Cardiac resynchronization therapy in a case of myotonic dystrophy (Steinert's disease) and dilated cardiomyopathy.

Steinert's disease (Dystrophia myotonica type 1) is an autosomal dominant neuromuscular disease characterized by myotonia, muscle weakness, frontal balding, cataracts, cardiac conduction abnormalities, especially long PR interval and wide QRS complex. Although subclinical mild myocardial dysfunction may be detected in this syndrome, overt myocardial dysfunction with heart failure called as "myotonic heart disease" is not frequent. Cardiac resynchronization therapy is an effective treatment modality to improve morbidity and mortality in patients with intraventricular conduction delay and conges…

[Cardiac resynchronization therapy in patients with permanent atrial fibrillation--with or without atrial lead?].

A 59-year-old patient received a biventricular defibrillator for cardiac resynchronization therapy (CRT) due to severe heart failure and a left bundle branch block. He had suffered from mitral stenosis and had received valve replacement 16 years earlier. Because he had permanent atrial fibrillation since that time, no atrial lead was implanted. His symptoms improved with CRT until he received adequate shock therapy for a rapid ventricular tachyarrhythmia. After that his symptoms deteriorated again severely. The ECG recorded during an unscheduled follow-up visit 1 week after the shock explains …

Cardiac resynchronization therapy in congestive heart failure: The state of the art and future perspectives.

Congestive heart failure (CHF) is one of the leading causes of morbidity and mortality worldwide. Up to 50% of CHF patients have intraventricular conduction disturbances such as left bundle branch block or nonspecific wide QRS complex on the body surface ECG. Intraventricular conduction delays cause dyssynchrony of the ventricles which in turn leads to regional movement abnormalities and worsening of cardiac function. Recent clinical trials have indicated that cardiac resynchronization therapy or biventricular pacing in CHF patients with left bundle branch block or wide QRS complex improves ca…

Complete Left Bundle Branch Block Resolution After Cardiac Resynchronization Therapy With Serial Phonocardiographic Changes.

BACKGROUND: Cardiac resynchronization therapy (CRT) induces structural and electrical reverse remodeling in patients with heart failure. However, resolution of complete left bundle branch block (CLBBB) following CRT has rarely been described, particularly with serial phonocardiographic assessment. CASE SUMMARY: A 67-year-old man with heart failure associated with atrial fibrillation and CLBBB underwent CRT pacemaker implantation following pulsed-field ablation. Initially, the optimal interventricular (VV) delay required left ventricular pre-excitation, but shifted toward right ventricular pre…

Non-Invasive Methods for Identifying Electrical Responders to Conduction System Pacing Cardiac Resynchronization Therapy.

BACKGROUND: Conduction system pacing (CSP) is an emerging alternative modality of cardiac resynchronization therapy (CRT). However, not all patients obtain electrical resynchronization with CSP, there is a clinical need for reliable non-invasive predictors of response. We aimed to assess the ability of several non-invasive modalities to predict electrical resynchronization with CSP-CRT. METHODS: Patients with standard heart failure CRT indications (excluding right-bundle branch block) underwent CSP-CRT using His-bundle and left-bundle branch pacing. Electrical resynchronization was defined as…

Effects of adaptive left bundle branch-optimized cardiac resynchronization therapy: a single centre experience.

BACKGROUND: Adaptive cardiac resynchronization therapy (aCRT) is associated with improved clinical outcomes. Left bundle branch area pacing (LBBAP) has shown encouraging results as an alternative option for aCRT. A technique that can be accomplished effectively using LBBAP combined with coronary venous pacing (LOT-aCRT). We aimed to assess the feasibility and outcomes of LOT-aCRT. METHODS: LOT-aCRT, capable of providing two pacing modes, LBBAP alone or LBBAP combined with LV pacing, was attempted in patients with CRT indications. Patients were divided into two groups: those with LBBAP and LV …

Shortening of time-to-peak left ventricular pressure rise (Td) in cardiac resynchronization therapy.

AIMS: We tested the hypothesis that shortening of time-to-peak left ventricular pressure rise (Td) reflect resynchronization in an animal model and that Td measured in patients will be helpful to identify long-term volumetric responders [end-systolic volume (ESV) decrease >15%] in cardiac resynchronization therapy (CRT). METHODS: Td was analysed in an animal study (n = 12) of left bundle-branch block (LBBB) with extensive instrumentation to detect left ventricular myocardial deformation, electrical activation, and pressures during pacing. The sum of electrical delays from the onset of pacing …

Right ventricular function assessed by cardiac magnetic resonance predicts the response to resynchronization therapy.

BACKGROUND AND AIM: Cardiac resynchronization therapy (CRT) reduces mortality and morbidity in chronic heart failure symptomatic patients with broad QRS who are already undergoing optimal medical treatment. However, approximately one-third of implanted patients do not show any benefit from this treatment. Right ventricle (RV) dysfunction leads to a worse outcome in patients with heart failure, but its role in predicting the response to CRT has shown conflicting results. The purpose of our study was to investigate how the RV function, assessed by cardiac magnetic resonance (CMR), could influenc…
