[Adrenergic innervation of lymph nodes and the thoracic duct].

By means of incubation of slices in 2% solution of glyoxylic acid distribution of adrenergic fibers in the rabbit lymph nodes and in the thoracic lymphatic duct has been studied. Adrenergic fibers get into parenchyma of the lymph nodes via two ways. The first--the perivascular, when the nervous fibers make a plexus and get into the node along the blood vessels, the second--diffuse nervous fibers get together with trabecules in between the lymphoid nodules. The distribution density of the adrenergic fibers is not the same in different groups of the lymph nodes. In the lumbar nodes it is the hig

[Combined surgical and physical treatment in traumatic painful syndromes of the cervical spine].

Clinical observations suggest the need for changing therapeutic management to a more active one in cases of cervical spine injury with damage to the spinal cord and nerve roots or brachial plexus. In 248 patients with these injuries treated initially conservatively the incidence of cervicobrachial pain was analysed. Neuralgic pains were present in 31.5% of cases, causalgic pains in 2.4% and sympathalgic pains in 2%. Conservative treatment conducted in these patients (89 cases) during many months after trauma had no effect on return of mobility. Long-term application of physioterapy prevented o

Cervical flexion-induced myelopathy (Hirayama disease): A radiology case report.

Hirayama disease is a self-limiting cervical flexion myelopathy that typically affects adolescent and young adult males presenting with unilateral or asymmetric distal upper-limb weakness and atrophy. Early diagnosis is important because avoiding neck flexion with cervical collar immobilization may limit ongoing spinal cord injury and selected patients may require surgery. We report a 19-year-old male with 1 year of left-hand weakness involving the middle and ring fingers with progressive difficulty in daily activities. Neurologic examination revealed distal hand muscle wasting and weakness wi

Ultrasound-Guided Greater Auricular Nerve Block as Sole Anesthetic for Ear Surgery.

A greater auricular nerve (GAN) block was used as the sole anesthetic for facial surgery in an 80-year-old male patient with multiple comorb … vention of inadvertent vascular puncture or blockade of the phrenic nerve, brachial plexus, or deep cervical plexus. The increasing access to ultrasound technology for medical care providers outside the operating room makes this ultrasound guided block an increasingly viable alternative.

Monoparesis with complex regional pain syndrome-like symptoms due to brachial plexopathy caused by the varicella zoster virus: a case report.

Viral invasion of the motoneurons and the subsequent inflammation in the anterior horn cells by the varicella zoster virus results in a weakness in the area of the cutaneous eruption. The exact mechanism of zoster paresis is uncertain. The occurrence of symptoms resembling complex regional pain syndrome (CRPS) is common in subjects where the herpes zoster (HZ) outbreak affects an extremity, particularly if it is the distal extremity that is involved. We report the case of a 54-year-old man with monoparesis, hyperalgesia, allodynia, edema, and both color and skin-temperature changes in his left

[The differential diagnosis of lesions of the rotator cuff and of neurological disorders in the shoulder joint area].

Lesions of the rotating cuff is a most frequent cause of disorders in the shoulder joint. However, they are frequently misdiagnosed and their manifestations use to be considered as those of scapulohumeral periarthritis, cervical osteochondrosis, or even brachial plexus palsy. The paper deals with symptoms and signs of rotating cuff lesions that are fairly different from those of the conditions above cited. It is emphasized that the deltoid muscle palsy cannot result in a permanent loss or restriction of active elevation of the arm when the function of the rotating cuff remains undisturbed. 16

[Intramedullary topography of the motor neurons of the brachial plexus of the rat. Preliminary study].

The exact localisation of the motor pools of anterior horn providing the different nerves of the brachial plexus remains unclear. The data obtained have been furnished by different methods: Nissl methods furnish cytoarchitectonic data -- Golgi methods furnish dendro-architectonic data. The topographical data were obtained by the chromatolytic modifications of the moto-neurons after a peripheral nerve injury. The recently used method of the retrograde transport of HRP permits to get information about the exact localisation and the dendritic extent of the neurons of a motor pool related to a par

Contralateral brachial plexus neuropathy after arthroscopic shoulder surgery.

This case report presents a neurologic complication that occurred after shoulder arthroscopic surgery on the contralateral arm. This brachial plexus palsy has been documented as a reversible C7-T1 lesion, which was the consequence of the patient's unknown cervical rib and the applied lateral position during the operation. In this case, the recovery was almost complete within 3 months postoperatively; however, one should take care to prevent these complications with an appropriate positioning of patients with an anatomic variant.

Cervical nerve roots and the dural sheath: a histological study using human fetuses near term.

We have previously reported that the thoracolumbar posterior nerve root shows a tortuous epidural course, based on studies of human fetuses near term. For comparison with the cervical nerve, examinations were conducted using frontal, sagittal and horizontal sections of cervical vertebrae from 22 fetuses at 30-38 weeks of gestation. The cervical nerve root showed a short, straight and lateral course near the zygapophysial joint. Multiple rather than single bundles of the cervical posterior root seemed to account for the majority of sensory nerve fibers innervating the upper extremity. Fascicula

Microanatomical considerations for safe uncinate removal during anterior cervical discectomy and fusion: 10-year experience.

Cervical radiculopathy from uncovertebral joint (UVJ) hypertrophy and nerve root compression often occurs anterior and lateral within the cervical intervertebral foramen, presenting a challenge for complete decompression through anterior cervical approaches owing to the intimate association with the vertebral artery and associated venous plexus. Complete uncinatectomy during anterior cervical discectomy and fusion (ACDF) is a controversial topic, many surgeons relying on indirect nerve root decompression from restoration of disc space height. However, in cases of severe UVJ hypertrophy, indire

Transverse cervical and great auricular nerve distribution in the mandibular area: a study in human cadavers.

The angle of the jaw is innervated by the auricular branch of the superficial cervical plexus (SCP). Cervical cutaneous nerves of the CP carry the sensation from the antero-lateral cervical skin. It is clinically relevant to identify the cervical cutaneous nerve distribution and the nerve point using superficial landmarks but published studies describing the emerging patterns and cervical cutaneous nerve branch distributions in the mandible are rare. The overlap between the cervical and trigeminal and facial nerve distributions and anastomoses is highly variable. The objective of this study wa…

Spear tackler's spine. An entity precluding participation in tackle football and collision activities that expose the cervical spine to axial energy inputs.

We describe spear tackler's spine, a clinical entity that constitutes an absolute contraindication to participation in tackle football and other collision activities that expose the cervical spine to axial energy inputs. A subset of football players were identified who demonstrated: 1) developmental narrowing (stenosis) of the cervical canal; 2) persistent straightening or reversal of the normal cervical lordotic curve on erect lateral roentgenograms obtained in the neutral position; 3) concomitant preexisting posttraumatic roentgenographic abnormalities of the cervical spine; and 4) documenta

Brachial plexus injury and resting-state fMRI: Need for consensus.

OBJECTIVE: The purpose of the study is to conduct the systematic review of literature available on resting-state functional MRI (fMRI) and brachial plexus injury. METHODS: We reviewed all the literature that are available on PubMed; keywords used were resting state, brachial plexus injury, and functional imaging. The reference papers listed were also reviewed. The research items were restricted to publications in English. Some papers have also incorporated studies such as task-based fMRI and transcranial magnetic stimulation (TMS), but only resting-state studies were included for this review.

Superficial siderosis of the central nervous system.

Superficial siderosis of the central nervous system (CNS) is a rare entity characterized by the deposition of hemosiderin in the leptomeninges. In most cases it is caused by chronic and recurrent bleeding into the subarachnoid space as a subclinical form and for long periods of time. The cases described in the literature are associated with tumors, aneurysms, arteriovenous malformations, changes in post-surgical, traumatic cervical and brachial plexus injuries. However, the cause of bleeding is unclear in 40-50% of cases. This report describes the case of a 38-year-old man with a history of tr

Neurolymphomatosis in follicular lymphoma: an autopsy case report.

Neurolymphomatosis is a neurological manifestation of lymphoma that involves the cranial or spinal peripheral nerves, nerve roots, and plexus with direct invasion of neoplastic cells. Neurolymphomatosis is rare among patients with low-grade lymphoma. We report an autopsied case of neurolymphomatosis that arose from follicular lymphoma. A 49-year-old woman who presented with pain of her neck and shoulder and numbness of her chin. Computed tomography revealed enlarged lymph nodes in her whole body, and biopsy from the axillary lymph node revealed grade 2 follicular lymphoma. Although the patient

Clonidine as an adjuvant to local anesthetics for peripheral nerve and plexus blocks: a meta-analysis of randomized trials.

The effect of adding clonidine to local anesthetics for nerve or plexus blocks remains unclear. The authors searched for randomized placebo-controlled trials testing the impact of adding clonidine to local anesthetics for peripheral single-injection nerve or plexus blocks in adults undergoing any surgery (except eye) without general anesthesia. Twenty trials (1,054 patients, 573 received clonidine) published 1992-2006 tested plexus (14 brachial, 1 cervical) and nerve blocks (2 sciatic/femoral, 1 midhumeral, 1 ilioinguinal/iliohypogastric, 1 ankle). Clonidine doses ranged from 30 to 300 microg;

Stretch-induced spinal accessory nerve palsy.

Left spinal accessory nerve palsy occurred in a young man when he quickly turned his head to the right while his shoulders were pulled down by heavy hand-held objects. Electrophysiologic studies demonstrated partial axonotmesis of the spinal accessory nerve branches innervating the sternocleidomastoid and upper and middle trapezius and complete axonotmesis of spinal accessory branches to the lower trapezius. There was a separate, although functionally minor, cervical plexus innervation of the lower trapezius.

Evaluation of Avulsion-Induced Neuropathology in Rat Spinal Cords with 18F-FDG Micro-PET/CT.

Brachial plexus root avulsion (BPRA) leads to dramatic motoneuron death and glial reactions in the corresponding spinal segments at the late stage of injury. To protect spinal motoneurons, assessment of the affected spinal segments should be done at an earlier stage of the injury. In this study, we employed 18F-FDG small-animal PET/CT to assess the severity of BPRA-induced cervical spinal cord injuries. Adult Sprague-Dawley rats were randomly treated and divided into three groups: Av+NS (brachial plexus root avulsion (Av) treated with normal saline), Av+GM1 (treated with monosialoganglioside),

Iatrogenic neurological injury after radiofrequency ablation and epidural steroid injections: illustrative cases.

BACKGROUND: Neck pain is often chronic and disabling. Cervical facet joint injections and epidural steroid injections are frequently used to manage chronic neck pain and cervicogenic headaches. While minimal side effects are commonly associated with these treatments, severe complications are exceedingly rare. OBSERVATIONS: The authors report 4 cases of iatrogenic neurological injury after radiofrequency ablation (RFA) and epidural steroid injections. One patient experienced left shoulder, scapular, and arm pain with left arm and hand weakness that developed immediately after RFA for chronic n
