The cerebral hemispheres become atrophic with age. The sex of the individual may affect this process. There are few studies of the effects of age and sex on the brain stem and cerebellum. We used MRI morphometry to study changes in these structures in 152 normal subjects over 40 years of age. In the linear measurements, men showed significant age-associated atrophy in the tegmentum and pretectum of the midbrain and the base of the pons. In the early stage of their illness, neurological disorders were too subtle to be recognized as significant by the physicians or to be noted by the parents. We describe the relations between primary brain stem or cerebellar infarct or haemorrhage and secondary inferior olivary pseudo-hypertrophy (OPH). We identified 17 patients (43.6%) among 39 with brain stem or cerebellar vascular disease who had MRI follow-up more than 3 months after their ictus, with OPH. A characteristic appearance of occult cerebrovascular malformations (OCVMs) on high-field spin-echo magnetic resonance imaging (MRI) has been described previously. A patient with an intracerebral hematoma and associated edema in the high right hemisphere parasagittal convexity had several episodes of apnea at night and during the day when she was asleep. On computed tomography (CT) scan, the hematoma demonstrated no evidence of brainstem compression. Sagittal magnetic resonance image (MRI), revealed the hematoma and edema mass resulted in central herniation with axial deformity of the brainstem. This is believed to be the cause of the apneic episodes. Treatment with mannitol and steroids promptly relieved the symptom.

In the comatose patient the auditory brain stem response may be one of the few indices of CNS function available. Because barbiturates may be administered to control intracranial pressure and its sequelae, demonstration of the effect of barbiturates on the brain stem response is desirable. Several days after receiving a blow to the occipital region, a 5-year-old boy presented with acute symptoms and signs of a space-occupying-lesion in the posterior fossa. A CAT scan revealed a mass with increased density in the region of the fourth ventricle. A suboccipital craniectomy disclosed a subependymal hematoma bulging into the fourth ventricle. The hematoma was evacuated, and the child subsequently made a complete recovery. It is stressed that surgical intervention should be considered in all such cases and particularly in young patients who show progressive neurological deficits.

The authors report six cases of encephalitis localised in the brain stem. This disease occurred frequently during 1975, with the following symptoms and signs: fever, paralysis of several cranial nerves, temporary mental disturbance and C.S.F. changes. Several patients required tracheotomy and aritificial respiration. All patients recovered without any recurrences. Viral studies were negative. The possibility of a Listeria infection cannot be entirely eliminated.

An oscilloscopic analysis of several features of the stapedius muscle reflex was carried out in a group of normal subjects and in patients affected by dysfunction at the brain stem level due to a pathologic lesion or temporary reduction by small doses of barbiturates. Under these conditions, the increased latency and reduced amplitude of the reflex, but particularly the reduced velocity and, not infrequently, the step-like pattern of the tracings, are considered to be data of considerable value in disorders of the brain stem structues.
