Mechanistic aspects of inducible nitric oxide synthase-induced lung injury in burn trauma.

INTRODUCTION: Although the beneficial effects of inducible nitric oxide synthase (iNOS) inhibition in acute lung injury secondary to cutaneous burn and smoke inhalation were previously demonstrated, the mechanistic aspects are not completely understood. The objective of the present study is to describe the mechanism(s) underlying these favourable effects. We hypothesised that iNOS inhibition prevents formation of excessive reactive nitrogen species and attenuates the activation of poly(ADP) (poly(adenosine diphosphate)) ribose polymerase, thus mitigating the severity of acute lung injury in sh

Extensive burns in an elderly man found in a brush fire.

This past spring, a flight team was activated by a rural advanced life support ground service for a scene call approximately 30 minutes from their base for an 80-year-old man found lying in the middle of a brush fire. The ground paramedics did not know how the patient came to this position. During the initial call to the flight service, ground emergency medical services (EMS) reported that the patient had 100% burns that varied in severity from superficial to full thickness. The intercept was requested for emergent transport to a burn center, analgesia, and advanced airway management. On arriv

Significance of Contusion at the Vocal Cord and Perivocal Cord Area in Cases of Non-intubated Death.

Every finding during a neck autopsy may be essential for accurately diagnosing and explaining the mechanism of death. In this prospective study in the Department of Forensic Medicine and Toxicology of Jordan University Hospital, 17 out of 95 neck autopsies revealed contusions of the laryngeal mucosa in the vocal or perivocal area. These contusions were found to be associated with various causes of death, including mechanical asphyxia (such as throttling, ligature strangulation, hanging, smothering, choking, plastic bag asphyxia, gagging, and inhalation of blood) and other causes (such as atypi

Postoperative acute kidney injury and risk factors for major adverse kidney events within 30 days of burn surgery.

BACKGROUND: Burn injuries can lead to severe complications across multiple organ systems, with kidney impairment being particularly common and clinically significant. Major adverse kidney events within 30 days post-surgery (MAKE30)-including death, initiation of new renal replacement therapy, or prolonged renal dysfunction-are increasingly used in clinical research to assess mid-term renal outcomes. However, studies on the incidence and risk factors of MAKE30 in burn patients remain limited. METHODS: We included burn patients admitted to a single-center burn intensive care unit between 2012 a

Abdominal complications after severe burns.

BACKGROUND: Abdominal catastrophe in the severely burned patient without abdominal injury has been described. We perceived an alarming recent incidence of this complication in our burn center, both during acute resuscitation and later in the hospital course. We sought to define incidence, outcomes, and associated factors, such as excessive resuscitation volume and treatment issues. STUDY DESIGN: We examined all severely burned military and civilian patients with abdominal pathology between March 2003 and February 2008. Data included age, gender, total body surface area burn, inhalation injury

[Fungal identification in burn patients. A retrospective study and risk factors].

A retrospective analysis of fungal infections was carried out in a health-care burn center between 2011 and 2014 using the patients' medical records (n = 36). Patients ranged from 18 to 87 years of age, with 23 (63.9%) being women. The most prevalent widespread total body surface area affected (TBSA) was 30-50% (p = 0.03), and 71-100% in younger patients, mainly associated with femicide. Fourth degree burns were revealed in 69.4% of the patients while in 50%, inhalation injuries were observed to represent a higher mortality rate (p = 0.04). The Candida score was 3 in 77.7% of cases respectivel

Fungal infection after a tragedy: a report of three cases of candidosis in a fire accident.

Patients who suffer severe burns are at increased risk for local and systemic infections. The incidence of fungal infections has increased in recent years, and these infections represent a major issue in burn intensive care units. Herein, we report three cases of fungal infection due to Candida species occurring in patients undergoing supportive therapy and antibiotic treatment during their hospitalization. Two of these patients were infected with Candida parapsilosis, and one was infected with Candida albicans. The risk factors for these patients' Candida infections were multiple and prolonge

The acute effect of pulmonary burns on lung mechanics and gas exchange in the rabbit.

To determine the role of the thermal element in the production of lung injury uncomplicated by the inhalation of fumes and soot, 18 anaesthetized, paralysed and ventilated rabbits were subjected to a steam burn of the respiratory tract. Transpulmonary pressure (difference between airway and pleural pressure) was recorded under static conditions between functional residual capacity and 40 ml above this point and the values obtained were used to calculate lung compliance. Static compliance decreased progressively with time in each of the experimental animals, the maximum decrease occurring withi

Establishing a novel combustion generator system to simulate and study emissions from diverse applications.

Inhalation of emissions from combustion events such as military burn pits and wildland-urban interface fires result in exposures to complex aerosols that may cause adverse health effects. A surrogate combustion generator was created to study these events. A pellet stove was modified to burn diverse fuels comprised of materials found in burn pits (plastic, rubber, and wood). Jet fuel (JF) was added during combustion. The purpose of this project was to: 1) operate the generator over diverse parameters; and 2) characterize the resultant emissions. Pellet combustion (8 g/minute; range: 1-34) ± JF

Toxicological evaluation of potassium sorbate added to cigarette tobacco.

Potassium sorbate (PS) may be incorporated in blended cigarette tobacco either as a mold growth inhibitor in processed tobacco sheet material, or as a preservative in flavor systems or paper adhesives. To evaluate the effect of PS addition, neat material pyrolysis studies, smoke chemistry and biological activity studies (bacterial mutagenicity, cytotoxicity, in vivo micronucleus, and 90-day nose-only rat inhalation) with mainstream smoke, or mainstream smoke preparations from cigarettes containing various measured levels of PS (0%, 0.15%, 1.6%, and 3.7%) were performed. At simulated tobacco bu

Bronchopleural fistula secondary to smoke inhalation injury: a case report.

BACKGROUND: Bronchopleural fistula is a rare and severe condition characterized by an abnormal connection between the bronchial tree and pleural space, often leading to significant morbidity and mortality. Although typically resulting from surgical procedures, trauma, or pulmonary infections, bronchopleural fistula caused by smoke inhalation injury is exceptionally uncommon. Smoke inhalation can lead to extensive thermal and chemical damage to the respiratory tract, increasing the risk of infection and necrotic tissue changes that can complicate recovery. CASE PRESENTATION: We report a case o

Dynamic changes of lung lymph flow and the release of lysosomal enzyme from the lungs after severe steam inhalation injury in goats.

Chronic lung lymph fistulae were produced in six goats according to Winn's and Stothert's methods with our modification to define the pathophysiology of pulmonary oedema after severe steam inhalation injury. Arterial blood gas, lung lymph flow (QLym), lymph/plasma total protein concentration ratio (L/P), and beta-glucuronidase (beta-G) in plasma and lung lymph were monitored for 24 h post-injury. The pathological changes in the lung tissues were also determined at the end of the study. It was found that directly after injury, QLym increased steadily to a peak value at 6 h, followed by declinin
