A Multisource Approach to Assessing Child Maltreatment From Records, Caregivers, and Children.

Practitioners and researchers alike face the challenge that different sources report inconsistent information regarding child maltreatment. The present study capitalizes on concordance and discordance between different sources and probes applicability of a multisource approach to data from three perspectives on maltreatment-Child Protection Services (CPS) records, caregivers, and children. The sample comprised 686 participants in early childhood (3- to 8-year-olds; n = 275) or late childhood/adolescence (9- to 16-year-olds; n = 411), 161 from two CPS sites and 525 from the community oversample

The incidence of fractures in children under the age of 24 months--in relation to non-accidental injury.

INTRODUCTION: Fractures in children are the second most common presentation of child abuse. In younger children, especially in those less than 18 months, fractures should be considered suspicious of a non-accidental injury (NAI). Risk factors associated for abuse are: age younger than 12 months, non-ambulatory status, delayed presentation, unknown or inconsistent history of mechanism of injury, and presence of any other injuries. Our objective was to identify the incidence of fractures in children below the age of 24 months who presented to our institution's Emergency Department (ED), and iden

Correlates of disrupted behaviors in high-risk and maltreating parents: A comparison of the AMBIANCE-Brief and full versions.

No study has yet examined the convergence between the AMBIANCE-Brief's assessment of disrupted behavior and the full AMBIANCE version and its associations with child attachment and parental sensitivity among parents reported for maltreatment. Based on a sample of 71 parents and their young children receiving child protection or community services in Canada due to maltreatment or elevated risk for maltreatment, this study showed a moderate convergence between scores of the brief and full versions of the AMBIANCE, but the correspondence between the disrupted/non-disrupted classifications was wea

We Are Family: Specifying the Unique Contribution of Abuse and Neglect of Siblings on the Prevalence, Severity, and Chronicity of Maltreatment in the Household.

Prior research typically focuses on a single child ("index" victim) when measuring child abuse and neglect (CAN) using Child Protective Services (CPS) records. However, excluding siblings has the potential to underestimate estimates of the prevalence, severity, and chronicity of CAN in the household, which includes all children in the family. CPS maltreatment records were searched in 2005 for 366 "index" victims who were surveyed for 5 consecutive years (from 1998 to 2002) for the Mobile Youth Survey (MYS) as well as other siblings in the household. The estimate of prevalence of CAN in the hou…

The whole picture: Child maltreatment experiences of youths who were physically abused.

The purpose of the current study was to describe the maltreatment experiences of a sample of urban youths identified as physically abused using the Maltreatment Case Record Abstraction Instrument (MCRAI). The sample (n=303) of 9-12 year old youths was recruited from active child protective services (CPS) cases in 2002-2005, and five years of child protective service records were reviewed. The demographic and maltreatment experiences of MCRAI-identified youths with physical abuse were compared to maltreated youths who were not physically abused and youths who were identified as physically abuse…

The Effects of Father-Perpetration of Maltreatment on Adolescent Health Risk.

Maltreatment perpetrated by fathers may entail distinct characteristics and threats, and therefore differing effects from maltreatment perpetrated by mothers alone. This study examines the extent to which father perpetration of maltreatment is associated with variability in subsequent adolescent health outcomes relative to mother-alone maltreatment. A sample of youth (N = 377) with recently completed Child Protective Services investigations concerning reports of maltreatment attributed to fathers and/or mothers was drawn from the second National Survey of Child and Adolescent Well-being. Youth…

Risk of fatal injury in young children following abuse allegations: evidence from a prospective, population-based study.

OBJECTIVES: We examined variations in children's risk of an unintentional or intentional fatal injury following an allegation of physical abuse, neglect, or other maltreatment. METHODS: We linked records of 514 232 children born in California from 1999 to 2006 and referred to child protective services for maltreatment to vital birth and death data. We used multivariable Cox regression models to estimate variations in risk of fatal injury before age 5 years and modeled maltreatment allegations as time-varying covariates. RESULTS: Children with a previous allegation of physical abuse sustained…

Trajectories of maltreatment re-reports from ages 4 to 12:: evidence for persistent risk after early exposure.

This study identified trajectories of maltreatment re-reports between ages 4 and 12 for children first referred to Child Protective Services (CPS) for maltreatment prior to age 4 and either removed from the home or assessed by a CPS intake worker as moderately or highly likely to be abused/neglected in the future, absent intervention. Participants (n = 501) were children from the Southwest and Northwest sites of the Consortium for Longitudinal Studies of Child Abuse and Neglect (LONGSCAN). During the 8-year follow-up period, 67% of children were re-reported. Growth mixture modeling identified …

Markers for occult liver injury in cases of physical abuse in children.

Children suffering physical abuse are at risk for abdominal trauma. When the abdominal trauma is not evident clinically and remains occult, the child's condition can rapidly deteriorate before the diagnosis is made, and delay may occur in the activation of child protective services. The use of markers for abdominal trauma in the evaluation of cases of child physical abuse was examined. Children younger than 12 years presenting to the Emergency Department at the Children's Hospital of Buffalo from August 1989 through April 1990 for the evaluation of suspected physical abuse were prospectively e…

The Hypocritical Oath? Unintended Consequences of Prenatal Substance Use Policies and Considerations for Health Care Providers.

Prenatal substance use (PSU) is a serious perinatal health issue in the United States with consequential health effects. To address this issue and protect children from the detrimental effects of substance exposure during pregnancy, the US government amended the Child Abuse Prevention and Treatment Act to provide funding to states with protocol to notify child protective services of PSU cases and develop treatment plans for affected families. Although well-intentioned, this statute resulted in diverse inter- and intrastate interpretations and implementation of PSU regulations nationwide, ultim…

Children's Exposure to Violence: The Underlying Effect of Posttraumatic Stress Symptoms on Behavior Problems.

In this study we investigated whether witnessing violence and violence victimization were associated with children's internalizing and externalizing behavior problems and examined the mediating role of posttraumatic stress (PTS) symptoms in these relationships. Secondary data analysis was conducted using 3 waves of data from the National Survey of Child and Adolescent Well-Being. Path analyses were conducted to test direct and indirect effects of violence exposure on behavior problems, using 2,064 children (ages 8-15 years) reported to Child Protective Services for maltreatment. Being a victim…

The influence of differential response and other factors on parent perceptions of child protection involvement.

As Child Protective Services (CPS) agencies examine how to produce better outcomes with the families they serve, child welfare literature has increasingly focused on the perspectives, emotional responses, and engagement of CPS-involved parents or other primary caregivers. Despite this encouraging trend, the construct of engagement is ill-defined and our understanding of precursors to and factors affecting parent engagement is limited. This article extends the literature by presenting a conceptual framework for examining engagement and associating the identified constructs with parent outcomes.…

Worker Perceptions of the Multi-Agency Investigation & Support Team (MIST): A Process Evaluation of a Cross-Agency Response to Severe Child Abuse.

The Multi-agency Investigation & Support Team (MIST) was a new approach to abuse investigations that aimed to minimize the distress and uncertainty experienced by children and non-abusive caregivers in dealing with the many agencies typically involved in a case post-disclosure, while also attempting to improve the accessibility of supportive and therapeutic services. As part of a broader evaluation, this study examined worker perceptions early in the implementation of this new approach. Thirty-three (33) interviews were conducted with workers affected by this new pilot. The interviews identifi
