Maternal diabetes and fracture risk in offspring: a population-based analysis.

Factors affecting intrauterine environment exerts influence on skeletal health and fracture risk in later life. Diabetes during pregnancy is known to influence birth weight and is associated with fetal overgrowth. However, the effects of maternal diabetes on fracture risk in offspring is unknown. This study was aimed to evaluate the association between maternal diabetes and fracture risk in offspring. Using population-based administrative health data for Manitoba, Canada, we identified deliveries complicated by gestational diabetes and type 2 diabetes between April 1, 1980, and March 31, 2020.

Bilateral axillary arterial aneurysms in siblings with Marfan syndrome.

Axillary arterial aneurysm associated with Marfan syndrome is an extremely rare and dangerous disease because of its potential risk for rupture and impairment of neurologic function in the upper extremities. Treatment of large axillary aneurysms can be surgically challenging because the axillary artery is anatomically located under the clavicle bones and the brachial plexus runs dorsal to the aneurysm. We report 2 cases of bilateral axillary arterial aneurysms in brothers with Marfan syndrome. Revival of a novel approach and cautious attempts during the procedures are described in both cases.

Does obesity affect fracture healing in children?

BACKGROUND: Obesity is a risk factor for various orthopaedic diseases, including fractures. Obesity's influence on circulating hormones and cytokines and bone mineralization ultimately influences the body's osteogenic response and bone mineralization, potentially increasing the risk of fracture and impacting fracture healing. QUESTIONS/PURPOSES: Does obesity delay fracture recovery in overweight or obese children as measured by the time to release to normal activity? Is this average time for return to activity influenced by the mechanism of the injury? Does obesity's effect on mineralization

Intraosseous Arteriovenous Malformations in the Extremities Managed with Coils and Absolute Ethanol Treatment.

BACKGROUND: The extremity intraosseous arteriovenous malformations (AVMs) are rare high flow vascular lesions for which the treatment remains challenging. The aim of the study wasto assess treatment methods, interim results, and complications of coils and absolute ethanol in managing extremity intraosseous AVMs via direct puncture approach. METHODS: From 2009 to 2017, 12 patients (mean age, 27.5 years; range, 3-54 years) with extremity intraosseous AVMs underwent staged coils and absolute ethanol treatment via the direct puncture approach. All patients were symptomatic before the procedure as

Reflex sympathetic dystrophy of the lower extremity in tetraplegia: case report.

Reflex sympathetic dystrophy (RSD) of the upper extremities has been reported to occur following complete and incomplete injuries of the cervical cord. Such reports describe the value of the three-phase radionuclide bone scan (TPBS) in differentiating RSD from pain of other sources. To our knowledge, RSD of the lower extremities has not been reported in a patient with tetraplegia. We report a case of lower extremity RSD in a patient with complete traumatic injury of the cervical cord. The case illustrates the use of the TPBS to differentiate RSD from heterotopic ossification (HO) in the lower

Management of Bone Stress Injuries: A Holistic Approach.

Stress fractures are fatigue fractures of bone that result from an overuse mechanism. These injuries present most commonly in the legs and feet of distance runners and military personnel but can also occur in the upper extremity and the spine depending on the causative activity. Nutritional, hormonal, and biomechanical factors influence the development of bone stress injuries, and no two bone stress injuries behave identically. A thorough history, detailed physical examination, and proper imaging are required to diagnose and classify these injuries. Management of stress injuries requires nutri

Pitfalls and complications of fractures of the distal radius and ulna in childhood.

Fortunately, most bone forearm fractures in children heal with minimal functional disability. My approach to management of the most common injuries is presented. By following the guidelines outlined in this article, the generalist as well as the most sophisticated upper extremity surgeon should be able to avoid pitfalls and complications when managing forearm fractures in children.

Non-vascularised corticocancellous (tricortical) iliac bone graft longer than 3 cm for non-union after failed surgical treatment.

PURPOSE: We hypothesised that traditional iliac tricortical bone grafts (no vascularised) still have a reasonable role in promoting satisfactory bony healing in non-union defects of certain sizes. Here, we report the clinical/radiological outcomes through a retrospective case series. METHODS: We screened 74 patients with definitive non-union in the long bones of the upper extremities who visited the outpatient department from 2008 to 2018. Among these patients, 25 who met our inclusion/exclusion criteria were investigated. RESULTS: The mean age was 51.92 years, and there were 12, 9, 1, and 3

Concurrent ulnar plastic deformity and ipsilateral humeral condylar fracture: A case report.

INTRODUCTION AND IMPORTANCE: Plastic deformities usually occur in skeletally immature bones not completely ossified due to longitudinal forces on their distal ends, especially the forearm and lower legs. Pediatric lateral humeral condylar fractures (LHCFs) constitute the second most common intra-articular fracture in the upper extremity. Nevertheless, plastic deformities accompanied by other traumatic injuries, including LHCFs, are atypical and rare. This study presents an exceedingly rare case of the concurrence of a plastic deformity and an ipsilateral fracture of the lateral condyle of the

Chronic Overuse Injuries in Young Athletes-Upper Extremity.

Young athletes are uniquely susceptible to overuse injuries of the upper extremity because their open physes and apophyses are structurally weaker than adjacent bone, ligament, and tendon. Repetitive sport-specific loading-torsional stress in throwers, axial compression in gymnasts and climbers-can outpace the repair capacity of growing tissues, producing a spectrum of physeal and apophyseal stress injuries from widening and sclerosis to frank epiphysiolysis. This review examines the mechanisms, clinical presentation, and imaging findings of chronic overuse injuries at key upper extremity site

Paget-Schroetter syndrome in sports activities--case study and literature review.

The authors report 7 patients with thromboses in the upper extremity resembling Paget-Schroetter syndrome. According to their case histories, all patients had a temporal and causal relationship between partially unusual sports activities and the genesis of the thrombosis. The cause of this condition is a strain on the subclavian and axillary veins by retroversion or hyperabduction of the arm. This can entail microtraumatizations of the venous intima, consequently leading to a consecutive local activation of coagulation and to a possible thrombosis of the vessel. A mechanical compression of the

Hybrid fixation for adolescent both-bones diaphyseal forearm fractures: Preliminary results of a prospective cohort study.

INTRODUCTION: Both-bone forearm fractures account for 3.4% of all pediatric fractures and 26% of all pediatric upper extremity fractures. Although non-operative management remains a feasible treatment option for children older than 10 years, they have a higher incidence for malunion due to their limited bone remodelling potential. Therefore, surgical intervention could be advocated for this age group, however the optimal method of surgical fixation in this age group remains controversial.Authors wanted to evaluate hybrid fixation (plating of ulna & elastic nail for radius) of adolescent both-b

Fibrous Dysplasia of Radius Bone-excision and Fibula Graft: A Case Report.

INTRODUCTION: Fibrous dysplasia (FD) is a congenital disorder in which the bone is distorted and replaced by poorly organized and structurally unsound fibrous tissue. The disorder can be localized to a single bone or affects multiple bones. Although any bone can be affected, the bones of the upper extremity are less commonly involved by the disease. The disease process results in deformity of the bones and is often complicated by pathological fractures. CASE REPORT: A 14-year-old girl presented with gradually progressive deformity of the right forearm for the past 1 year associated with mild

Upper Extremity Infections in the Transplant Population.

PURPOSE: As the duration of lifetime survival after organ transplantation continues to increase, the consequences of long-term immunosuppression, such as opportunistic and rare infections, are a high-risk reality. This study examined upper extremity infections in the transplant population to determine the current clinical risk profile, management, and outcomes. METHODS: An institutional database of 16,640 patients who underwent transplantation was queried for upper extremity infections from 2005 to 2017, defined as the presence of infection from the shoulder to the fingertips. The resulting d

Do Patient-Answered Versus Caregiver-Answered Patient-Reported Outcomes Differ in Pediatric Upper Extremity Fracture Care?

INTRODUCTION: Patient-reported outcomes in children can be assessed using self-report or caregiver proxy-report; however, self-reported and proxy-reported outcomes often vary between respondents, a phenomenon called cross-informant variance. This phenomenon has not been studied in pediatric fracture care. This study compares self-reported and proxy-reported patient-reported outcomes in children being treated for upper extremity fractures. METHODS: Children aged 8 to 17 with upper extremity fractures completed pediatric self-report questionnaires and their caregivers completed parent proxy-rep

Comparison of four surgical methods for pediatric forearm double diaphyseal fractures: a retrospective analysis.

OBJECTIVE: One of the most frequent fractures in children is a double forearm fracture. They make up 26% of children's long bone fractures in the upper extremities and their incidence has increased in recent years. In this study, pediatric patients with double forearm fractures were treated using plate screw, intramedullary K-wire (I-KW), intramedullary titanium elastic nails (TENs), and hybrid fixation (HF) to compare the radiographic and functional results. PATIENTS AND METHODS: The printed and digital medical records were retrospectively examined from our hospital's archives after receivin
