Erosion of the Capital City Advantage in Child Survival and Reproductive, Maternal, Newborn, and Child Health Intervention Coverage in Sub-Saharan Africa.

The place of residence is a major determinant of RMNCH outcomes, with rural areas often lagging in sub-Saharan Africa. This long-held pattern may be changing given differential progress across areas and increasing urbanization. We assessed inequalities in child mortality and RMNCH coverage across capital cities and other urban and rural areas. We analyzed mortality data from 163 DHS and MICS in 39 countries with the most recent survey conducted between 1990 and 2020 and RMNCH coverage data from 39 countries. We assessed inequality trends in neonatal and under-five mortality and in RMNCH covera

Improving access to child health services at the community level in Zambia: a country case study on progress in child survival, 2000-2013.

Reductions in under-five mortality in Africa have not been sufficient to meet the Millennium Development Goal #4 (MDG#4) of reducing under-f … l health strategic plans and efforts to ensure universal access to effective maternal, neonatal and child health (MNCH) interventions, creating an environment that has promoted child health. Zambia has also focused on bringing health services as close to the family as possible through specific community health strategies. This includes actively involving commu …

The law of diminishing marginal returns in health insurance: evidence on child health outcomes from Pakistan's Sehat Sahulat program.

BACKGROUND: Child malnutrition remains a serious public health challenge in Pakistan. The national health insurance initiative, the Sehat Sahulat Program (SSP), aims to improve access to health care for low-income families. This study examines whether the effectiveness of SSP follows the Law of Diminishing Marginal Returns (LDMR), whereby the marginal health benefits of insurance are greatest for the poorest households and decline with increasing wealth. METHODS: We analyzed data on 4,499 children under 5 years of age in the Pakistan Demographic and Health Survey 2017-18. To address endogenei

Maternal oral health behaviors and status during pregnancy and child oral health: a study from a Brazilian birth Cohort.

OBJECTIVES: To evaluate whether dental caries and oral hygiene patterns during pregnancy are associated with the presence of caries and oral hygiene patterns in children. METHODS: A longitudinal study using data from the 2015 Pelotas Birth Cohort study. The exposures were dental caries and oral hygiene practices of the mothers, which were assessed through oral examination and interview during pregnancy. The outcomes were caries and oral hygiene patterns of children at 4 years old. Maternal age, education, skin color, family income, and use of dental services by children were considered potent

Comparative child mental health and academic outcomes after primary school teachers deliver task-shifted mental health care in India.

Teachers are promising lay counselors but have limited time for traditional therapy. To viably leverage teachers to deliver care, we developed Teachers Leading the Frontlines (Tealeaf), where teachers deliver a novel therapy, "education as mental health therapy" (Ed-MH); measures are an extension of classroom management, not an added task. We investigated whether Tealeaf teachers improved child mental health symptoms and academic achievement versus teachers delivering an enhanced usual care (EUC) comparator, Responding to Students' Emotions through Education (RE-SEED), a truncated version of T

The impact of child maltreatment on mental health outcome improvements among children and youth accessing community mental health care.

BACKGROUND: Although children with histories of maltreatment are more likely to engage with mental healthcare than non-maltreated children, few studies have examined the impact of maltreatment on children's mental health outcome improvement over time. OBJECTIVE: The current study addresses this gap in the present literature by exploring the ways that histories of physical, sexual, and emotional abuse and exposure to domestic violence can influence children's improvement on mental health outcomes commonly associated with trauma. PARTICIPANTS AND SETTING: De-identified routine care data from 5

Working with State Health Departments on Emerging Issues in Maternal and Child Health: Immediate Postpartum Long-Acting Reversible Contraceptives.

BACKGROUND: Immediate postpartum long-acting reversible contraceptives (LARC) are highly effective in preventing unintended pregnancy. State health departments are in the process of implementing a systems change approach to better apply policies supporting the use of immediate postpartum LARC. METHODS: Beginning in 2014, a group of national organizations, federal agencies, and six states have convened a LARC Learning Community to share strategies and best practices in immediate postpartum LARC policy development and implementation. Community activities consist of in-person meetings and a webi

Health-seeking in fragile and conflict-affected settings: how armed violent conflict shapes maternal and child health-seeking behaviours.

BACKGROUND: Maternal and child mortality rates remain unacceptably high in fragile and conflict-affected settings, where insecurity, displacement, and systemic disruption hinder access to healthcare. Nigeria is one of the countries most severely affected, and continues to experience widespread maternal and neonatal deaths despite global and national efforts to achieve SDG 3.1. This study explored stakeholders’ perspectives on how armed conflicts shapes maternal and child health-seeking behaviours and outcomes in northeastern Nigeria. METHODS: A qualitative approach was employed, involving Key …

Dental public health in action: 'First Dental Steps' - An oral health improvement intervention embedded in the healthy child programme.

Dental caries remains a significant public health problem with almost a quarter of children in England having experience of caries by the time they are five. This implementation focused paper presents an overview of the local dental public health team working collaboratively with health visiting teams in the South West of England to deliver a multistranded oral health improvement programme embedded into the Healthy Child Programme. At aged nine to 12 months children receive a mandated health check conducted by health visitors which provides an opportunity to deliver an oral health intervention

Associations between maternal mental health, child dental anxiety, and oral health of 6- to 12-year-olds in Nigeria.

Maternal mental health affects their children's oral health. This study assessed the associations between maternal mental health and dental anxiety level, dental caries experience, oral hygiene, and gingival status among 6- to 12-year-old children in Nigeria. This was a cross-sectional study that recruited mother-child dyad participants through a household survey conducted in Ile-Ife, Nigeria. Data collected included the independent (maternal mental health risk, depressive symptoms, and child's dental anxiety), and dependent (caries experience, oral hygiene status, and gingival health status)

Clinician-rated mental health in outpatient child and adolescent mental health services: associations with parent, teacher and adolescent ratings.

BACKGROUND: Clinician-rated measures are used extensively in child and adolescent mental health services (CAMHS). The Health of the Nation Outcome Scales for Children and Adolescents (HoNOSCA) is a short clinician-rated measure developed for ordinary clinical practice, with increasing use internationally. Several studies have investigated its psychometric properties, but there are few data on its correspondence with other methods, rated by other informants. We compared the HoNOSCA with the well-established Achenbach System of Empirically Based Assessment (ASEBA) questionnaires: the Child Behav

Financing of pediatric home health care. Committee on Child Health Financing, Section on Home Care, American Academy of Pediatrics.

In certain situations, home health care has been shown to be a cost-effective alternative to inpatient hospital care. National health expenditures reveal that pediatric home health costs totaled $5.3 billion in 2000. Medicaid is the major payer for pediatric home health care (77%), followed by other public sources (22%). Private health insurance and families each paid less than 1% of pediatric home health expenses. The most important factors affecting access to home health care are the inadequate supply of clinicians and ancillary personnel, shortages of home health nurses with pediatric exper

Health status of Vietnam veterans. III. Reproductive outcomes and child health. The Centers for Disease Control Vietnam Experience Study.

The Vietnam Experience Study was a multidimensional assessment of the health of Vietnam veterans. From a random sample of enlisted men who entered the US Army from 1965 through 1971, 7924 Vietnam and 7364 non-Vietnam veterans participated in a telephone interview; a random subsample of 2490 Vietnam and 1972 non-Vietnam veterans also underwent a comprehensive medical examination. During the telephone interview, Vietnam veterans reported more adverse reproductive and child health outcomes than did non-Vietnam veterans. However, children of Vietnam veterans were not more likely to have birth defe…

The use of case advocates to manage child survivors of sexual violence in public health facilities in Kenya: A qualitative study exploring the perceptions of child survivors, care givers and health care providers.

BACKGROUND: The uptake of and retention in health services by child survivors of violence (CSV) is a growing challenge, especially in sub-Saharan Africa. While lay health workers have been used in several settings to improve access to health services, there is limited literature on the use of such workers to support services for CSV. OBJECTIVE: To explore the acceptability of using trained lay health workers (case advocates) to provide basic information and escort CSV to various referral points within two public health facilities in Kenya. PARTICIPANTS: The following participants were enroll

The association between paternal PTSD and child mental health: a systematic literature review.

Fathers play a crucial role in their children's psychosocial development, positively influencing emotional, cognitive, and social competencies. Paternal Post-Traumatic Stress Disorder (PTSD) can disrupt this dynamic, potentially compromising the psychological well-being and developmental outcomes of their offspring. This systematic review aims to investigate the relationship between paternal PTSD and the prevalence of psychopathology in children and adolescents, providing a comprehensive understanding of how trauma experienced by fathers may influence their children's mental health. Household livelihood vulnerability and child health in southwestern coastal Bangladesh: determinants and implications.

BACKGROUND: Household livelihood vulnerability (HLV) remains a critical yet underexplored determinant of child health in climate-sensitive coastal regions of Bangladesh, where recurrent climate hazards disrupt livelihoods, reduce income stability, intensify food insecurity, and limit access to essential health services. Therefore, this study examines the extent and determinants of HLV and its implications for child health in southwestern coastal Bangladesh. METHODS: Following the survey method, this community-based cross-sectional study was carried out in six villages of Dacope upazila in Khu…

Justifying and Classifying Adults' Roles in Child Health State Valuation: A Novel Taxonomy.

For cost-utility analyses for children, health-related quality of life (HRQOL) is commonly measured using paediatric instruments such as EQ-5D-Y, yet associated value sets are typically derived from adult preferences. This practice raises a fundamental normative question: whose preferences, and thus whose values, should count when valuing child HRQOL? Recent academic critique has focused on whether children have the fundamental right and ability to provide preferences and hence should be involved in valuing child HRQOL, yet the default position, that preferences of adult members of the general
