The incidence, histologic distribution, and clinical manifestations of ovarian tumors in the pediatric population are distinct from those in adults. Although ovarian neoplasms in childhood and adolescence are rare, the diagnosis should be considered in young girls with abdominal pain and a palpable mass. Differential diagnosis in children and adolescents with ovarian tumors should be conducted on the basis of unique clinical manifestations, elevated serum tumor marker levels, and distinctive imaging findings. A retrospective review was done on all high volume choriocarcinomas and other germ cell tumors of men with serum beta-human chorionic gonadotropin (beta-HCG) levels greater than 50,000 mIU/ml to determine the incidence and characteristics of hyperthyroidism in this setting. Nineteen patients were identified with high beta-HCG levels, but because 2 did not have thyroid function tests performed, the cases of only 17 patients were evaluable. Ectopic-hCG production occurs in approximately 30% of urothelial cancers. However, the clinical features which might be expected to result from elevated hCG levels are rare. We now show that in six of seven metastatic and 20 of 21 localized (Ta-T4) positive hCG bladder cancer patients, only the free beta-subunit of hCG and its respective urinary breakdown product, beta-core, could be detected in clinical samples. Since only intact hCG is biologically active, this explains the discrepancy between biochemical and clinical findings.

The sonographic findings of the pelvic lesions in persistent trophoblastic disease were evaluated prospectively in 57 patients. The appearance ranged from minimal lesions in the myometrium to complex lesions extending into the parametrium. A grading system was devised based upon the extent of the disease in the pelvis. Semen quality and serum testosterone, LH, and FSH levels were studied in 54 patients with a unilateral nonseminomatous tumor of the testis--14 before and 40 after orchiectomy. Semen analyses before and after orchiectomy gave essentially no different results: a poor semen quality was found in most men. Forty-two women using intrauterine contraceptive devices volunteered in a study in which blood samples were drawn in late luteal phase 1-3 times during 73 menstrual cycles. In all 129 blood samples were collected. Pregnancy-specific beta 1-glycoprotein (SP1) and human chorionic gonadotropin (beta-hC) were measured by radioimmunoassays. METHODS: Serum beta-hCG levels were monitored after therapy and correlated to the type of management, gestational age, and initial serum beta-hCG levels in 119 patients with ectopic pregnancies detected at 5-7 postmenstrual weeks.
