Nutritional status and food intake of HCV/HIV coinfected patients.

INTRODUCTION: Hepatitis C virus (HCV) and human immunodeficiency virus (HIV) coinfection may cause nutrient deficiency and affect the nutritional status.Objetive: To assess nutritional status, and energy and macronutrient intake in HCV/HIV coinfected patients. METHODS: Cross-sectional study on HIV/HCV-coinfected patients treated in a public hospital. Nutritional status was assessed by measurements of weight, height, waist circumference (WC), arm circumference (AC), triceps skinfold thickness (TST), non-dominant hand gripltrength (NDHGS), body mass index (BMI) and mid-upper arm circumference (…

Daclatasvir plus Sofosbuvir for HCV in Patients Coinfected with HIV-1.

BACKGROUND: The combination of daclatasvir, a hepatitis C virus (HCV) NS5A inhibitor, and the NS5B inhibitor sofosbuvir has shown efficacy in patients with HCV monoinfection. Data are lacking on the efficacy and safety of this combination in patients coinfected with human immunodeficiency virus type 1 (HIV-1). METHODS: This was an open-label study involving 151 patients who had not received HCV treatment and 52 previously treated patients, all of whom were coinfected with HIV-1. Previously untreated patients were randomly assigned in a 2:1 ratio to receive either 12 weeks or 8 weeks of daclat

Genome scale patterns of recombination between coinfecting vaccinia viruses.

UNLABELLED: Recombination plays a critical role in virus evolution. It helps avoid genetic decline and creates novel phenotypes. This promot … DNA and correlated with increased plaque size. The recombinants produced through a single round of coinfection contain a disproportionate number of short conversion tracks (<1 kbp) and exhibited 1 exchange per 12 kbp, close to the ∼1 per 8 kbp in the literature. One by-product of this study was that rare mutations were also detected; VACV replication produce …

[Aids and tuberculosis: coinfection from the perspective of the quality of life of patients].

Tuberculosis (TB) and HIV coinfection adversely affects the lives of individuals in both the biological and psychosocial aspects. Aiming to describe the quality of life of individuals with HIV/TB coinfection, this descriptive cross-sectional study was conducted in Ribeirão Preto-SP. Participants were HIV-seropositive individuals with and without TB, using the WHOQOL HIV BREF. 115 individuals who were HIV-positive participated: 57 were coinfected and 58 were not; most were male heterosexuals, predominantly aged 40-49 years. Of those coinfected, most had lower education and income. In assessing …

Coinfections associated with human immunodeficiency virus infection: workshop 1A.

BACKGROUND: When asked, a significant number of patients report having experienced night sweats. Those who do are more likely to report decreased physical health, mental health, and quality of life. In most cases the cause of night sweats is unknown. We therefore do not know how much to worry about patients with this symptom. The present study examined associations between night sweats and mortality. METHODS: We used logistic regression and proportional hazards analyses to investigate potential predictors of mortality, including night sweats reported at baseline, among 2 different cohorts of

Simultaneous HAART improves survival in children coinfected with HIV and TB.

OBJECTIVES: This study assesses the outcome of current treatment guidelines and the effect of highly active antiretroviral therapy (HAART) on survival of HIV/TB-coinfected patients in a resource-limited setting. METHODS: Observational cohort study at the pediatric HIV Clinic, RML Hospital, Delhi. All HIV-infected patients who visited the clinic for the diagnosis of TB between 2002 and 2006 were observed until 31 March 2010. TB was diagnosed either at the time of enrolment or during follow-up visits. Clinical and epidemiological data were registered. We compared children who were given HAART w

STD coinfections in The Netherlands: Specific sexual networks at highest risk.

BACKGROUND: Specific subpopulations infected with multiple bacterial sexually transmitted diseases (STDs) may facilitate ongoing STD transmission. To identify these subpopulations we determined the extent of concurrent incident STD infections and their risk factors among the high-risk population seen at Dutch STD clinics. METHODS: STD surveillance data submitted routinely by STD clinics to the National Institute for Public Health on demographics, sexual behavior, STD testing, and diagnoses for the period 2004-2007 were analyzed. RESULTS: Bacterial STD coinfections were diagnosed concurrently…

[A case of HIV coinfected with hepatitis B virus treated by entecavir].

A 45-year-old male active homosexual was given a diagnosis of HIV-1 and acute hepatitis B in August 2007. Since his liver function became rapidly impaired, anti-HBV therapy with oral administration of entecavir (ETV) was started, and resulted in a favorable outcome. However, serum concentration of HIV-RNA decreased by log 1.26 within 60 days, which strongly suggested the inhibition of HIV proliferation by ETV. To prevent the appearance of mutated HIV, novel therapeutic strategies should be established in HIV/HBV-coinfected patients.

Cutaneous manifestation of disseminated strongyloidiasis in a patient coinfected with HTLV-I.

Strongyloidiasis is a potentially lethal parasitic infection. Coinfection of a patient with human T-lymphotropic virus type I (HTLV-I) can lead to a more severe disease course and treatment-refractoriness. Here we report a patient coinfected with HTLV-I and Strongyloides stercoralis who developed disseminated, treatment-resistant disease. The patient presented with serpiginous, nonpalpable, purpuric streaks on the abdomen and proximal lower extremities. A biopsy of this eruption demonstrating filariform larvae in the dermis was consistent with disseminated strongyloidiasis. The patient's immun…

Syphilis in the United States: an update for clinicians with an emphasis on HIV coinfection.

Diagnosis and treatment of syphilis are challenging because of its variable clinical presentation and course and the lack of definitive tests of cure after treatment. This review of the most recent literature on the epidemiology, clinical manifestations, current diagnosis, and treatment of syphilis is focused toward clinicians who treat patients with this disease. Syphilis coinfection with human immunodeficiency virus is emphasized because it is increasingly common in the United States and affects the initial presentation, disease course, diagnosis, and treatment of syphilis. Of particular con…

Lipid abnormalities in HIV/hepatitis C virus-coinfected patients.

BACKGROUND: Among HIV-infected patients, hepatitis C virus (HCV) coinfection is associated with increased rates of lipodystrophy and insulin resistance. Its impact on HIV-associated dyslipidaemia is less clear. METHODS: The lipid profiles of all HIV-infected patients and a subset of HCV-infected patients seen at the VA Medical Center in Dallas from January 2003 to March 2004 were analysed. Demographic data, HCV serostatus, and HIV treatment history were recorded. Lipid profiles of HIV/HCV-coinfected patients were compared with those of HIV-monoinfected and HCV-monoinfected patients. RESULTS:…

HCV/HIV-coinfection--is there a state of the art after APRICOT and RIBAVIC?

Today hepatitis C is one of the unsolved medical problems and results in a significant number of deaths in the HIV population, particularly in southern Europe and the USA, where the prevalence of HCV/HIV-coinfection in the HIV population ranges from 30-50%. Recent trials using pegylated interferon alfa in combination with low dose ribavirin have achieved overall sustained response rates of up to 42%. However, high discontinuation rates, adverse events associated with mitochondrial toxicities or treatment of patients with advanced cirrhosis may decrease response to treatment and result in serio…

Clinical challenges and controversies in the management of HIV/ HCV-coinfected individuals.

The natural history of HIV infection has been dramatically changed by the highly active antiretroviral therapies, reducing complications, morbidity and mortality of the disease. Approximately 25% of persons infected with HIV are co-infected with hepatitis C, and some high risk populations have a prevalence of HCV of more than 75%. Liver disease has become one of the principal causes of morbidity and mortality in this population. Co-infection increases viremia of hepatitis C, with increase in fibrosis progression, cirrhosis and death related to hepatitis C. The permanent state of chronic immune

Gonorrhea prevalence and coinfection with chlamydia in women in the United States, 2000.

BACKGROUND: No recent national data address the prevalence of gonorrhea. GOAL: The goal was to describe gonorrhea prevalence and chlamydial coinfection among women aged 15 to 24 years. STUDY DESIGN: Data were analyzed from tests for chlamydia and gonorrhea at family planning, STD, and prenatal clinics in 2000. Gonorrhea positivity, chlamydia positivity among women with gonorrhea, and the median and interquartile ranges (IQRs) were calculated. RESULTS: The median state-specific gonorrhea positivity among women aged 15 to 24 years was 0.9% (IQR, 0.7%-1.7%) in family planning clinics, 7.0% (IQ…

Coinfecting deer-associated zoonoses: Lyme disease, babesiosis, and ehrlichiosis.

The heightened worldwide recognition of the health burden of tickborne infection derives largely from the increasing incidence of Lyme disease, human babesiosis, and human granulocytic ehrlichiosis, both individually and in concert. Because these infections share the same rodent reservoir and tick vector hosts, they can be cotransmitted to human hosts. Indeed, human coinfections involving various combinations of these pathogens are common, and some tend to be particularly severe. Diagnostic procedures and clinical management of the resulting disease syndrome is rendered complex by the diversit…

Assessment and management of persons coinfected with tuberculosis and human immunodeficiency virus.

After eight decades of decline, cases of tuberculosis (TB) have increased substantially since 1985. This increase is attributed largely to a reactivation of disease in persons who are coinfected with TB and human immunodeficiency virus (HIV). Multiple-drug resistant tuberculosis (MDR-TB) has a high mortality rate (70 to 90%), mostly occurring in individuals with HIV. Death often results within 4 to 16 weeks of diagnosis. The pathogenesis, screening, chemoprophylaxis, diagnosis, and treatment of persons coinfected TB and HIV is reviewed. Attention is given to infection control and occupational
