Plasma exchange in inflammatory demyelinating disorders of the central nervous system: reasonable use in the clinical practice.

Plasma exchange (PLEX) is a therapeutic apheresis modality in which the plasma is separated from inflammatory factors such as circulating autoreactive immunoglobulins, the complement system, and cytokines, and its therapeutic effect is based on the removal of these mediators of pathological processes. Plasma exchange is well established for various neurological disorders, and it is applied successfully in central nervous system inflammatory demyelinating diseases (CNS-IDD). It mainly modulates the humoral immune system; thus, it has a greater theoretical effect in diseases with prominent humor

Effect of Reasons for Screen Failure on Subsequent Treatment Outcomes in Cancer Patients Assessed for Clinical Trials.

INTRODUCTION: The cancer research community increasingly question the rigidity of eligibility criteria in clinical trials. Common reasons for "screen failure" (RFSF) are well documented; however, their effect on subsequent standard therapy (SST) and outcomes is unclear. METHODS: This retrospective study evaluated patients aged ≥18 years with solid malignancy who were listed as ineligible on a screening log between February 2011 and March 2018. Patients screen-failed for biomarker results or incorrect cancer stage/prior treatment profile were excluded. Data were collected from electronic hospi

Automated Insulin Delivery Systems: Impact on Clinical Practice, Discontinuation, Rejection, and Reasons for Not Managing Well.

BACKGROUND: Automated insulin delivery (AID) systems can significantly improve glycemic outcomes in people with type 1 diabetes (PWT1D). Despite their clinical efficacy, little is known about their uptake in clinical care, or about the perspectives and experiences of health care professionals (HCPs) and people with diabetes (PWD) with this relatively new technology.Furthermore, research is limited on broader populations, such as people with type 2 diabetes (PWT2D) and cross-country comparisons. METHODS: This study analyzes data from the dt-report 2025, a multinational online survey of PWD and

Early termination of clinical trials enrolling patients with breast cancer: Reasons, determinants, and temporal patterns.

BACKGROUND: The occurrence of clinical trial termination before planned completion (or "early termination" [ET]) remains poorly characterized in the breast cancer field. This study investigates the reasons, determinants, and temporal patterns of ET in breast cancer trials. METHODS: We conducted a cross-sectional study using the Aggregate Analysis of ClinicalTrials.gov (AACT) database, including breast cancer trials initiated between 2001 and 2025. Reasons for ET were identified and classified into predefined categories. Factors associated with ET were evaluated using multivariable logistic re

Literature review of clinical benefits and reasons to prescribe palliative oxygen therapy in non-hypoxaemic patients.

AIMS:: to review current guidelines and studies available to health professionals in the UK and explore the literature to identify reasons for the prescription of palliative oxygen therapy in non-hypoxaemic patients. BACKGROUND:: oxygen therapy is often associated with the palliative treatment for breathlessness. Although prescription guidelines are available and the risks of oxygen therapy are known, the therapy is still prescribed for non-hypoxaemic patients. DESIGN:: a literature search was conducted using relevant databases. Cited evidence from published guidelines was also consulted. R

Using the theory of reasoned action (TRA) to understand the decision to use condoms in an STD clinic population.

The theory of reasoned action (TRA) provides useful information when designing health education interventions. In this study, 703 heterosexual STD clinic clients responded to a TRA-based survey. With steady partners, social norms and attitudes toward condom use were significant predictors of intention for both men and women. The interaction of attitude and norm increased prediction for men (R = .64, p < 0.001) and women (R = .70, p < 0.001). With casual partners, attitude was a predictor for men and social norm was a predictor for women. Prior use of condoms increased prediction for men (R = .

Disease prevalence among women attending a sexually transmitted disease clinic varies with reason for visit.

Prevalence of sexually transmitted diseases (STD) and selected behavioral and demographic variables were evaluated in 279 women attending a Baltimore STD clinic, using a standardized questionnaire and cultures for Neisseria gonorrhoeae, Chlamydia trachomatis, and Trichomonas vaginalis. Stratified by reason for clinic visit, 102 (37%) of 279 women attending the clinic stated that they were recent contacts to men with STDs with the majority (59 out of 102, or 58%) reporting gonorrhea contact as their reason for visit. Another 124 women (44%) came to the clinic for symptom evaluation, and 53 (19%

Co-designing a student-led digital assessment resource (SCAR-V) to enhance clinical reasoning in undergraduate nursing education: A qualitative study.

BACKGROUND: Developing clinical reasoning and assessment literacy is central to undergraduate nursing education, yet students often struggle to demonstrate higher-level reasoning in written assessments. This study explored how co-design, guided by the Double Diamond framework and Interpretive Descriptive analysis, could inform the creation of a digital resource to support this progression. OBJECTIVES: To examine students' and educators' experiences of assessment and learning across Levels 5 and 6, and to co-design a peer-led digital resource to enhance clarity, authenticity, and inclusion in …

Decomposing Persona Prompts for Simulated Clinical Reasoning: A Two-by-Two Factorial In Silico Experiment of Time Pressure and Safety Prioritization.

BACKGROUND: Persona prompting is widely used to steer large language models (LLMs), but its effects on safety-critical clinical reasoning are not well characterized. METHODS: We performed a two-by-two factorial in silico experiment crossing time-pressure framing (high versus low) with optimization target (safety-first versus lean-efficiency). We used 28 Japanese-language synthetic emergency department vignettes covering chest pain, abdominal pain, headache, and dyspnea. Four trap cases contained prespecified contraindication or sequencing rules. Each persona evaluated each vignette twice, yie…

Identifying and prioritizing educational content from a malpractice claims database for clinical reasoning education in the vocational training of general practitioners.

Diagnostic reasoning is an important topic in General Practitioners' (GPs) vocational training. Interestingly, research has paid little attention to the content of the cases used in clinical reasoning education. Malpractice claims of diagnostic errors represent cases that impact patients and that reflect potential knowledge gaps and contextual factors. With this study, we aimed to identify and prioritize educational content from a malpractice claims database in order to improve clinical reasoning education in GP training. With input from various experts in clinical reasoning and diagnostic err…

Clinical reasoning underlying acute care occupational therapists' assessment of rehabilitation potential after stroke or brain injury: A constructivist grounded theory study.

INTRODUCTION: In acute care hospitals, clinicians are expected to rapidly provide recommendations regarding patients' rehabilitation potential and candidacy for postacute rehabilitation. Some studies have investigated factors influencing referral to rehabilitation, but few have examined clinical reasoning underlying referral decisions. This study aimed to investigate what occupational therapists were thinking about (factors influencing reasoning), how they reasoned (thought processes) when evaluating stroke or traumatic brain injury patients' rehabilitation potential, and how they decided on r…

A clinical decision support system for the diagnosis of probable migraine and probable tension-type headache based on case-based reasoning.

BACKGROUND: The overlap between probable migraine (PM) and probable tension-type headache (PTTH) often confuses physicians in clinical practice. Although clinical decision support systems (CDSSs) have been proven to be helpful in the diagnosis of primary headaches, the existing guideline-based headache disorder CDSSs do not perform adequately due to this overlapping issue. Thus, in this study, a CDSS based on case-based reasoning (CBR) was developed in order to solve this problem. METHODS: First, a case library consisting of 676 clinical cases, 56.95% of which had been diagnosed with PM and 4

Evaluation of large Language model performance on Persian rheumatology board exams: accuracy and clinical reasoning of GPT-4o vs. GPT-5.1.

Large language models are increasingly integrated into medical education, yet their performance in non-English clinical examinations, particularly Persian, remains limited. This study evaluated how GPT-4o and GPT-5.1 perform on Iranian Rheumatology Board examination questions. A total of 204 multiple-choice items were administered in Persian using a similar prompt. Accuracy was determined using the official answer key, and six board-certified rheumatologists independently scored each model’s clinical reasoning on a 1–5 scale. GPT-5.1 demonstrated markedly superior performance, achieving 76% ac…

Clinical Reasoning in the Absence of a Protocol: A Case Series of Physical Therapy Following Orthobiologic Interventions for Chronic Musculoskeletal Conditions.

Physical therapy following orthobiologic interventions occurs in a guidance vacuum, with limited standardized rehabilitation protocols to direct clinical decision-making; in this context, clinical reasoning becomes the operative mechanism through which therapists navigate the uncertainty inherent in postprocedural care. This retrospective case series documents and describes the clinical reasoning processes employed by a physical therapist managing three patients with chronic musculoskeletal conditions following orthobiologic interventions, without evaluating the efficacy of the orthobiologic o…
