Scedosporium apiospermum brain abscess treated with surgery and voriconazole. Case report.

Scedosporium apiospermum is a fungus found in the soil and in contaminated water. Common cutaneous manifestations include Madura foot, a painless swelling on the sole of the foot. Invasive infection is usually associated with immunosuppression. The authors present a 16-month-old immunocompetent boy who had a near-drowning event. Following this, he was severely disabled with spastic quadriparesis. Early computerized tomography scans revealed diffuse hypoxic injury. Magnetic resonance images obtained 3 months after the initial event demonstrated multiple ring-enhancing lesions in the brain. Aspi

[Brain abscess secondary to rhinosinusitis. Therapeutical modalities. Exposition of one case].

Brain abscess (BA) as complication of sinusitis represent around 3% of the cases. Other suppurative intracraneal complications such as epidural abscess, subdural empyema and meningitis are more common than BA. The frontal and ethmoid sinuses form an integral part of anterior and to a lesser extent middle skull base to which the dura is intimately related. Thrombophlebitis of veins associated with paranasal sinuses is considered to be the main route of intracraneal spread of infection. However, osteitis is an uncommon mechanism that typically involves frontal sinus. Chronic sinusitis more than …

Nocardiosis with brain abscess due to an unusual species, Nocardia transvalensis.

The identification of Nocardia transvalensis, an unusual and probably underrecognized cause of nocardial infection, is clinically significant because of this species' resistance to aminoglycosides, a standard antinocardial therapy. Diagnosis requires analytic methods available predominately in reference laboratories. We report a case of disseminated infection with N transvalensis with primary pulmonary involvement and subsequent development of brain abscesses, and review the literature to date. Familiarity with the epidemiology, pathologic findings, and clinical significance of this and other …

An unusual case of neonatal brain abscess following Klebsiella pneumoniae septicemia.

A case of solitary brain abscess in a term neonate caused by Kiebsiella pneumoniae is described. K. pneumoniae, although a common cause of neonatal septicemia, is rarely implicated as an etiological agent for cerebral abscess in this age-group. The interest of this case lies in the rarity of the causative organism and atypical features. In the absence of predisposing factors in the neonate, we suspect that the infection was transmitted vertically from the mother before or at the time of delivery since there was evidence of asymtomatic urinary tract infection in the antenatal period.

Primary brain abscess with Nocardia otitidiscaviarum in an intravenous drug abuser.

A primary brain abscess with Nocardia otitidiscaviarum in an intravenous drug abuser is reported. Nocardia brain abscess has been reported infrequently and normally only in immunocompromised patients. The lungs are the most common primary focus, but brain abscess may also occur following direct cutaneous inoculation. In this case the clinical presentation was first diagnosed as an astrocytoma. However, N. otitidiscaviarum was isolated from the lesion after emergency craniotomy. In contrast to five cases described previously the patient survived after surgical removal and antibiotic treatment w…

Brain abscess in solid organ transplant recipients receiving cyclosporine-based immunosuppression.

OBJECTIVE: To determine the incidence, clinical presentation, and outcome and confounding factors associated with the development of a brain abscess in solid organ transplant recipients. DESIGN: A 14-year retrospective survey. SETTING: A single, multiorgan, academic transplantation center. PATIENTS: A total of 2380 liver transplant recipients, 1650 kidney transplant recipients, and 598 heart, heart-lung, or lung transplant recipients of all ages (pediatric and adult) were included. All patients were given cyclosporine-based immunosuppression during this period. MAIN OUTCOME MEASURE: A brai…

Ten year review of brain abscess in Children's Hospital Bangkok, Thailand.

Brain abscesses are the most common form of intracranial suppurative process in children. In ten years, 54 cases of brain abscess in the Children's Hospital, Bangkok were diagnosed mostly on the basis of clinical features, roentgenogram, CT scanning and basic laboratory findings. The age distribution of brain abscess was from 3 months to 14 years. Evidence of signs of increased ICP was frequently present. Almost half of the patients had associated congenital heart disease particularly Tetralogy of Fallot. CT scanning was very useful in the advantage of rapid diagnosis, early management and sho…

[Solitary listerial abscess of the brain stem. Cure with antibiotic treatment].

A healthy 26-year-old man, without initially presenting fever, rapidly developed a focal right pontomedullary deficit associated with an aseptic lymphocytic meningitis. The diagnosis of Listeria infection was confirmed by blood cultures. CT and MRI demonstrated an abscess extending from the superior cerebellar peduncle to the lateral portion of the medulla. Immunological controls gave no indication of deficiency. With ampicillin therapy, started on the 5th day, clinical recovery was almost complete, but a soft palate right paresis persisted as the unique sequel. Antibiotic therapy was maintain

Listeria brain abscess associated with steroid therapy: successful non-surgical treatment.

A patient is reported who developed a brain abscess due to Listeria monocytogenes while on long-term steroid therapy and who was successfully treated medically without surgical intervention. The literature on listeria brain abscess is reviewed, and the importance of early recognition of this infection in any immunocompromised patient who develops focal cerebral dysfunction is emphasised.

Brain abscess due to Listeria monocytogenes: case report and literature review.

Listeria monocytogenes is an uncommon cause of brain abscess. Of a total of 14 cases of L. monocytogenes brain abscess (one described for the first time and 13 reported previously in the English-language literature), seven (50%) occurred in patients with leukemia and recipients of renal transplants; four (29%) of the cases occurred in previously healthy individuals. Common clinical findings were similar to those in brain abscess due to other causes and included fever (57%), headache (57%), and focal neurologic signs (64%). Distinctive, however, was the unusually high frequency of associated me…

Tuberculous brain abscess: report of a case with computed tomography correlation.

A 27 year-old Vietnamese male immigrant to Canada developed a hemispheric cerebellar abscess. The patient presented at the hospital with osteomyelitis of the 5th finger of the left hand. He complained of lassitude, weight loss, and early morning headache, nausea, and vomiting, and he developed a left facial weakness. A computed tomographic scan demonstrated the distinctive appearance of an abscess of the left cerebellar hemisphere. Aspiration of the abscess afforded immediate relief of obstructive hydrocephalus and provided pus from which Mycobacterium tuberculosis was grown, thus permitting s

Effectiveness of Surgical Excision in Preventing Recurrence of Multilocular Brain Abscess.

Background Despite advances in surgical treatment and antimicrobial therapy reducing mortality and recurrence rates of brain abscesses, many patients still face a challenging clinical course. To assess whether selecting an appropriate surgical approach influences patient outcomes, we retrospectively analyzed cases at our hospital, with a particular focus on abscess morphology. Methods We retrospectively analyzed 20 patients who underwent surgery for brain abscesses at our hospital between March 2005 and April 2022. Brain abscesses were classified as either simple or multilocular. Recurrence wa…

Brain abscess due to clostridium celerecrescens: first report and literature review.

BACKGROUND: Brain abscess caused by atypical pathogens presents significant diagnostic and therapeutic challenges. The unusual clinical presentations, coupled with incomplete or inaccurate patient histories, often result in misdiagnosis and inappropriate treatment. CASE PRESENTATION: We report a case of a retained intracranial bamboo foreign body resulting in a brain abscess. A female adolescent presented with a newly developed mass on the eyelid. Medical imaging identified a foreign body that had penetrated the frontal lobes via the transorbital route, leading to the formation of a brain abs…
