Median Nerve Entrapment in the Ulna in a Both-Bone Pediatric Forearm Fracture: Case Report and Review of the Literature.

This case report presents entrapment with subsequent complete disruption of the median nerve in the distal ulna in a both-bone fracture of the forearm in a 9-year-old girl. Closed fractures of the radius and ulna commonly occur in pediatric populations. Postinjury nerve dysfunction is often seen. Severe nerve injury is rare. There are only 3 reports of the median nerve becoming entrapped in an ulnar fracture in a child, with no reports of entrapment in the distal forearm. Features seen on examination and the radiographs supported possible nerve entrapment. Although uncommon, nerve entrapment o

Traumatic dislocation and fracture dislocation of the hip. A long-term follow-up study.

Of 84 fracture dislocations of the hip reviewed, 73 were posterior fracture dislocations, nine were central, and two were obturator. Fifty-five were followed from one to 30 years (average, 4 1/2 years). The patients' ages ranged from three to 83 years (average, 33.1 years). All had primary attempt at closed reduction, with 15 treated with subsequent open reduction (six immediate and nine delayed). Satisfactory results were obtained in 42% of hips treated with closed reduction and 40% of those treated with open reduction. Statistical analysis of the results was done. A stable closed reduction p

Reduction Techniques for Trochantericand Subtrochanteric Fractures of the Femur: a Practical Guide.

Trochanteric intramedullary nailing has gained widespread acceptance and popularity among orthopedic trauma surgeons. Whereas some simple fracture patterns are easily reduced and nailed, others may present a major challenge for the surgeon. Anatomical reduction and optimal placement of the intramedullary implants are the most important factors for fracture healing and good functional outcome. Closed anatomical reduction is to be achieved before the nail is inserted. However, especially in inter- and subtrochanteric fractures, a limited open or even open reduction technique may be necessary to

A simple technique to repair a hand fracture in a captive Siamang.

A 5-year-old female Siamang (Hylobates syndactilus) in captivity sustained a displaced closed fracture of the proximal phalanx of the middle digit, resulting in finger deformity and hand functional disability. Anatomical reduction and stabilization of the fracture were obtained surgically using two multifilament wire sutures. The primate had a successful functional recovery.

Operative treatment of fractures about the knee.

It should be emphasized that most fractures about the knee in children and adolescents can be treated by closed methods. It follows that open reduction and fixation is employed when closed reduction fails. An interposed flap of torn periosteum may prevent closed reduction. Internal fixation is often required in avulsion fractures of the tibial tubercle, reduced displaced fracture-separations of the proximal tibial epiphysis, fracture-separations of the distal femoral epiphysis displaced in the sagittal plane, and displaced Salter-Harris type III and IV fracture-separations. A displaced avulsio

Reduction of angulated metacarpal fractures with a custom fracture-brace.

A thermoplastic fracture-brace was compared to closed reduction with ulnar gutter splinting in the treatment of 38 isolated fractures of the fifth metacarpal with angulation more than 40 degrees at the neck or 25 degrees in the diaphysis. Patient profiles, fracture patterns, compliance rates, and time to clinical union were similar in the two treatment groups. The two treatment methods improved fracture angulation by the same amount, to about one half of the initial angulation. At the time of splint or brace removal, there was more restriction of wrist, metacarpophalangeal, and proximal interp

Safety and Efficacy of Using Fracture Tables for Prosthetic Hip Dislocations.

The incidence of prosthetic hip dislocation continues to increase because of the overall increase in volume of total hip replacement surgery. Closed reduction is often the preferred treatment, particularly in the first few months after surgery. No matter the closed reduction technique, linear traction is a requirement, thus posing a physically demanding stress opening both surgeon and patient to potential injury. We describe a fracture table closed reduction technique along with outcomes and safety data for a sample of patients. In all 10 reduction procedures, reduction was achieved quickly an

A closed phalangeal neck fracture with 180-degree rotation of the phalangeal head and concurrent displaced Salter-Harris II fracture.

Closed type III phalangeal neck fractures with 180-degree rotation is a rare iatrogenic injury that occurs following failed attempts at closed reduction. Prior to closed reduction, the phalangeal head is in 90-degree rotation. Longitudinal traction during closed reduction then converts the deformity into 180-degree rotation. We present the first documented noniatrogenic case of phalangeal neck fracture with 180-degree rotation that was also associated with a displaced Salter-Harris II fracture at the same joint. The blood supply of the phalangeal head in that scenario is discussed along with p

Open reduction by vestibular approach in the treatment of segmental alveolar fracture.

Fracture of the alveolar process is a common injury; the majority of alveolar fractures may be managed by closed reduction. However, some cases with severe segmental alveolar fracture cannot be reduced by close reduction, usually resulting in occlusion disturbance. This article describes open reduction by vestibular approach in the treatment of severe segmental alveolar fractures, with the aim of evaluating the prognosis. Fifteen patients with severe segmental alveolar fractures that could not be reduced by closed method were included in our case series. Open reduction by vestibular approach w

Complex Nasal Fractures in the Adult-A Changing Management Philosophy.

Acute management of complex nasal fractures in the adult nose is still frequently conducted using closed reduction techniques as first-line therapy. Treatment outcomes from closed reduction are often disappointing and secondary surgical corrections are required in a sizeable subset of patients. In response to the shortcomings of closed fracture manipulation, classic rhinoplasty techniques have been introduced to improve anatomic fracture reduction. Although these techniques improve the accuracy of skeletal reduction, they also weaken the nose, leaving it susceptible to the deformational forces

Outcomes of Intramedullary Nailing with Cerclage Wiring in Subtrochanteric Femoral Fractures.

AIMS: The aim of this study was to compare the outcomes of closed reduction against open reduction with cerclage wires in patients with subtrochanteric fractures treated with intramedullary nailing (IMN). MATERIALS AND METHODS: We identified 141 patients who had an IMN over a 4-year period. They were classified into three groups based on fracture pattern and whether open or closed reduction was performed. Type I was a transverse fracture, type II, a spiral fracture with an intact posterior and medial wall in the proximal fragment, and a type III fracture without intact posterior or medial wal

A treatment strategy to avoid iatrogenic Pipkin type III femoral head fracture-dislocations.

INTRODUCTION: Reduction is urgently required in cases of traumatic hip dislocation to decrease the risk of avascular necrosis of the femoral head. However, successful reduction may not always be feasible for hip dislocations associated with femoral head fractures. This irreducibility may provoke further incidental fractures of the femoral neck with resultant Pipkin type III injuries. The purpose of this study was to describe an appropriate treatment strategy for irreducible femoral head fracture-dislocations. MATERIALS AND METHODS: We treated nine patients with irreducible hip dislocations wi

Closed versus open treatment of mandibular condylar process fractures: A meta-analysis of retrospective and prospective studies.

INTRODUCTION: The treatment of fractures of the mandibular process remains controversial, although there is a trend towards open reduction and internal fixation. This study compared open and closed treatments and assessed the results with a meta-analysis. MATERIALS AND METHODS: A literature search of PubMed found eight studies that met the search criteria and were included in the meta-analysis. RESULTS: The studies increasingly suggest better results for open treatment, in terms of mouth opening, protrusion, laterotrusion, pain, and malocclusion. In the meta-analysis, the outcome was signifi

Posterior fracture-dislocations of the hip; long-term follow-up.

Of 242 posterior fracture-dislocations of the hip, of which 150 had been followed for from four to twenty-seven years (over-all average, seven years and three months), 112 were treated by closed reduction; and fifty-six, by primary open reduction. Satisfactory results were obtained in 12 per cent of the hips treated by closed reduction, in 42 per cent of those treated by closed reduction followed by open reduction, and in 63 per cent of those treated by primary open reduction with removal of all loose fragments of bone and cartilage and restoration of stability by internal fixation of the frac

Closed Management of a Distal Radius Refracture Through a Bent Volar Plate: Can an Open Procedure be Avoided?: A Case Report.

CASE: A 55-year-old man sustained a distal radius fracture, which was fixed with a volar plate 20 years ago. He then fell from 2 m and sustained a refracture with a bent but intact volar plate. A manipulation under anaesthetic/anaesthesia was attempted. Satisfactory reduction was confirmed and maintained, with bony union confirmed at 6 months. Regular follow-up showed continuing functional improvement. CONCLUSION: This rare case avoided surgical intervention and its associated risks and costs. Attempted closed reduction can be considered a treatment option before open revision fixation, with

Irreducible Salter Harris type II distal tibial physeal fracture secondary to interposition of the posterior tibial tendon: a case report.

Pediatric distal tibial fractures generally occur without significant long-term sequelae, and patients are commonly able to return to their preinjury activities after proper management. The literature reports excellent outcomes after anatomical reduction of distal tibial and ankle physeal fractures with closed or open treatment. Treatment options include simple immobilization of nondisplaced fractures, and closed or open reduction for restoration of anatomic alignment of displaced fractures. Soft tissue interposition within the fracture can threaten successful closed reduction, and may warrant

A Salter-Harris II Distal Radius Fracture Irreducible by Closed Methods.

CASE: A 10-year-old boy presented with a Salter-Harris II distal radius fracture that was irreducible by closed methods. An open reduction was performed in the operating room where a sleeve of periosteum was found interposed between the fracture fragments. Successful reduction was performed without difficulty after the periosteum was removed from the fracture. CONCLUSION: Soft-tissue interposition must be kept in mind when having difficulty performing closed reductions of pediatric distal radius physeal fractures to avoid excessive reduction attempts.
