improvement in clinical practice. PURPOSE: This study was to integrate clinical and biomechanical approaches to investigate and demonstrate the effectiveness of oblique lateral interbody fusion combined with bone cement-augmented anterolateral screw (OLIF-BCAAS) in such patients. STUDY DESIGN: A single-center,

Fusionless All-Pedicle Screws for Posterior Deformity Correction in AIS Immature Patients Permit the Restoration of Normal Vertebral Morphology and Removal of the Instrumentation Once Bone Maturity Is Reached.

The aim of this study was to report the restoration of normal vertebral morphology and the absence of curve progression after the removal of instrumentation in AIS patients that underwent posterior correction of the deformity by a common all-screws construct without fusion. A series of 36 AIS immature patients (Risser 3 or less) were included in the study. Instrumentation was removed once the maturity stage was complete (Risser 5). The curve correction was assessed pre- and postoperatively, before instrumentation removal, directly post-removal, and more than two years after instrumentation was

"Close to the tip, with little bone to grip": stabilizing two periprosthetic proximal femur fractures above a distal femur megaprosthesis using a combination of DHS and 3.5 mm screws.

The incidence of periprosthetic fractures is increasing, presenting significant challenges due to patient longevity and the complexity of repeated surgeries. This report details the successful treatment of a previously unreported periprosthetic fracture pattern using a modified dynamic hip screw (DHS) technique. Two cases involved patients with extracapsular fractures in short proximal femur segments above megaprostheses. The fractures were reduced and stabilized with a DHS device, complemented by 3.5 mm screws from a different manufacturer to achieve effective bicortical fixation around the t

Comparison of Outcomes Between a Fixed-Loop Cortical Suspensory Device and Metal Interference Screw for Femoral Fixation of Bone-Patellar Tendon-Bone Grafts in Anterior Cruciate Ligament Reconstruction: A Scandinavian Registry Study.

BACKGROUND: Use of a cortical suspensory device (CSD) is an alternative fixation method for bone-patellar tendon-bone (BPTB) in the femur. To the authors' knowledge, no study has compared revision outcomes and patient-reported outcomes (PROs) with the traditional interference screw (IS) fixation. PURPOSE: To compare the survival rate of grafts fixated on the femoral side with either a fixed-loop CSD or IS for primary anterior cruciate ligament reconstructions (ACLRs) with BPTB autograft 2 and 5 years after index surgery, as well as to compare PROs between the same 2 treatment groups based on

Cancellous Autograft Transfer From Screw-Hole to the Proximal Tibial Osteotomy Opening Gap for Early Bone Consolidation Without Donor-Site Morbidity.

Autologous bone grafting can enhance gap healing after medial open-wedge high tibial osteotomy and open-wedge distal tibial tuberosity osteotomy, but iliac-crest harvest adds donor-site morbidity and is therefore not routinely performed. We describe a bone autografting technique, termed cancellous autograft transfer from screw-hole, which leverages routinely drilled locking plate screw pilot holes as a local donor source to obtain cancellous autograft without a separate donor-site or additional incision. During plate fixation, cancellous bone is cored from the proximal screw pilot holes using
