Cerebral blood flow autoregulation after experimental subarachnoid hemorrhage during hyperventilation in rats.

The purpose of the current study was to investigate whether impaired cerebral blood flow (CBF) autoregulation after experimental subarachnoid hemorrhage (SAH) could be restored by hyperventilation. SAH was induced in rats by injection of autologous blood into the cisterna magna. CBF was measured using intracarotid injection of 133Xe. Autoregulation was studied in four groups of animals: normoventilated controls, hyperventilated controls, normoventilated animals with SAH, and hyperventilated SAH animals. PaCO2 in the normocapnic groups was 38-42 mm Hg and 25-30 mm Hg in the hypocapnic groups. C…

[A case of filamentous fungal infection showing tumor-like expansion in the cisterns].

A 67-year-old woman was admitted for headache. The initial MRI showed a gadolinium-enhanced lesion in the prepontine area. Initial and repeated CSF examinations were negative for the fungal infection. Since the enhanced lesion expanded in the cisterns, and showed tumor-like appearance, brain biopsy was performed at 3 months from her first admission. Histological studies revealed filamentous fungal infection probably caused by the pseudallescheria boydii. Intrathecal miconazole injection through the Ommaya reservoir successfully ameliorated patient's symptoms and the MRI findings. Primary ciste

A case of a Dandy-Walker variant: the importance of a multidisciplinary team approach using complementary techniques to obtain accurate diagnostic information.

Dandy-Walker malformation is a rare abnormality of the central nervous system (CNS) with a reported incidence of 1 in 25,00-35,00 live births and a slight female predominance. It accounts for 1-4% of cases of antenatally detected hydrocephalus. Dandy Walker variant forms part of the spectrum of Dandy Walker malformation. It is characterized by partial agenesis of the vermis, resulting in communication between the fourth ventricle and the cisterna magna. Dandy Walker malformation demonstrates an enlarged posterior fossa, with high insertion of the torcula herophili, as well as other features. I…

Intracisternal sodium nitroprusside fails to prevent vasospasm in nonhuman primates.

OBJECTIVE: Hemoglobin contributes to vasospasm after subarachnoid hemorrhage. One mechanism may involve binding of nitric oxide, destruction of nitric oxide, or both. Support for this mechanism would be evidence that nitric oxide donors prevent vasospasm. This study attempted to provide such evidence. METHODS: A randomized, blinded study was conducted in which 13 monkeys underwent cerebral angiography and creation of a right subarachnoid hemorrhage. Subcutaneous osmotic pumps were implanted to deliver sodium nitroprusside (n = 7) or vehicle (n = 6) via catheters into the right basal cisterns.

Assessment of delayed cerebral vasospasm using intracisternal echography--technical note.

BACKGROUND: A new technique using intravascular ultrasound has been used for diagnosis of coronary artery in order to obtain intravascular echo images. In this study, an intracisternally positioned ultrasound catheter was introduced obtaining serial echo images of the first segment (MI) of the middle cerebral artery in order to detect cerebral vasospasm following subarachnoid hemorrhage (SAH). METHODS: Thirteen patients were admitted to Osaka Neurological Institute with SAH due to ruptured intracranial aneurysm. All patients underwent surgical neck clipping on the day of admission. Thermoregulatory response in rats following administration of histamine in different CSF compartments.

Recently histamine is being considered as an important neurotransmitter/neuromodulator in the central nervous system. This has been supported from the present observations with regard to thermoregulatory responses elicited following histamine administration into different CSF compartments. Administration of histamine into the right lateral cerebral ventricle of rats showed a dose dependent fall in rectal temperature. This hypothermic response was evident only at moderately low or at thermoneutral ambient temperature. Administration of histamine into fourth ventricle produced hypothermic respon

Effects of intracisternal glucose or insulin injections on glucose homeostasis in cat.

The injection of glucose (100 mg) into the cisterna magna of intact anesthetized cats elicited immediate glycosuria and natriuresis without significant changes in blood glucose concentration. Immunoreactive insulin (IRI) increased 140% in plasma, and Na+ concentration decreased in cerebrospinal fluid (CSF). After kidney denervation there was a significant decrease in glucose and Na+ concentrations in urine. Control injections with mannitol did not elicit changes in the studied parameters. Abdominal vagotomy abolished the rise in IRI levels and the decrease in Na+ concentration in CSF. Vagotomy…

A further study of the cardiovascular responses to central administration of acetylcholine in rats.

General characteristics and the nature of cardiovascular responses to acetylcholine (ACh) injected into the lateral cerebral ventricle (LCV), the 4th cerebral ventricle (4th CV) and the cisterna magna (CM) were studied. Acetylcholine, depending on the route of injection, the dose and the peripheral vascular tone, produced both increases and decreases in blood pressure and heart rate. After injection of central anticholinesterase agents (physostigmine, neostigmine, armin), the pressor response to ACh was reversed to a depressor response, with or without a secondary pressor response, accompanied…

[CT in normal pressure hydrocephalus--correlation between CT and clinical response to shunting (author's transl)].

CT scans were obtained on 33 patients (age 73y. to 31y.) with the diagnosis of normal pressure hydrocephalus. In each case, the diagnosis was made on the basis of the symptoms, CT and cisternographic findings. Underlying diseases of normal pressure hydrocephalus are ruptured aneurysms (21 cases), arteriovenous malformations (2 cases), head trauma (1 case), cerebrovascular accidents (1 case) and idiopathy (8 cases). Sixteen of 33 patients showed marked improvement, five, moderate or minimal improvement, and twelve, no change. The results were compared with CT findings and clinical response to s

Intracisternal dose-response analysis of 6-hydroxydopamine-induced noradrenergic sprouting in the neonatal rat cerebellum.

The dose-response relationship between neonatal 6-hydroxydopamine and noradrenergic regenerative sprouting in the cerebellum was characterized by using intracisternal doses of 10 to 80 micrograms of the drug. Noradrenergic regeneration was assessed in seven subregions of the cerebellum by measurement of norepinephrine (NE) levels once the rats were one-month-old. Regenerative sprouting occurred to a significant extent at drug doses between 10 to 40 micrograms, with peak NE elevations occurring after a 20-microgram dose. Recovery of NE was not uniform throughout the cerebellum but was maximal i

Miconazole therapy for fungal meningitis.

Eight patients with fungal meningitis (5 with the coccidioidal type, 2 with cryptococcal, and 1 with histoplasmosis) were treated with intravenous (IV) and intrathecal (IT) miconazole after previous therapy with amphotericin B proved unsuccessful. Miconazole was well tolerated with both IV and IT administration. The CSF concentration of miconazole one hour after an IV infusion of 800 mg was 0.1 to 0.3 microgram/ml. When 20 mg of miconazole was administered intrathecally via lumbar injection in patients with coccidioidal meningitis, 6.5, 2.4, 0.77, and 0.24 microgram/ml, respectively, was found

Studies on cyclic AMP in different compartments of cerebrospinal fluid.

Adenosine 3', 5'-monophosphate (cAMP) was measured in the CSF of 42 patients undergoing radiological investigation, neurosurgical procedures, or investigation of hepatic coma. The concentration of cAMP was significantly higher in ventricular CSF than in lumbar CSF. Premedication with pentobarbitone plus promethazine increased cAMP in lumbar CSF. There was no difference in cAMP concentration in lumbar CSF obtained before or after injection of air or after the administration of diazepam during lumbar pneumoencephalography. Lumbar CSF cAMP concentration was significantly increased in patients in

Centrally mediated cardiovascular effects of intracisternal application of carbachol in anesthetized rats.

The pressor response to the intracisternal (i.c.) injection of carbachol (1 mug) in anesthetized rats was analyzed. This response was significantly reduced by the intravenous (i.v.) injection of guanethidine (5 mg), hexamethonium (10 mg) or phentolamine (5 mg), and conversely, potentiated by i.v. desmethylimipramine (0.3 mg), while propranolol (0.5 mg) i.v. selectively inhibited the enlargement of pulse pressure and the tachycardia following i.c. carbachol (1 mug). On the other hand, the pressor response to i.c. carbachol (1 mug) was almost completely blocked by i.c. atropine (3 mug) or hexame

Effect of adenovirus-mediated nitric oxide synthase gene transfer on vasospasm after experimental subarachnoid hemorrhage.

OBJECTIVE: Evidence indicates that vasospasm after subarachnoid hemorrhage (SAH) is caused in part by a decrease in the vasodilator nitric oxide (NO), which is produced mainly in endothelial cells. This study tested whether intracisternal injection of adenovirus-expressing endothelial NO synthase (eNOS) would decrease vasospasm in dogs. METHODS: In 12 dogs, baseline cerebral angiography was performed, and then SAH was produced by two injections of blood into the cisterna magna. The dogs were randomized (n = 6/group) to intracisternal injection of adenovirus-expressing lacZ (Ad327beta-Gal) or …

Gene therapy for Parkinson's disease: trials and technical advances.

Gene therapy has long held promise as a method for targeted alteration of neuronal function in Parkinson's disease. Different gene-therapy approaches aim to correct dysfunctional circuits or have attempted to protect vulnerable neurons to slow disease progression. Clinical trials have used viral vectors to deliver genes either directly into brain regions through stereotaxic injection or globally through infusion into the CSF of the cisterna magna. Bilateral delivery of GAD into the subthalamic nucleus has resulted in some clinical improvements, and the delivery into the putamen of genes that c…

Adeno-Associated Virus-Mediated Dorsal Root Ganglion Toxicity in the New Zealand White Rabbit.

Recombinant adeno-associated virus (AAV)-mediated degeneration of sensory neurons in the dorsal root ganglia (DRG) and trigeminal ganglia (TG) has been observed in non-human primates (NHPs) following intravenous (IV) and intrathecal (IT) delivery. Administration of recombinant AAV encoding a human protein transgene via a single intra-cisterna magna (ICM) injection in New Zealand white rabbits resulted in histopathology changes very similar to NHPs: mononuclear cell infiltration, degeneration/necrosis of sensory neurons, and nerve fiber degeneration of sensory tracts in the spinal cord and of m…
