Boric acid for recurrent vulvovaginal candidiasis: the clinical evidence.

BACKGROUND: Recurrent vulvovaginal candidiasis (VVC) remains a challenge to manage in clinical practice. Recent epidemiologic studies indicate that non-albicans Candida spp. are more resistant to conventional antifungal treatment with azoles and are considered as causative pathogens of vulvovaginal candidiasis. METHODS: We searched PubMed and Scopus for studies that reported clinical evidence on the intravaginal use of boric acid for vulvovaginal candidiasis. RESULTS: We identified 14 studies (2 randomized clinical trials [RCTs], 9 case series, and 4 case reports) as eligible for inclusion i…

Esophageal candidiasis--an adverse effect of inhaled corticosteroids therapy.

Over the last few decades, inhaled corticosteroids (ICs) became the cornerstone in the treatment of persistent asthma. Their use improved asthma control, decreased mortality and also minimized adverse reactions associated with systemic steroid. Esophageal candidiasis is a rare complication resulting from the use of ICs. Although, in recent years, as their prescriptions has increased, more cases have been reported, especially in Japan. Listed are 4 case reports regarding esophageal candidiasis in asthmatic patients associated with inhaled budesonide administration. In the cases reported herein,…

Oral health in Autoimmune Polyendocrinopathy Candidiasis Ectodermal Dystrophy (APECED).

BACKGROUND: APECED (Autoimmune Polyendocrinopathy Candidiasis Ectodermal Dystrophy) is a rare autosomal recessive disease characterised primarily by sequential immune-mediated destruction of endocrine tissues, chronic oral or mucocutaneous candidiasis and ectodermal disorders, including hypoplasia of dental enamel. AIM: This was to investigate the oral health and presence of enamel defects in a cohort of patients with APECED. METHODS: 16 patients with APECED (mean age of 13.9 years) were matched for age and gender with healthy controls. A comprehensive medical, dental and drug history was re…

Pituitary autoantibodies in autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED).

Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) is an autosomal recessive disease due to mutations in the AIRE (AutoImmune REgulator) gene. The role of pituitary autoimmunity in APECED is not known. We determined the prevalence of pituitary autoantibodies in a cohort of 67 Finnish patients with APECED from 217 serum samples collected over 26 years by one investigator. Overall, autoantibodies to the 49 kDa cytosolic autoantigen, human pituitary enolase were detected in 39 of the 67 patients (58%). On their first sample, 25 patients had autoantibodies compared to 5 of 68 …

Chronic mucocutaneous candidiasis and primary hypothyroidism in two families.

UNLABELLED: We describe the clinical and immunological features of two families with chronic mucocutaneous candidiasis (CMC) and primary hypothyroidism. Family A includes three siblings with both candidiasis and hypothyroidism and four individuals with hypothyroidism only. Family B includes four members with candidiasis, of whom one (a male child) also had hypothyroidism. All individuals affected with CMC had suffered from oral candidiasis and onychomycosis since infancy. Facial seborrhoic dermatitis, general folliculitis and scaling blepharitis were main manifestations. Hypothyroidism became …

Cerebral vasculitis in chronic mucocutaneous candidiasis: autopsy case report.

An autopsy case of chronic mucocutaneous candidiasis (CMCC) is reported here, in which cerebral vasculitis developed in the final stage. A 32-year-old man who had suffered from superficial candidial infection since his childhood was diagnosed as having CMCC. During the past 7 years the patient had developed various associated disorders including insulin-dependent diabetes mellitus (IDDM), common variable immunodeficiency (CVID), candidial esophagitis, multiple digestive tract ulcers and pyothorax. In 1998, at the age of 32, he developed convulsions that were accompanied by impairment of consci…

Congenital cutaneous candidiasis: clinical presentation, pathogenesis, and management guidelines.

We describe a term infant with congenital cutaneous candidiasis (CCC), and review all cases in the English literature that reported birth weight and outcome. Presence of an intrauterine foreign body was a predisposing factor for development of CCC and subsequent preterm birth. The most common presentation of CCC in neonates weighing >1000 g was a generalized eruption of erythematous macules, papules, and/or pustules that sometimes evolved to include vesicles and bullae. Extremely low birth weight, premature neonates weighing <1000 g most often presented with a widespread desquamating and/or er…

[Neonatal candidiasis and liposomal amphotericin B treatment: our experience].

OBJECTIVE: Nosocomial Candidiasis in low birth weight (LBW) infants have increased. Toxic side effects limit the use of conventional Amphotericin B for treatment of fungal infections. The liposomal forms have lowered this risk considerably, even at higher doses. Our aim was to evaluate treatment response to liposomal Amphotericin B in neonates with Candidiasis. PATIENTS AND METHODS: Fifteen neonates diagnosed both clinically and biologically of Candidiasis infection and who were treated with liposomal Amphotericin B from June 1994 through July 1997 were included. Duration of treatment, when c…

Neutrophil phagocytosis in AIDS patients with azole resistant candidiasis.

OBJECTIVE AND SUBJECTS: This study aimed to evaluate phagocytosis of C. albicans by neutrophils in 10 AIDS patients and 50 control subjects. Five of the AIDS patients were colonised with azole-resistant C. albicans isolates and five with azole-sensitive isolates. RESULTS AND CONCLUSIONS: Percentage phagocytosis was within normal limits for seven of the 10 AIDS patients and was reduced in the remaining three patients. Phagocytosis was unaffected by the carriage of azole resistant C. albicans, and the patients' own strains were phagocytosed as readily as a standard strain of C. albicans. This s

Successful peritransplant therapy in children with active hepatosplenic candidiasis.

Two pediatric leukemic patients with hepatosplenic candidiasis during multidrug antileukemic chemotherapy successfully underwent bone marrow transplantation (BMT) after aggressive antifungal chemotherapy employing fluconazole and amphotericin B with or without splenectomy. One patient received allogeneic marrow graft and the other received an autologous graft. One patient has been disease-free for more than 21 months after BMT without any recurrence of Candida infection. The other patient showed tentative reactivation of hepatic lesions just after BMT by CT scanning, but these lesions disappea…

[Symptomatic renal candidiasis with multiple mycetomas: a case report].

Candida albicans is the most prevalent and pathogenic fungal infection of the urinary tract. Although fungal urinary tract infection occur less frequently than bacterial infections, their incidence has increased during the last decades. Parenteral nutrition, corticosteroids, immunosuppressive drugs and prolonged antibiotic therapy are often predisposing factors. We report a case of renal pelvic fungus balls in a 6-month old female who developed candidal infection after surgical treatment for bilateral vesico-ureteric reflux; the renal candidiasis was treated successfully with fluconazole.

Invasive neonatal candidiasis: comparison of albicans and parapsilosis infection.

Severe infections caused by non-albicans Candida species are being increasingly reported among infants in neonatal intensive care units. To assess relative severity, mortality rates for C. albicans (CA) and C. parapsilosis (CP) infections in one neonatal intensive care unit from 1980 to 1990 were compared. Invasive candidiasis was defined as Candida recovery from a normally sterile body fluid or site with clinical signs of infection. Invasive candidiasis was diagnosed and systemic antifungal therapy initiated in 45 infants, 29 with CA and 16 with CP. No differences were found between CA and CP…

Experimental candidiasis associated with liver injury. Role of transferrin.

In an attempt to perform a further investigation on the proposal that an increasing susceptibility to Candida infection in liver injury may be related to unsaturated transferrin level (UIBC) and/or to a total amount of transferrin represented by TIBC, we conducted experimental candidiasis using mice with galactosamine-induced liver injury and investigated the effect of preadministration of transferrin prior to inoculation of Candida albicans. Final mortality was 10% in the mice without liver injury and without transferrin (Group 1) and ones with liver injury and with transferrin (Group 3). By …

Ectodermal defects and chronic mucocutaneous candidiasis in idiopathic hypoparathyroidism.

Idiopathic hypoparathyroidism is a rare disorder produced by parathyroid hormone deficiency of unknown cause. It is often associated with other endocrine abnormalities. Patients with idiopathic hypoparathyroidism frequently develop ectodermal disease, including dry, rough skin: coarse, brittle hair; and lusterless, distally split nails. All of these complaints are relatively common in a dermatologic practice. Chronic mucocutaneous candidiasis is also a manifestation of idiopathic hypoparathyroidism. A patient is presented with a prominent ectodermal dysplasia and a chronic mucocutaneous candid…

Oral candidiasis--its pleomorphic clinical manifestations, diagnosis and treatment.

Up to 60% of the population carry Candida albicans as part of the oral flora without having evidence of candidiasis. The pleomorphic clinical manifestations of oral candidiasis viz. thrush, denture stomatitis, angular cheilitis, median rhomboid glossitis, speckled leukoplakia, and chronic mucocutaneous candidiasis and its variants are briefly discussed. Current diagnostic techniques of oral candidiasis (OC) are reviewed. A simple and quick method of helping the clinician in the diagnosis of OC by taking a direct smear of the lesion is emphasized. OC is a 'disease of the diseased'. As a routine…

Chronic mucocutaneous candidiasis and keratitis associated with malignant thymoma.

Chronic mucocutaneous candidiasis is a rare skin condition that may be associated with immunologic defects, failure of the endocrine system, or a neoplasm, especially thymoma. Ocular findings are rare. Our patients, a middle-aged man, had chronic mucocutaneous candidiasis in conjunction with an aggressive malignant thymoma. He was initially seen with recurrent Candida keratitis and subsequently had Candida esophagitis and a polymicrobial lung abscess. The skin, nail, mucous membrane, and conjunctival involvement responded to oral ketoconazole therapy. This syndrome consists of at least five gr…
